[Total duodenal diversion in the treatment of complex peptic esophagitis].

AIMS OF THE STUDY: The aims of this study was to report the results of total duodenal diversion in patients with complex peptic esophagitis (peptic stenosis, acquired short esophagus, columnar lined esophagus, previous surgery). PATIENTS-METHODS: Total duodenal diversion has been performed in 107 patients with complex peptic esophagitis. The standard procedure--including a troncular vagotomy, an antrectomy and a 70 cm Roux-en-Y gastro-jejunostomy--was used in 68 cases (64%). Technical adjustments were necessary in the 39 others patients. RESULTS: Two patients (1.8%) died postoperatively. Permanent healing of esophagitis was observed within 3 months in 88% of patients. Esophagitis healed in all patients operated with the standard technique. Three hours postprandial pH-monitoring was normal postoperatively in 92% of patients. Four anastomotic ulcers occurred in patients who did not have vagotomy. Among patients with columnar lined esophagus, one complete and six partial regressions were observed; no malignant degeneration was observed with a 210-patient-year follow-up. Among the 39 peptic stenoses, all except one (2.6%) resolved. Functional disorders occurred in 27% of patients within the first postoperative months; these disorders persisted in 14% of patients (Visick III or IV) after 3 years. The main disorders (dumping syndrome, anastomotic ulcer, diarrhea) were observed when a two-thirds distal gastrectomy has been performed to avoid the dangerous completion of vagotomy after a previous Heller's myotomy. CONCLUSION: These results suggest that total duodenal diversion is a suitable treatment of complex peptic esophagitis.

Laparoscopic fenestration in polycystic liver disease.

Sixteen laparoscopic cyst fenestrations for symptomatic adult polycystic liver disease (APLD) were performed in 13 women, including four patients who had had previous attempts at treatment (percutaneous sclerotherapy in two and fenestration via a laparotomy in two). The median number of cysts deroofed was 32 (range 18-58) during the 13 primary procedures. There was no in-hospital death. Postoperative transient ascites occurred in six patients. After operation 11 patients experienced immediate relief of symptoms but during a median follow-up of 26 (range 6-49) months, eight of these patients developed recurrent symptoms. Two patients underwent three repeat laparoscopic fenestrations, at which time perihepatic adhesions were rare and no complication occurred. Because repeat procedures may be performed, laparoscopic fenestration appears to be useful for the treatment of symptomatic APLD. However, it is less effective than fenestration at open surgery or liver resection and should be employed only in patients with predominantly large cysts.

[Total duodenal diversion in the treatment of complex peptic esophagitis].

Two duodenal diversion was performed in 107 patients with complex peptic oesophagitis (peptic stenosis, Barrett's mucosa, past history of gastro oesophageal surgery). A standard operation included truncular vagotomy, antrectomy and gastro-jejunal anastomosis on a 70 cm Y loop in 68 patients. Technical adaptations were required in 39 patients. Two patient died (pulmonary embolism and duodenal fistula). The operation was successful with stable cure of the oesophagitis at 3 months in 89% of the patients. Post-prandial pHmetry over 3 hours confirmed control of the reflux in 92% of the cases. Anastomotic ulcer occurred in 4 patients who did not have a vagotomy. One complete regression of Barret's oesophagitis was achieved and in 6 other cases the regression was partial. Stenosis improved in all patients except 1, sometimes after 1 or several dilatations. Digestive sequellae, were observed during the first few months after surgery in 27% of the cases. Persistant sequellae were found in 14% of the operation patients after a delay of 36 months. These results suggest that duodenal diversion is a useful treatment for complicated and complex peptic oesophagitis.

Drainage after elective hepatic resection. A randomized trial.

OBJECTIVE: This prospective randomized study determined the influence of closed-suction drainage on the incidence of postoperative complications after elective hepatic resection. SUMMARY BACKGROUND DATA: Routine drainage is no longer advocated after several intra-abdominal surgical procedures. METHODS: A series of 81 patients who underwent elective hepatic resection were randomly allocated to either a nondrainage group (n = 39) and a drainage group with closed-suction drainage (n = 42). Indications for resection were 42 benign lesions and 39 malignant tumors, including 19 with cirrhosis. Major hepatic resection was performed in 25 patients and minor resection, in 56. All patients underwent ultrasonography with puncture for bacteriologic cultures of all fluid collections within the first 5 postoperative days. RESULTS: One patient died in each group. Ultrasonography found a significantly higher rate of subphrenic collections in the drainage group compared with the nondrainage group (respectively, 36% vs. 15%, p < 0.05). These collections were more frequently infected in the drainage group (n = 6) than in the nondrainage group (n = 2). After major liver resection, the rate of intra-abdominal postoperative complications (i.e., subphrenic fluid collections, hematomas, and bilomas) was similar between the two groups. CONCLUSIONS: Minor liver resection is safer without drainage. Major liver resection can be performed with or without abdominal drainage.

[Gastroesophageal reflux treated by posterior hemifundoplication. 251 cases].

Between September 1983 and March 1991, 251 consecutive patients with gastro-oesophageal reflux resistant to medical treatment underwent posterior hemifundoplication (modified Toupet procedure). One hundred and seventy-seven patients (71 percent) had peptic oesophagitis. pH monitoring showed a mean Kaye's score of 278 +/- 245 with a 29 percent part of total recording time at pH < 4. The mean low oesophageal sphincter pressure was 8.5 +/- 6.5 cm H2O. No patient died in the postoperative period. Morbidity consisted of 8 splenic injuries, as well as 8 pulmonary and 23 thromboembolic complications. Assessment of 199 patients (79 percent) with a mean follow-up of 32 +/- 21 months showed complete symptomatic relief in 96.5 percent, and complete endoscopic healing of oesophagitis was noted in 96 percent. Restoration of the pH profile to normal levels was obtained in 86 percent of the cases. The mean low oesophageal sphincter pressure had risen to 17 +/- 6 cm H2O. Early postoperative dysphagia was noted in 46 patients (18 percent); one of them required reoperation. Reflux symptoms persisted in 9 patients (4.5 percent). pH monitoring revealed abnormal levels in 3 patients. The results of this study demonstrate that effective gastro-oesophageal reflux control can be achieved with the modified Toupet procedure.

[Complications of peroperative sterilization of hydatid cysts of the liver. Apropos of 6 cases].

Injection of scolicidal solution into the hydatid cysts has been used to sterilize the cyst and to prevent intra-abdominal dissemination of the parasite during surgery. We report six cases of complications of this measure. Intraoperative collapse in one patient and an immediate postoperative death occurring after injection of 10% H202. Postoperative sclerosing cholangitis occurred in 4 patients in whom 2% formalin or 20% sodium chloride was injected into the cyst. Cholangiography showed strictures affecting the intrahepatic biliary tree in 2 and both the intra and extra biliary tree in 3. Sclerosing cholangitis in these patients was likely to result from the caustic effect of the scolicidal solution having diffused from the cyst into the biliary tree. Jaundice developed in 3 patients from 2 to 5 months after operation. The intra-operative collapses is related to the injection of H202 into the cyst. The dramatic increasing of the volume of the cyst may fissure of the cyst-wall and allows the passage of gaz into the circulation. As the efficacity of intracystic injection of a scolicidal solution in preventing the dissemination of the parasite is still unproven, we recommend the rejection of this maneuver in the surgical treatment of hydatid disease of the liver.

[Adenocarcinoma of the duodenum. 17 cases].

Seventeen cases of duodenal adenocarcinoma are reported and compared with cases from the literature. Ten tumours were located in the second part of the duodenum, 9 were around the hepatopancreatic ampulla, 4 were above and 4 below this ampulla. Four tumours (23 per cent) had limited intramural expansion (Dukes' stage A or B) and 6 (35 per cent) involved the lymph nodes. The major clinical signs were loss of weight (11 cases), signs of obstruction (9 cases), bleeding (6 cases) and jaundice (6 cases). The paraclinical examinations that were determinant for the diagnosis were gastroduodenal radiographic study (16 cases) and endoscopy with biopsy (9 cases). Computerized tomography was useful for preoperative evaluation. The respective values of these examinations for early diagnosis and excisability of these tumors are discussed. All 17 patients were operated upon: 11 (64.7 per cent) underwent radical excision, 5 had duodenal bypass and 1 had exploratory laparotomy. The overall operative mortality rate was 5.8 per cent, with no deaths for bypasses and excisions. Mean survival after bypass was 5 1/2 months. The one year survival was 63.5 per cent in 5 patients with excision and 83.3 per cent in 6 patients with cephalic duodenopancreatectomy, but none of these patients survived for more than 5 years.
