The authors participated in the European multicenter investigation, ESPRIT, organized by the Wellcome Research Laboratories. Thrombolytic treatment by intravenous tissue plasminogen activator was performed in 25 patients with early (less than 6h) myocardial infarction. The efficacy of the treatment was controlled by repeat coronary arteriography at 60 minutes, at 90 minutes and at 24 hours of the tpA treatment. The infarct related artery was reperfused in 9/25 patients at 60 minutes, in 16/25 at 90 minutes and 17/18 at 24 hours. Four patients died after unsuccessful treatment or reocclusion. In two patients significant bleeding occurred at the puncture site but no transfusion was required. No other untoward effect was registered. The left ventricular function did not change significantly during the first day of infarction. It is concluded, that tpA is a safe thrombolytic agent in myocardial infarction. Its thrombolytic efficacy is similar to that of streptokinase.

Seven of 214 patients (3%) with acute myocardial infarction (120 inferior and 94 anterior) developed atrial fibrillation within 3 hr of the onset of chest pain. All seven patients had an inferior infarction and in all seven the left circumflex artery was occluded proximal to the origin of its left atrial circumflex branch. In five patients this occlusion was acute and was the cause of inferior infarction and in the remaining two patients the occlusion was old and the inferior infarction was due to an acute occlusion of the right coronary artery that also supplied extensive collaterals to the previously occluded left circumflex artery. All seven patients also had impaired perfusion to the atrioventricular nodal artery, as evidenced by total occlusion proximal to its origin or by stenosis proximal to its origin associated with second- or third-degree atrioventricular block. In contrast, early atrial fibrillation did not occur in any of the 18 patients with inferior myocardial infarction due to acute occlusion of the distal left circumflex artery or in any of the five patients with inferior infarction due to acute occlusion of the proximal left circumflex artery if perfusion to the atrioventricular nodal artery was not impaired. Early atrial fibrillation did not occur in any of the 90 patients with inferior infarction due to acute occlusion of the right coronary artery, including 12 patients with occlusion proximal to the sinus nodal artery, but without coexistent occlusion of the left circumflex artery.(ABSTRACT TRUNCATED AT 250 WORDS)

The case of a 34-year-old female patient is presented. During attempted left anterior descending coronary angioplasty, the exchange guidewire (0.018 inch, 300 cm) snapped and the distal 20 cm of the wire caused immediate thrombotic occlusion of both the left anterior descending and circumflex arteries. The resulting complication was treated by emergency bypass surgery.
Out of 11,000 cardiac catheterizations in adult patients, perforation of the heart occurred in four cases (0.036%). Left atrial wall perforation by an NIH catheter occurred in one patient without sequelae. In three patients, right ventricular perforation by a Zucker-type catheter was observed, causing pericardial tamponade. This observation may discourage angiographers from the routine use of this type of catheter.

A 48-year-old man sustained an inferior myocardial infarction resulting in a left ventricular aneurysm, ventricular septal defect and mitral regurgitation. Aneurysmectomy, closure of the ventricular septal defect, mitral valve replacement and coronary artery bypass were successfully performed.
Serum total cholesterol (TC), high density lipoprotein cholesterol (HDL-C), very low density lipoprotein cholesterol (VLDL-C) and total triglyceride levels were determined in children with high risk arteriosclerotic family history. Significantly higher TC and lower HDL-C levels were found in children whose parents' first arteriosclerotic sign had appeared before 40 years of age. There were no similar significant alterations observed in children whose parents' first arteriosclerotic symptom appeared after the forties. Screening therefore seems to be necessary in the offsprings of patients if the first sign of arteriosclerosis has been detected before 40 years of age.
Left ventriculography and coronary arteriography were performed in 47 patients with hyperacute myocardial infarction prior to recanalization of the infarct-related vessel. Mitral regurgitation was found in ten patients. After successful recanalization, left ventriculography was repeated in eight of the ten patients with mitral incompetence, and the mitral regurgitation had disappeared in seven. Selective intracoronary thrombolysis resulted in improved left ventricular function and disappearance of mitral incompetence.

The case of a young female patient with inferior infarction is presented. In the fifth hour after the onset of MI coronary angiography revealed complete obstruction of the right coronary artery. This obstruction has been recanalized by selective intracoronary infusion of streptokinase. At the site of occlusion there remained a 90% coronary stenosis, causing anginal attacks. By means of balloon-catheter angioplasty the stenosis was reduced to less than 50%. The coronary angioplasty resulted in an improvement of left ventricular function, physical working capacity and cessation of anginal attacks.
