[Low birth-weight neonates in Tunisia (author's transl)].

From a prospective analysis performed in a maternity-hospital in Tunis, incidence, characteristic features and etiologic factors responsible for birth-weight below 2,5000 g have been studied. Incidence of low birth-weight is accepted as an indicator of the level of health care in a country; this incidence, established from 6,028 living births was 7.2%, similar to that in developed countries. On the contrary, intra-uterine growth retardation rates of 69.6% and prematures rates of 30.4% are closer to those in developing countries. Altogether, anomalies of height and of the height/head circumference ratio were present in 36 children only and, in 12 out the these, abnormalities of the placenta or membranes or evidence of fetal disease were found. A factor responsible for low birth weight was found in 182 neonates: previous severe disease in the mother (7.7%), toxemia (9.4%) twin pregnancy (22.41%), fetal pathology (3.5%) and pathology of the placenta and membranes (4.6%).

[Clinical aspects and course of Adamantiades-Behçet disease in Tunisia: a report on 41 cases (author's transl)].

The clinical aspects and course of Behçet's disease in Tunisia are discussed. This affection is of frequent occurrence and should be systematically searched for in all patients with uveitis. Ocular manifestations are observed in 95 p. cent of cases. These are characterized by their polymorphic nature and the diagnosis should be suspected in cases with uveitis, periphlebitis, venous thrombosis, vitreous hemorrhage, congestive glaucoma, or macular edema. The characteristic signs of the disease appear over an extended period of time, making definite diagnosis difficult, but early, vigorous therapy is necessary to ensure a favorable prognosis.

[Outcome of corrected and uncorrected tricuspid insufficiency after repair with prostheses in mitral and mitro-aortic valve diseases. Apropos of 159 cases].

This is a study of 159 patients with tricuspid incompetence (TI) associated with a mitral or mitro-aortic valve defect. The mean age of the patients at operation was 25.5 years. The TI was left untreated in 84, corrected by semi-circular annuloplasty in 39, and corrected by a prothesis in 28, by a Carpentier ring in 6, and by Kay's plasty in 2. The 33 early deaths (21%) are regrettable. The long-term outlook was studied with a mean follow-up period of 26 months. The overall mortality rises, if these are included, to 42 (26%). By comparing the preoperative findings in those with good and poor results, we have been able to identify a certain number of risk factors: a preoperative course of greater than or equal to 6 years, a number of attacks of cardiac failure greater than or equal to 3, a cardiothoracic ratio greater than or equal to 0.70, a mean right atrial pressure greater than 12, a mean pulmonary arterial pressure of greater than or equal to 50. Severe TI is also a poor prognostic factor. If the three groups of patients are compared, bearing in mind the fact that their preoperative state was very similar, and that the TI was either left untreated or repaired by annuloplasty or prosthesis, it appears that such a correction improves neither the surgical mortality nor the long-term prognosis. It therefore seems to us that a functional TI can be treated expectantly since its resolution depends more on the unified treatment of the whole underlying disorder and on the state of the myocardium than on any localised correction of the TI.

Erythrophagocytic T-cell lymphoma: immunologic and cytogenetic evidence for a clonal malignancy of the CD8-positive T-cell subclass.

We report a case of an erythrophagocytic T-cell lymphoma with autoimmune hemolytic anemia, osteolytic lesions, hypercalcemia, hepatomegaly, and marrow invasion without evident lymph node, skin or peripheral blood involvement. The malignant cells were large immunoblastic cells with pleomorphic nuclei and occasional phagocytic vacuoles containing erythrocytes. Immunologic studies showed the tumor cells to have the phenotype of activated T-cells of the cytotoxic/suppressor subclass (HLA- Dr-positive, CD8 positive) with surface receptors for IgG Fc but lacking functional activity in assays of natural killer activity and antibody-dependent cytotoxicity. The tumor cells appeared to express receptors for T-cell growth factor (Tac). Cytogenetic study of marrow mononuclear cells revealed complex but non-random karyotypic abnormalities (45 XX, +11, -12, -16, 17 p+). The clinical and laboratory features indicate that erythrophagocytic T-cell lymphoma represents a true clonal malignancy of the CD8-positive subclass of T-lymphocytes.
