A report of recurrent anencephaly with trisomy 2p23-2pter: additional evidence for the involvement of 2p24 in neural tube development and evaluation of the role for cytogenetic analysis.

A woman carrying a balanced reciprocal translocation, 46,XX,t(2;5)(p23;p15)pat, was ascertained following the delivery of an anencephalic fetus whose karyotype was 46,XY,5p+. She subsequently had two pregnancies with a similar unbalanced karyotype (trisomy 2p23-2pter and monosomy 5p15-5pter), one of which was also anencephalic. She has three living children, two of whom are balanced translocation carriers. This history raises questions regarding the necessity of cytogenetic assessment of cases identified by ultrasound with 'isolated' neural tube defects. The observation of duplication of the 2p23-2pter region in conjunction with anencephaly also adds to the growing body of evidence suggesting an association of this region and neural tube development.

Epidermal growth factor receptor in vulvar malignancies and its relationship to metastasis and patient survival.

OBJECTIVE: To evaluate the level of epidermal growth factor receptor (EGF-R) expression in vulvar malignancies and to determine if a correlation exists between EGF-R levels and metastasis or patient survival. METHODS: All patients with a diagnosis of invasive squamous cell carcinoma of the vulva who were treated at our institution with a primary radical vulvectomy and inguinal lymph node dissection from 1983 to 1993 were eligible for the study. Sixty-one patients with available tissue blocks of benign vulvar epithelium, the primary malignant vulvar lesion, and groin node metastasis (when positive) were included in the study. Semiquantitative EGF-R expression was determined in a blinded fashion utilizing immunohistochemical staining of appropriate tissue samples. Survival was calculated utilizing Kaplan-Meier life table analysis based upon disease-free survival. RESULTS: A significant increase (P < 0.001) in mean EGF-R levels was demonstrated in the primary tumor (67%) versus benign vulvar epithelium (31%). In the 14 patients with lymph node metastasis, the mean EGF-R level in the primary tumor was 65% versus 88% in the metastatic lesion (P < 0.001). The likelihood of lymph node metastasis was elevated in those patients with a benign tissue EGF-R level > or =40% (P < 0.03) and in those patients with a primary tumor EGF-R level > or =90% (P < 0.025). Life table analysis revealed a cumulative disease-free survival of 45% for all patients. Disease-free survival in those patients with EGF-R levels > or =90% in the primary tumor was 25%, contrasting with a disease-free survival of 54% in those patients with EGF-R levels <90% (P < 0.05). CONCLUSIONS: There is a progressive increase in EGF-R expression from benign vulvar epithelium to primary malignant tissue to metastatic lesions within the same patient. Increased expression of EGF-R in the primary vulvar malignancy is significantly associated with lymph node metastasis and decreased patient survival. Increased expression of EGF-R in histologically benign vulvar epithelium has a significant association with lymph node metastasis and may predict decreased patient survival.

Effects of cadmium and nisoldipine on the delayed rectifier potassium current in guinea pig ventricular myocytes.

Block of the slow inward calcium current (Isi) during assessment of the delayed rectifier potassium current (IK) of cardiac ventricular myocytes is commonly achieved by use of either inorganic compounds such as cadmium or dihydropyridine derivatives such as nisoldipine. Effects of these two Isi blockers on IK characteristics of guinea pig ventricular myocytes were compared in this study. Currents were measured in the whole cell configuration of the patch-clamp technique and IK tail amplitudes were measured at -40 mV after depolarizations to various test potentials (voltage steps, -20 to +50 mV) for either 250 (IK250), 450 (IK450) or 5000 (IK5000) msec. Activating and tail currents measured in the presence of cadmium were of greater amplitudes when voltage steps were more positive than 0 mV but were of smaller amplitudes at Vtest < or = 0 mV compared to currents measured in the presence of nisoldipine or Tyrode solution. In the presence of the rapid component of the delayed rectifier E-4031, a blocker of cadmium increased IKs amplitude during high voltage tests and caused a positive shift in the voltage dependence of IKs activation at low depolarizing potentials. In contrast, no effect on IK was observed when nisoldipine was added to Tyrode solution. In conclusion, results obtained in this study suggest that cadmium depresses and/or shifts the activation curve of the rapid component and increases and positively shifts the slow component of IK in guinea pig ventricular myocytes. These observations lead us to propose that nisoldipine may be a better tool to inhibit long lasting inward calcium current during assessment of IK.

Inducing the quality of home health care theory through the use of grounded theory methodology.

Spinal cord injury primarily affects those aged 15-29 years. This age group's life experience may be affected by multiple sequelae of the injury. The quality of the care provided to these young individuals is an important area of research for the following reasons: (1) the topic has not been reported broadly in the nursing literature; and (2) there is a lack of empirical findings related to the quality of home health care. This investigation has used grounded theory as a qualitative approach to explore the quality of home health care and its related factors from the perspective of the injured persons and to generate the theory that includes the quality of home health care and its related factors. The investigator conducted intensive interviews with 10 spinal cord injured persons. These interviews were transcribed verbatim, analyzed, and interpreted. Findings indicated that the quality of home health care is affected by multiple factors including knowledge of disability, stressful life events, self-monitoring, activities of daily living, involvement of significant others, and self-satisfaction. Numerous nursing implications can be drawn for assessment and intervention in the early post-injury period that may help to improve and maintain optimum health care.

The loglinear model as a tool for the analysis of demographic data.

The author discusses the use of loglinear models in demographic analysis, particularly with regard to the analysis of contingency tables. Their use is illustrated using data from the 1980 World Fertility Survey for Sudan to analyze the determinants of fifth-birth intervals. The results show "the ability of the technique to improve our understanding of the mechanism of fertility differentials using individual--instead of aggregate--data. It has shown how each variable affects the process when all other variables are held constant."

Synthesis and biological activity of certain derivatives of 2,4-dioxo-1,2,3,4-tetrahydroquinazoline. Part 2.

The synthesis of 6-bromo-1-ethyl-2,4-dioxo-1,2,3,4-tetrahydroquinazoline, its benzoyl and its benzyl derivatives is described. The unreported 5-bromo-N-phenyl-anthranilic acid was synthesized from anthranilic acid adopting a procedure, which involved a novel synthesis of 2,5-dibromobenzoic acid. Direct bromination of N-phenylanthranilic acid and N-p-tolyl-anthranilic acid afforded the tribromo-and dibromoderivatives respectively.

Birth control practice in Khartoum.

The results presented here are based on a fertility survey carried out in Khartoum in 1978. It was shown that the knowledge of family planning is widespread in the city and that the level of ever-use of birth control is becoming significant. The popularity of the different methods of birth control is investigated, revealing that the pill is used by nearly all ever-users. IUDs, rhythm, and withdrawal are also relatively popular. A great degree of consistency was found to exist between reports given by husbands and their wives in this respect. The purpose of using birth control is not yet clear. The significant ever-use, noted before the 3rd live birth, indicates that these methods are used for the purpose of spacing births rather than limiting the number of children.
