Epithelial ingrowth of anterior chamber and anterior surface of vitreous.

This study is a case report of the histopathologic findings of the anterior chamber epithelial ingrowth in a patient who had penetrating injury in the right eye from an arrow approximately 20 years ago. The patient underwent the enucleation in the right eye due to pthisis bulbi and was fitted with a prosthetic eye. Specimens were prepared from the enucleated right eye for histopathologic observation using hematoxyllin-eosin to be observed under light microscopy. Epithelial ingrowth in the anterior chamber was noted in one layer or multi-layered epithelial cell growth. The ingrowth had spread to the posterior surface of the cornea to the anterior chamber angle, to the iris surface, and to the anterior surface of the vitreous. The finding suggests that epithelial ingrowth could invade even through a perforation site and spread wherever the cells could reach.

Peripapillary circle of Zinn-Haller revealed by fundus fluorescein angiography.

AIMS: To observe the vascular pattern of the peripapillary circle of Zinn-Haller in humans by fundus fluorescein angiography. METHODS: 307 cases (from 212 patients) of fundus fluorescein angiograms performed in patients with myopic degeneration were evaluated to find the circle of Zinn-Haller and to observe its fundus fluorescein angiographic features. RESULTS: 15 cases (from 13 patients) with the circle of Zinn-Haller were found. It appeared as concentric or zigzag-shaped vascular fillings within the temporal crescent region. All cases were observed in pathological myopia with peripapillary atrophy and a tilted disc. Each arterial circle showed variations in location and shape. CONCLUSIONS: The temporal part of the circle of Zinn-Haller can be revealed by fundus fluorescein angiography particularly in pathological eyes with prominent peripapillary atrophy and a tilted disc. The morphological variation of this arterial circle should be considered.

Infectivity of basal epithelial cells in herpetic dendritic epithelial keratitis.

In patients with herpes virus dendritic epithelial keratitis, the pathologic morphogenesis of the dendritic lesion was investigated by means of impression cytology. Impression specimens were taken from the lesions of 24 consecutive patients assessed with a slit lamp biomicroscope. After a Millipore membrane impression was stained with hematoxylin-eosin and examined by light microscope, cellular patterns were analyzed. The results of these examinations showed that most cells were epithelial cells; there were also occasional multinucleated syncytial giant cells, inclusion bodies and inflammatory cells. Basal epithelial cells were most prominent at the actively infected lesion, whereas superficial epithelial cells were shown at the margin of the active lesion and terminal bulb. These findings suggest that in dendritic keratitis, viral spread and replication in the dendritic keratitis may progress through basal epithelial cells along the nerve within the basal epithelium-stromal interface.

Increased numbers of Langerhans cell and expression of HLA-Dr antigen in the giant papilla of patients with giant papillary conjunctivitis.

A study of histopathologic changes, ultrastructure, and expression of the HLA-Dr antigen within the giant papillae of patients with giant papillary conjunctivitis was performed to determine whether cell-mediated immune response is related to this condition. Conjunctival giant papillae from ten patients with giant papillary conjunctivitis were examined by light and electron microscopy and by the indirect immunofluorescent staining method with HLA-Dr antibody. The infiltration of eosinophilic neutrophils and granules was most prominent, with the occasional infiltration of mast cells, as shown by light microscopy. The infiltration of activated fibroblasts and Langerhans cells was also observed. Cells expressing HLA-Dr antigen were also markedly increased, as shown by the immunofluorescent method. These findings suggest that delayed hypersensitivity may, along with the processes of antigen presentation by HLA-Dr-expressing (including Langerhans) cells, contribute to the pathogenesis of giant papillary conjunctivitis.

Morphological changes of the sensory retina following the occlusion of ciliary arteries in rabbits.

The whole retina, except for the medullary fiber zone in a rabbit eye, is supplied by choroidal circulation. Therefore, the histopathological changes of the sensory retina due to choroidal circulatory disturbance in rabbits may be comparable to that of the human sensory retina in the case of ophthalmic artery occlusion. This study was carried out to evaluate the histopathological changes of the ischemic retina secondary to the occlusion of choroidal circulation. The experimental occlusion of all posterior ciliary arteries and anterior ciliary arteries in the horizontal rectus muscle of rabbit eyes was performed and the subsequent histopathological changes of the sensory retina were observed by transmission electron microscopy. The morphological changes of the sensory retina following the occlusion of the ciliary arterial system are as follows: severe loss of the inner and outer segments of the photoreceptor, mild to moderate degeneration of the ganglion cells, and excellent preservation of the Müller's cell fibers and the extension of the cytoplasmic villous processes to the cytoplasmic vacuolar spaces of other degenerated cells. These findings indicate that the Müller's fibers in the ischemic condition of retina might contribute to the formation of gliosis or scarring of a damaged retina.

Ultrastructural change of the Müller cell in the culture of sensory retina.

This study was performed to investigate the sequential changes of the retinal tissue in tissue culture condition. The human sensory retinal tissues were cultured for up to 2 weeks and 4 weeks, respectively. The initial changes showed the separation of the intercellular space and the consequent widening of the intercellular space with prolapse of cytoplasmic processes into the widened intercellular space. The internal limiting membrane was also separated from the inner retina, which led to the prolapse of the cytoplasm of the Müller cell. The growth of the Müller cell was most prominent during the 4-weeks' tissue culture period. These findings suggest that the Müller cell might contribute to the formation of cellular membrane in case of the defect of the internal limiting membrane in several pathologic conditions.

Schrimer test in Parkinson's disease.

We carried out the Schirmer test to measure objectively the amount of lacrimation among 51 clinically diagnosed parkinsonian patients (33 men and 18 women aged 50 to 79 years, mean 64) and 75 age-matched controls (42 men and 33 women aged 50 to 76, mean 62). Whatman No. 2 paper, prepared in precut strips 5mm by 35mm, was placed in the cul-de-sac for five minutes, after which the wetted length of the strip was studied. It was noted that the lacrimation amount decreased in patients with Parkinson's disease compared with controls: the average amount of lacrimation was 3.4 +/- 2.3mm in the former group and 8.1 +/- 6.5mm in the latter group (p < 0.01). We believe that the decrease in the amount of lacrimation is associated with emotional disturbance and autonomic dysfunction, and presume that the lacrimation may be under the control of the basal ganglia which has a connection with the superior salivatory nucleus downward and the limbic system upward.

Infrared measurements of HF and HCl total column abundances above Kitt Peak, 1977-1990: seasonal cycles, long-term increases, and comparisons with model calculations.

Series of high-resolution (approximately 0.01 cm-1) solar absorption spectra recorded with the McMath Fourier transform spectrometer on Kitt Peak (altitude 2.09 km, 31.9 degrees N, 111.6 degrees W) have been analyzed to deduce total column amounts of HF on 93 different days and HCl on 35 different days between May 1977 and June 1990. The results are based on the analysis of the HF and H35Cl (1-0) vibration-rotation band R(1) lines which are located at 4038.9625 and 2925.8970 cm-1, respectively. All of the data were analyzed using a multilayer, nonlinear least squares spectral fitting procedure and a consistent set of spectroscopic line parameters. The results indicate a rapid increase in total HF and a more gradual increase in total HCl with both trends superimposed on short-term variability. In addition, the total columns of both gases undergo a seasonal cycle with an early spring maximum and an early fall minimum, with peak-to-peak amplitudes equal to 25% for HF and 13% for HCl. In the case of HF, the changes over the 13 years of measurement are sufficiently large to determine that a better fit is obtained assuming a linear rather than an exponential increase with time. For HCl, linear and exponential models fit the data equally well. Referenced to calendar year 1981.0 and assuming a sinusoidal seasonal cycle superimposed on a linear total column increase with time, HF and HCl increase rates of (10.9 +/- 1.1)% yr-1 and (5.1 +/- 0.7)% yr-1 and total columns of (3.17 +/- 0.11) x 10(14) and (1.92 +/- 0.06) x 10(15) molecules cm-2 (2 sigma) are derived, respectively; the corresponding best fit mean exponential increase rates are equal to (7.6 +/- 0.6)% yr-1 and (4.2 +/- 0.5)% yr-1 (2 sigma). Over the 13-year observing period, the HF and HCl total columns increased by factors of 3.2 and 1.8, respectively. Based on HF and HCl total columns deduced from measurements on the same day, the HF/HCl total columns ratio increased from 0.14 in May 1977 to 0.23 in June 1990. Short-term temporal variations in the HF and HCl total columns are highly correlated; these fluctuations are believed to be caused by dynamical variability in the lower stratosphere. The results of this investigation are compared with previously reported measurements and with time-dependent, two-dimensional model calculations of HF and HCl total columns based on emission histories and photo-oxidation rates for the source molecules.

Post-traumatic hyperlipofuscinosis in the human retinal pigment epithelium.

Light microscopy (including fluorescence microscopy) and electron microscopy were applied to a study of the photoreceptor-retinal pigment epithelium (RPE) complex in a human eye which had been severely traumatised nine months prior to enucleation. The main feature of interest was a massive accumulation of lipofuscin in the retinal pigment epithelium at the posterior pole, and quantitative fluorescence microscopy provided values three times those obtained in appropriate control tissue. The photoreceptor layer was normal at the posterior pole but became progressively atrophic towards the periphery. The concentration of lipopofuscin was proportional to the degree of preservation of the retinal photoreceptors. By electron microscopy the cells in the RPE were seen to be packed with a mixture of lipofuscin granules and melanolysosomal complexes, but occasional photoreceptor phagosomes were found. Bruch's membrane and the choriocapillaris were normal. We attribute this hitherto unreported abnormality of the RPE after trauma to a dysfunction consequent on an overload of the monolayer by photoreceptor debris at the time of trauma.

Fibronectin in the trabecular meshwork: immunofluorescent and immunoelectron microscopic findings.

We examined the fibronectin (FN) secretion of cultured trabecular meshwork (TM) cells in a normal human eye by indirect immunofluorescent technique using mouse anti-human FN monoclonal antibody and FITC-conjugated goat anti-mouse IgG. To localize FN on frozen sections of normal TM, which were obtained from 7 enucleated eyes owing to traumatic eyeball rupture, the same indirect immunofluorescent method was used. Immunoelectron microscopy was applied to demonstrate the distribution pattern of FN in the normal TM of 2 human eyes using an avidin-biotin-peroxidase complex method. In the tissue culture of TM, the TM cell walls and extracellular matrices showed an intense staining with antibody to FN. Indirect immunofluorescent staining of FN on frozen sections of TM showed strong positive reactions in the subendothelial region. There was no reaction in the central core of the trabecular beam. Immunoelectron microscopy revealed the reaction products to FN in the areas lining the trabecular endothelial cells.

Ultrastructure and blood-iris barrier in experimental rubeosis iridis in rabbit.

Iris neovascularization was produced in rabbits by hypotony following repeated aspiration of the vitreous. The hypotony was produced after 0.3 ml of vitreous fluid was aspirated using a 25-gauge needle through the pars plana of 10 rabbits. For the histochemical study, horseradish peroxidase(HRP) was injected through the ear lobe vein. After fixation of the iris tissue, the tissue was treated with diaminobenzidine and examined with both light microscopy and transmission electron microscopy. The newly-formed vessel was abundant, particularly on the upper stroma of the iris. The new vessel formation was evident due to the proliferation of endothelial cells, which may have been derived from preexisting iris vessels. The endothelial cells of the newly-formed vessels revealed prominent villous processes into the vascular lumen, formation of the marginal flap, numerous fenestrations in the endothelial junction, and reaction product onto extravascular space by the cytochemical electron microscopy. These results suggest that hypotony in the rabbit produces the disruption of the blood-iris barrier and the balance between angiogenesis-antiangiogenesis modulation.

Outflow of aqueous humor following cyclodialysis or ciliochoroidal detachment in rabbit.

Cyclodialysis and ciliochoroidal detachment were performed in three eyes of three rabbits and in three eyes of another three rabbits, respectively. After aspiration of the aqueous humor, 0.1 ml of 10% sodium fluorescein was injected intracamerally, and the eyeball was enucleated between 30 minutes and one hour after injection and prepared for fluorescence microscopy. Sodium fluorescein concentrations in the supraciliary space were much greater in the group with cyclodialysis or ciliochoroidal detachment than in the normal control group. These results suggest that (1) in the eye with cyclodialysis, the aqueous humor may freely gain access to the supraciliary space through the cleft between the anterior chamber and the supraciliary space and then be removed rapidly and (2) in the eye with ciliochoroidal detachment, the aqueous humor may pass through the uveoscleral outflow pathway.

Anterior uveal neurilemmoma--a rare neoplasm simulating malignant melanoma.

A 30-year-old woman presented with dilated episcleral vessels in the right eye which were found to be associated with an underlying ciliary body tumour. Contact lens examination, transpupillary transillumination, ultrasonography, computerised tomography, magnetic resonance imaging, and fine needle aspiration biopsy produced apparently conflicting and inconclusive results, and the eye was enucleated. Light and electron microscopy showed the lesion to be a neurilemmoma (schwannoma), of which very few cases have been reported. The clinical and pathological features are described and discussed.

Hyaloid vascular pattern in the human fetus.

The hyaloid vascular system of developing human eyeballs is studied to elucidate the relationship to the retinal vascular patterns. The process of the regression of the hyaloid vascular system in fetuses consisted of gradual shrinkage of the wall of the vessels with lessening of the lumen and eventually, thread-like acellular strands that remained as a consequence. The intercapillary bridge in the retinal vessel appeared to be a remnant of the degenerated vessel as in the hyaloid vascular system. At the ninth month of intra-uterine life, the regression of the hyaloid vascular system was almost complete.
