Spectrum of benign breast disorders in a university hospital.

In a period of 2 years, 234 cases of benign breast disorder were studied. Breast pain and modularity was the commonest group (70.1%) followed by fibroadenoma (17.5%). Cyclical mastalgia (61.5%) is more common than non-cyclical mastalgia (38.5%). The age of the patients with cyclical mastalgia was significantly lesser than patients with non-cyclical mastalgia. Cyclical mastalgia was seen only in premenopausal females while non-cyclical mastalgia was also seen in postmenopausal females. Treatment with vitamin E showed 41% response rate with minimal side-effects while treatment with danazol showed 72.1% response rate but was associated with side-effects in one third of the patients.

Simplified sutured sacral rectopexy for complete rectal prolapse in adults.

OBJECTIVE: To assess the results of simplified sutured sacral rectopexy for complete rectal prolapse. DESIGN: Prospective study. SETTING: University hospital, India. SUBJECT: 65 patients with complete rectal prolapse treated by modified sutured sacral rectopexy. MAIN OUTCOME MEASURES: Morbidity, mortality and recurrence. RESULTS: During a follow-up of 2-10 years (mean 5.4 years) none of the patients developed a recurrence of complete prolapse of the rectum but 3(5%) developed mucosal prolapse. The incontinence improved in 12 of 16 patients (75%) and constipation improved in 5 of 6 (83.3%). CONCLUSION: We conclude that the simplified sutured sacral rectopexy is a simple operation for treating complete rectal prolapse in adults, and gives good results.

Extraperitonealization for sigmoid volvulus: a reappraisal.

A prospective study was carried out in a university hospital to evaluate the results of extraperitonealization of the sigmoid colon in non-gangrenous sigmoid volvulus. Eighty-eight patients with non-gangrenous sigmoid volvulus were operated upon using the technique of extraperitonealization of the sigmoid colon. There was no recurrence of the volvulus at 1 to 6 years follow up but 8 patients (9.1%) had constipation and 2 patients (2.2%) had distension of the lower abdomen. Extraperitonealization of the sigmoid colon is a reasonable non-resection alternative for non-gangrenous sigmoid volvulus and has minimal morbidity, mortality and no recurrence in our series.

Serum malate dehydrogenase (MDH) in portal hypertension--its value as a diagnostic and prognostic indicator.

Serum MDH levels were estimated in 29 consecutive patients of portal hypertension with the objective of ascertaining its diagnostic value as to the cause of portal hypertension and its prognostic significance. Higher levels of serum MDH were found in cirrhotic patients as compared to noncirrhotic ones. There were however no significant differences found in the values when extrahepatic obstruction group was compared with controls. Furthermore, higher MDH activity was found in those patients who died as compared to the surviving ones.

Fine-needle aspiration cytology of abdominal masses.

An investigation of the role of blind fine-needle aspiration cytology (FNAC) in the assessment of palpable abdominal masses was carried out on 196 patients: 124 hepatic, 30 retroperitoneal, and 42 other masses. All of the smears were stained either by Papanicolaou stain or by hematoxylin and eosin stain. The results of FNAC were confirmed by further investigations in all cases. FNAC correctly diagnosed 166 (84.6%) cases. Twenty (10.2%) reports were false negative, and 10 (5.1%) smears were unsatisfactory for any diagnosis. Of 124 hepatic masses, the correct diagnosis was obtained in 106 (85.4%), false-negative reports in 14 (11.2%), and unsatisfactory smears in 4 (3.2%). There were no false-positive reports. For all the lesions, the sensitivity of FNAC was 87.8%; specificity, 100%; positive predictive value, 100%; and negative predictive value, 52.4%. Five (2.5%) patients had considerable pain after the procedure.

Mitomycin C extravasation ulcers.

In a four-year period, eight patients with mitomycin C extravasation ulcers were encountered. Mitomycin C extravasation produces a painful indolent ulcer that does not have any tendency to heal. If extravasation of the drug is recognized, infusion should be stopped immediately, and the site of infusion should be changed. The ulcers should be excised, and primary closure is recommended; if it is not possible then the defect is covered by a partial thickness skin graft.

Typhoid perforation of the gut.

One-hundred consecutive patients with typhoid perforation of the gut admitted in the same surgical unit of the University Hospital have been studied. The cases were diagnosed on the basis of history, clinical examination, exploratory findings, histopathological examination, Widal test and blood culture. Forty-six patients had perforation in the second week of fever. Sixty-one patients presented 48-96 hr after perforation. All the patients were subjected to surgery, 16 under local anesthesia. Mortality rate increased from 25% to 83% as the duration between perforation and operation increased.

Adolescent

Canine shock: irreversibility and coagulopathy.

Canine shock was experimentally induced by "superior mesenteric artery (SMA) occlusion followed by release:" two hours of occlusion produced reversible shock while four hours of occlusion caused irreversible shock. In both situations, the animals showed coagulopathy but the magnitude of same was more severe in animals with irreversible shock. The nature of coagulation changes was suggestive of intravascular coagulation. Histological changes showed widespread intravascular fibrin thrombi in the liver and the kidney in animals with irreversible shock and only limited local intravascular coagulation in the subcapsular region of the liver in those with reversible shock. The importance of diffuse intravascular coagulation in the irreversibility of shock was also corroborated by the "reversion" of shock by prior heparinization.
