Synchronous balneophototherapy is effective for the different clinical types of psoriasis.

BACKGROUND: The efficacy of synchronous balneophototherapy in clearing psoriasis is based on the multiple-targeted effects of UVB light and Dead Sea salt. Their synchronous application produces a synergic effect. OBJECTIVE: The purpose of this retrospective study is: 1) to evaluate the efficacy of synchronous balneophototherapy for treating different clinical types of psoriasis; 2) to determine whether there is any difference between response to treatment, and 3) to gain more data in order to predict the effect of treatment in different clinical types, and thus to support the selection of patients for treatment. METHODS: Patients received a basic course of synchronous balneophototherapy according to the Regensburg scheme, consisting of 35 treatment sessions, followed by a maintenance course of a further 25 treatments. The patients' skin status was monitored by weekly assessment using the PASI score throughout the course. The efficacy of the treatment was evaluated through the results of 373 patients treated according to protocol during the basic course, and the results of 78 of these patients during the maintenance course. One hundred and eighty-six patients were enrolled into the study comparing the efficacy of the basic course for the different clinical types of psoriasis: data of patients with large plaques, small plaques, guttate and confluating type of skin signs were summarized and compared. RESULTS: During the basic course of treatment 70.7% improvement of the average PASI index was observed; the average PASI index decreased from 16.14 to 4.73. A further improvement from 4.58 to 4.27 of the average PASI was found during the maintenance therapy. Small plaque-type skin signs showed the best response with a PASI decrease of 76.1%; Guttate type had a PASI decrease of 73.7%, large plaque type, 67.1% and confluating type, 62%. Comparing data with the average PASI decrease, a statistically significant lower decrease was found in confluating type cases. CONCLUSIONS: These results confirm that synchronous balneophototherapy is an effective treatment modality for different clinical types of psoriasis. Patients with small plaques have the greatest chance of the most marked clinical clearing; guttate and large plaque types of psoriasis also respond well to the treatment.

[Gardner syndrome].

A syndrome going with skin-bone signs and precancerous gastrointestinal polyposis is presented. Interest of our case is given by the extension of polyposis onto the whole digestive canal, early appearances of abdominal signs accompanying polyposis, and as a new symptom polycystic lung and extended superficial naevus lipomatosus.

[Surfen allergy causing insulin depot S hypersensitivity].

Allergic cutaneous reaction developed in 3 patients upon the use of Insulin-depot-S-Richter injections. Allergological examinations confirmed the eliciting role of surfen, the material being responsible for the depot effect. The patients were treated thereafter with surfen-free insulin. The authors deal also with the side-effects of insulin treatment as well as with certain forms of insulin allergy.

[Anti-androgen treatment of hirsutism with cyproterone acetate].

22 female patients suffering from idiopathic hirsutism were treated according to Hammerstein's reversed sequential scheme over 12 months. During each treatment cycle of 21 days, the daily medication consisted of 1 coated tablet containing 0.035 mg ethinyl estradiol and 2 mg cyproterone acetate (Diane-35); in addition, the patients received one i.m. injection of 300 mg cyproterone acetate (Androcur-Depot) on the first day of each cycle. This therapy resulted in improvement of the disease in 19 of our patients (86.4%). The transitory, mild side effects observed did not require interruption of the treatment. The reversed sequential scheme making use of the depot form of CPA can successfully be administered in patients with hirsutism.

[Gardner syndrome].

A case of Gardner syndrome associated with alterations of the skin and bones and precancerous gastrointestinal polyposis is presented. Particular interest attached to the extension of polyposis to the entire digestive tract, the early appearance of abdominal signs attributable to the polyposis, and - as new symptoms - polycystic lung and widespread superficial naevus lipomatosus.

Bone-scintigraphic examinations in patients treated with retinoids: a prospective study.

Possible early side-effects of retinoid treatment on bones were studied with the aid of bone-scintigraphy. Isotretinoin treatment (1.0 mg/kg daily for 4 months) was given to 18 patients with acne. Fifteen patients with psoriasis received etretinate treatment (0.7-1.0 mg/kg/day) for 4 months. In the group treated with isotretinoin, pathological uptake of radiolabel was found in three cases, while in the group treated with etretinate, no bone changes attributable to treatment were found. During isotretinoin treatment, a decrease in growth-plate activity was observed. Bone-scintigraphy is considered to be a suitable method for early screening for bone changes occurring in retinoid treatment.

On the anti-inflammatory effect of sendai virus inoculation.

Sendai virus given intravenously into CFLP mice produced dose-related suppression of the acute inflammation produced by carrageenan and 5-HT. The inhibition correlated with interferon levels in the blood, suggesting that interferon may account for the virus-induced anti-inflammatory effect.

The effect of anti-lymphocyte and anti-thymocyte serum on the pathogenesis of Rauscher leukaemia in inbred mouse strains.

Rauscher virus causes in inbred Balb/c mice a rapidly progressing hyperacute, in C57B1/10Sn mice an incipient, spontaneously healing leukaemia. In DBA/1 and DBA/2 mice the appearance of leukaemia is followed by a partial remissions then by an exacerbation of the disease. The infection in C57B1/10Sn, DBA/1 and DBA/2 mice results in a significant tumour-specific immune response. Inhibition of the immune response is followed by an increased progression of leukaemia in DBA/1 and DBA/2 mice only. It is assumed that in C57B1/10Sn mice the remission of incipient leukaemia is associated with a resistance determined in the target cells, whereas in DBA/1 and DBA/2 mice the remission is due to a tumour-specific immune response. As in animals treated with anti-lymphocyte and anti-thymocyte serum the course of the disease runs proportionally to the degree of the inhibition of the immune response, the tumour-specific antibodies play a decisive role in the elimination of the tumour cells. In Balb/c, DBA/1 and DBA/2 mouse strains failing to exhibit a spontaneous reversion of the tumour cells, the appearance of a significant tumour-specific immune response depends on the resistance against the helper component of the Rauscher virus complex.

Immunochemical examination of soluble antigens in the serum of Balb/c mice infected with Rauscher leukaemia virus.

The serum of Balb/c mice infected with Rauscher leukaemia virus contained soluble antigens characterized by alpha2 and beta globulin electrophoretic mobility; their respective molecular weights, as determined with gel filtration, were 40 000 and 120 000. The antigens differed in specificity and their corresponding determinants were present on the surface of leukaemic cells. For Balb/c mice both antigens, for DBA/1 and C57B1/10Sn mice only the antigen showing alpha2 mobility was immunogenic.

Immune response to rauscher virus-induced leukemia in DBA mice. I. Role of cellular and humoral immunity in spontaneous regression.

DBA/1 and DBA/2 mice infected with the Rauscher leukemia virus developed a biphasic erythroleukemia. Transitory regression of the disease was closely associated with the appearance of tumor-specific antibodies and the exacerbation was preceded by the gradual decrease of antibody titer. The antibody-dependent cellular cytotoxicity could be detected earlier, than the complement-dependent cytotoxicity. Moreover, in each case the titer of antibody-dependent cellular cytotoxicity was higher than that of complement-dependent cytotoxicity. The results suggest that the antibody-dependent cellular cytotoxicity is mainly responsible for the rejection of tumor cells.

Lipid composition of the mouse spleen in Rauscher leukaemia.

Lipid constituents were examined in the spleen of mice 10, 14 and 18 days after infection with Rauscher leukaemia virus. (i) The total lipid content, as related to the amount of protein, decreased with the development of the tumour. (ii) Spleen cells of control and infected mice synthesized the same lipids. (iii) In the infected spleen, free fatty acids increased by 80% and triglycerides decreased by 44% as compared to the control. The percentage distribution of phospholipid components showed a change in tumour cells: lecithin and phosphatidylethanolamine increased, lysolecithin and sphingomyelin decreased. (iv) After intraperitoneal injection of [32P] orthophosphate, in infected mice the incorporation of 32P into phosphatidylcholine and phosphatidylethanolamine increased, while 32P incorporation into lysophosphatidylcholine + sphingomyelin and phosphatidylinositol + phosphatidylserine fractions decreased.

Spermatological investigations in patients treated with etretinate and isotretinoin.

Possible side effects of the retinoids etretinate and isotretinoin on the germinal epithelium were examined. 15 men received etretinate and 13 isotretinoin. After 16 weeks of treatment at therapeutic doses, no changes were observed in fundamental spermatological parameters. On the basis of examinations and data in the literature, we hold the opinion that retinoid treatment is to be considered safe from the andrological viewpoint.
