[Prosthetic valve function evaluated by ultrafast computed tomography (Imatron C-100)].

Ultrafast computed tomography (UFCT) is a new diagnostic modality that includes not only imaging of cardiac structures, but also movement of cardiac muscles and valves. It can be useful in the evaluation of prosthetic valves, because the scan time is extremely short and whole cardiac structures are imaged with a slice thickness of 8 mm and within 7 cardiac cycles. We review our experience using UFCT to observe prosthetic valve function in 22 cases with various valvular diseases.

[Visualization of the female breast in bone scintigraphy].

Breast cancer incidence rates in Japan have more than doubled in the decade between 1975 and 1985 due to a well-established and affluent Western life-style. One hundred patients having breast cancer, 107 patients having mastectomies, and 243 patients having no breast cancer were reviewed for whole body and spot views of bone scintigraphy with 99mTc-MDP. The frequency of positive breast uptake in patients with no breast cancer showed a maximum in those patients between the ages of 30 and 40, with a progressive rise and fall before and after the peak, respectively. Patients between the ages of 50 and 60 with breast cancer showed a maximum uptake of 83%, while those patients between the ages of 70 and 80 with breast cancer showed an uptake higher than 50%. There was a markedly higher breast uptake ratio in the breast cancer group compared to the breast cancer free group. Although this scanning agent was of no value in recognition of tumor localization from breast uptake, we have to suspect a high incidence of cancer existence whenever there was unilateral increased breast activity. Even though the mechanism of 99mTc-MDP uptake by breast tissue is unknown, breast concentration of 99mTc-MDP appears to be influenced by hormones, especially estrogen.

[Hepatocellular carcinoma associated with schistosomiasis japonica; CT and angiographic features].

One-hundred and fourteen cases of hepatocellular carcinoma (HCC) which underwent computed tomography (CT) and angiography at the University Hospital of Yamanashi were reviewed. These included 24 cases of HCC associated with hepatic schistosomiasis japonica. Compared with the other cases, radiological features of HCC with schistosomiasis japonica were clarified. Patients with multinodular HCC were most common (13/24), followed by those with single nodular HCC (7/24), while those with massive or diffuse HCC were few (4/24). In patients with marked CT changes (grade III or IV) of hepatic schistosomiasis japonica, most of HCC nodules were disclosed as a mass with homogenous hypodensity surrounded by "shell-like calcifications". Obstruction of the portal vein or its branches due to tumor thrombus was rarely noted (4/24). These features are presumably attributed to periportal fibrosis which is characteristic of heptic schistosomiasis japonica. Because of these features, it is expected that hepatocellular carcinomas with schistosomiasis japonica can be treated by transcatheter arterial embolization more effectively than those without schistosomiasis japonica.

[N-isopropyl-p-[123I]iodoamphetamine early dynamic image with ring-type emission CT in cerebrovascular disease].

N-isopropyl-p-[123I]iodoamphetamine (IMP) SPECT was done in 14 patients with cerebrovascular disease. Immediately after injection of 3 mCi IMP, sequential 2 minutes SPECT images (dynamic images) were obtained for 15 minutes using a circular detector array emission tomographic system (Shimadzu SET-030W). Then standard SPECT images were obtained at 30 minutes (early images) and 5 hours (delayed images) using a rotating scintillation camera ECT system (GCA-601E, Toshiba). Two patients out of 14 studies showed discrepancy of the findings between dynamic images and early images; dynamic images revealed low uptake area though early images showed no defect. Possible mechanism of this phenomenon might be due to so called redistribution phenomenon occurred during obtaining early images. In conclusion, early dynamic images is useful in cases with slightly decreased cerebral blood flow.

Hepatocellular carcinoma: metastatic abdominal lymph nodes identified by computed tomography.

Out of 79 consecutive cases of hepatocellular carcinoma (HCC) examined with computed tomography (CT), five patients revealed metastatic swollen abdominal lymph nodes. Involved were portal, retropancreatic, common hepatic, superior mesenteric, and paraaortic nodes. CT appeared to be an excellent modality in the diagnosis of metastatic abdominal lymph nodes of HCC.

Intraluminal papillary carcinoma of the gallbladder: prognostic value of computed tomography and sonography.

Twenty-seven consecutive cases with a prospective diagnosis of cancer of the gallbladder were analyzed to clarify the computed tomographic (CT) and ultrasonographic features, behavior, and prognosis of intraluminal papillary carcinoma (n = 7) of the organ. With CT and ultrasonography, these intraluminal carcinomas were further classified into 3 subtypes: single polyp, multiple polyp, and jam-packed gallbladder. Preoperative diagnosis, evaluation of local invasiveness, and detection of metastatic lesions were reliably done. Carcinomas of this type were locally less invasive, less frequently metastasized, and resulted in a longer survival than the other massive and thickened-wall carcinomas.

[Ga-67 scintigraphy for the localization and staging of malignant lymphoma].

Thirty-eight Ga-67 scintigraphies have been evaluated in regard to patients with an untreated malignant lymphoma or a recurrent lymphoma after a complete remission. Thirty-four studies (89%) showed positive findings. Waldeyer's ring and lymph node involvement were detected in 66% of the cases. The recurrent type was detected less often than the untreated malignant lymphoma, and there was no significant difference in detectability between Hodgkin's disease and non-Hodgkin's lymphoma. Although the malignancy grade for NHL did not affect the detectability, the intermediate and high grade NHLs tended to show a greater uptake than the low grade NHLs. The detectability was particularly good for neck and intrathoracic lymph nodes. Ga-67 scintigraphy tended to downgrade a malignant lymphoma to a lower stage, especially if in recurrence.

Exophytic regenerating nodule of the liver: misleading appearance on iodized-oil CT.

Iodized-oil CT in a patient with chronic liver disease revealed focal accumulation of iodized oil in an exophytic mass. Single photon emission CT (SPECT) with [99mTc]phytate showed the existence of functioning Kupffer cells, excluding hepatocellular carcinoma. Biopsy revealed a regenerating nodule. Positive iodized-oil CT does not always indicate hepatoma. In such cases, SPECT with [99mTc]phytate will help to make a correct diagnosis.

A case of intracranial involvement of Hodgkin's disease.

We report a rare case of Hodgkin's disease in which intracranial involvement developed during the course of the patients illness. A 20-year-old man who had complained of lymph node swelling on the right neck was admitted to a hospital in December 1978. Lymph node biopsy revealed Hodgkin's disease, and he was treated by various series of chemotherapy and radiotherapy with unsatisfactory results. He was transferred to Yamanashi Medical College Hospital in June 1985. He was in a far-advanced state at the time, and palliative treatment was applied. In the middle of May 1986, he complained of headache, tinnitus, and sleeplessness. Vomiting and tremor were observed by the end of May 1986. Brain CT scan revealed a space occupying lesion in the right temporal region. Whole brain irradiation of 45 Gy was effective, and the lesion disappeared. However, his general condition deteriorated and he died in November 1986. Brain autopsy could not be performed.
