Hepaticocholecystojejunostomy as a simple technique for decompression of the biliary tract.

A technique for decompressing the biliary tree obstructed by tumors in the upper part of the bile duct is described. It involves draining intrahepatic bile ducts through the gallbladder to a loop of the jejunum. The problem of bleeding is obviated by compression of the crushed liver by the balloon of a Foley catheter which also forms a temporary drain. Only two simple anastomoses are involved and complications are avoided if due attention is paid to the anatomy of the bed of the gallbladder.

C controlled trial of taurolin in established bacterial peritonitis.

Taurolin administered intraperitoneally has effected a significant reduction in morbidity in peritonitis even when all other methods of intensive care, including powerful antibiotics, have been used. Easy to use clinically, it is effective locally against all fecal pathogens and is nontoxic.

The in vitro and in vivo activity of taurolin against anaerobic pathogenic organisms.

An antibacterial agent, taurolin, formed by the combination of 2 molecules of taurine with 3 molecules of formaldehyde has been tested against anaerobes in vitro and in vivo. In vitro, taurolin is highly active against Bacteroides species and anaerobic Streptococci as measured by the minimum inhibitory concentration. In vivo, small dosages of taurolin given intraperitoneally increase the mortality in mice with Bacteroides peritonitis. However, by increasing the dosage of taurolin, survival is prolonged and mortality reduced. Intravenously administered taurolin protects rabbits from the effects of Bacteroides fragilis infection whether the organisms are given intravenously or intraperitoneally.

Animal models of peritonitis.

A reproducible animal model of peritonitis has been developed. It uses standard animals, standardized cultures of bacteria and simple techniques. Use of this model would facilitate comparison of data from workers in this field. The use of small rodents permits the use of comparatively large populations for statistical evaluation. The method is cheap and economic of laboratory space and does not require surgical expertise.

Secondary carcinoma of the thyroid gland.

Two patients are described who presented with clinically solitary thyroid nodules which were found to be due to metastatic disease from the kidney and colon respectively. Removal of the solitary metastatis in one patient resulted in disease-free survival for over 5 years.
