[Levels of 1,25-dihydroxyvitamin D3 concentration in renal venous blood serum of patients with renovascular hypertension caused by unilateral renal artery stenosis].

UNLABELLED: Functionally significant, unilateral renal artery stenosis is characterized by reduced renal perfusion and lateralization of both excretory and endocrine function. The present study aimed to assess the influence of unilateral renal artery stenosis on plasma 1,25-dihydroxyvitamin D3 level. In sixteen patients (10 males and 6 females) with unilateral renovascular hypertension blood samples for estimation of plasma renin activity (PRA) and 1,25-dihydroxyvitamin D3 concentration were withdrawn from the renal vein of the ischaemic and normal kidney, from the femoral artery and from the vena cava inferior distally to the orifices of renal veins. Significantly elevated PRA (20.1 +/- 4.52 ng/ml/h vs 6.0 +/- 1.76 ng/ml/h, p < 0.005) but significantly reduced level of 1,25-dihydroxyvitamin D3 (99.25 +/- 4.8 pmol/l vs 115.75 +/- 6.7 pmol/l, p < 0.05) were found in renal vein blood of the ischaemic kidney as compared with the respective values of the normal kidney. No significant correlation was found between PRA and 1.25-dihydroxyvitamin D3 irrespective of the site of blood withdrawal. CONCLUSION: Renal ischaemia seems to exert a suppressive effect on 1,25-dihydroxyvitamin D3 synthesis by the kidney.

[Level of vasopressin in renal venous blood of patients with renovascular hypertension due to unilateral stenosis of renal arteries].

UNLABELLED: Assessment of plasma renin activity (PRA) in renal vein blood is used in the diagnosis of unilateral renovascular hypertension (URVH). Recently also other markers of renal ischaemia (atrial natriuretic peptide, adrenalin, nonadrenaline and dopamine) have been described. The present study aimed to assess renal handling of vasopressin (AVP) by the ischaemic kidney in patients with URVH. In 16 patients with URVH, PRA and AVP were estimated in renal vein blood of the ischemic (IK) and non-ischemic kidney (NK), in arterial blood (A) and in blood samples withdrawn from the inferior vena cava (VCI) below the orifices of the renal veins. In contrast to PRA no significant difference between plasma levels of AVP in renal vein blood of the ischaemic and non-ischaemic kidney was noticed (4.8 +/- 0.9 pg/ml vs 5.1 +/- 0.8 pg/ml respectively). CONCLUSION: Chronic hypoperfusion of the kidney does not influence renal handling of AVP in patients with URVH. Thus assessment of AVP in renal vein blood in these patients is deprived of diagnostic value.

[A case of kidney stone with unusual frequency of renal colic--possible influence of environment on lithogenesis].

In the present paper we described a 36 years old female, who expelled approximately 2000 stones from the urinary tract during the last 18 years. According to the clinical and laboratory tests the lithogenic factors such as abnormalities of calcium, phosphate, oxalate, magnesium and purine metabolism were excluded. The cystinuria and other forms of tubular nephropathies did not exist. Taking into account two following facts: 1) no evidence of renal calculi when the patient was outside her own house and 2) very high concentration of calcium in her home tap water, the role of environmental factors in the pathogenesis of nephrolithiasis in presented patient is very likely.

[Behavior of sex hormone and gonadotropin secretion in men with active nephrolithiasis].

Urinary excretion of calcium, magnesium, phosphate and oxalate in the condition of low-calcium diet, and the secretion of LH, FSH, testosterone and estradiol following LH-RH stimulation test have been determined in 26 men with active nephrolithiasis and in 14 healthy male subjects. Significantly higher urinary excretion of calcium and oxalate, and significantly lower excretion of magnesium was observed in men with nephrolithiasis as compared to healthy men. In addition, the patients with nephrolithiasis had significantly higher concentrations of testosterone, estradiol and FSH than the healthy controls. The results obtained suggest the participation of the gonadal hormones in the pathogenesis of active nephrolithiasis.

Enhanced activity of sympathetic renal nerves in hypertensive patients with unilateral renal ischemia--its relationship to plasma renin activity in renal venous blood.

In 16 hypertensive patients with significant unilateral renal artery stenosis plasma renin activity (PRA) and plasma levels of adrenaline (A), noradrenaline (NA) and dopamine were assessed in arterial blood, renal venous blood of the ischemic (IK) and normally (NK) perfused kidney and in blood withdrawn from the inferior vena cava, distally from the orifices of the renal veins. Plasma levels of A (= 661.8 +/- 187.7 pg/ml) and NA (= 396.3 +/- 72.5 pg/ml) in renal venous blood of the ischemic kidney were significantly greater than in renal venous blood of the normally perfused kidney (A = 123.4 +/- 16.0 pg/ml; NA = 277.2 +/- 55.6 pg/ml) and than in arterial blood (A = 68.44 +/- 8.4 pg/ml; NA = 192.8 +/- 28.9 pg/ml) and inferior vena cava blood (A = 67.8 +/- 7.5 pg/ml; NA = 182.6 +/- 26.8 pg/ml). In contrast to A and NA, plasma D level in renal venous blood of the normally perfused kidney was significantly higher (= 27.9 +/- 4.9 pg/ml) than in renal venous blood of the ischemic kidney (= 14.3 +/- 2.1 pg/ml) and than in arterial blood (= 16.1 +/- 1.9 pg/ml) and in blood of the inferior vena cava (= 16.3 +/- 1.8 pg/ml). A significant positive correlation was found between PRA and plasma levels of A and NA respectively only in renal venous blood of the ischemic kidney.(ABSTRACT TRUNCATED AT 250 WORDS)

Impaired phagocytosis in chronic renal failure is mediated by secondary hyperparathyroidism.

Patients with chronic renal failure (CRF) display impaired phagocytosis by the polymorphonuclear leucocytes (PMNL), and these cells have elevated basal levels of cytosolic calcium ([Ca2+]i) and reduced ATP content. It has been suggested that these changes in PMNL metabolism and function are mediated by the state of secondary hyperparathyroidism of CRF. To examine the role of excess PTH in these derangements of PMNL, we studied [Ca2+]i, ATP and phagocytic ability of PMNL in five groups of rats including: CRF, CRF normocalcemic parathyroidectomized (CRF-PTX), CRF and normal animals treated with verapamil (CRF-V), and normal-V, respectively. The level of [Ca2+]i in the PMNL of CRF rats (149 +/- 2.7 nM) was significantly (P less than 0.01) higher and the ATP content (4.2 +/- 0.17 nmol/5 x 10(6) PMNL) significantly lower (P less than 0.01) than in normal (108 +/- 2.4 nM; 9.5 +/- 0.15 nmol/5 x 10(6) PMNL), CRF-PTX (103 +/- 2.9 nM; 9.2 +/- 0.19 nmol/5 x 10(6) PMNL), CRF-V (107 +/- 2.2 nM; 9.0 +/- 0.2 nmol/5 x 10(6) PMNL) and normal-V (106 +/- 1.8 nM; 9.2 +/- 0.2 nmol/5 x 10(6) PMNL), despite sustained elevation in blood PTH in the CRF-V group. Phagocytosis was significantly (P less than 0.01) impaired in CRF animals (5.6 +/- 0.25 micrograms oil/10(7) PMNL/min) but was normal in CRF-PTX (9.3 +/- 0.21 micrograms oil/10(7) PMNL/min) and CRF-V (9.5 +/- 0.22 micrograms oil/10(7) PMNL/min) rats. The values of phagocytosis in normal and normal-V rats were 9.6 +/- 44 and 9.6 +/- 0.18 micrograms oil/10(7) PMNL/min, respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

Decreased O2 consumption by PMNL from humans and rats with CRF: role of secondary hyperparathyroidism.

Bactericidal ability of polymorphonuclear leukocytes (PMNL) is impaired in chronic renal failure (CRF). This function of PMNL is mediated by the generation of oxidizing radicals and the latter event requires O2 consumption by these cells. The present study examined both basal and FMLP-stimulated rise in cytosolic calcium ([Ca2+]i) and O2 consumption of PMNL from normal subjects and hemodialysis patients and from CRF rats, and evaluated the potential role of secondary hyperparathyroidism of CRF on these properties of PMNL. Basal levels of [Ca2+]i were significantly higher, and FMLP-induced increments in [Ca2+]i were significantly lower in PMNL of both humans and rats with CRF than in normals. Basal and FMLP-stimulated O2 consumption were significantly lower in CRF subjects and rats than in normals. These derangements were prevented by prior parathyroidectomy of CRF rats or by their treatment with verapamil from day one of CRF. Also, therapy of rats with pre-existing CRF with this drug reversed the abnormalities in [Ca2+]i and in O2 consumption of PMNL. The data indicate that: (1) CRF is associated with derangements in the homeostasis of [Ca2+]i of PMNL and their oxygen consumption, (2) these abnormalities are, most likely, mediated by the state of secondary hyperparathyroidism of CRF, and (3) verapamil, which blocks the PTH-induced entry of calcium into cells, and prevents as well as reverses these PMNL dysfunctions. These results implicate the excess PTH of CRF in the genesis of the defective bactericidal function of PMNL, and assign a new dimension to PTH toxicity in CRF.

Plasma erythropoietin concentrations in renal venous blood of patients with unilateral renovascular hypertension.

In 19 patients with unilateral renal artery stenosis and subsequent renovascular hypertension plasma renin activity (PRA), plasma concentrations of atrial natriuretic peptide (ANP), erythropoietin (Epo), H+ and HCO3-, as well as pO2 and pCO2 were assessed in renal venous blood of the 'ischaemic' and normally perfused kidney, both in arterial blood and in the inferior vena cava distally from the orifices of the renal veins. PRA and ANP were significantly elevated in venous blood of the ischaemic kidney as compared with the normally perfused kidney. In contrast to PRA and ANP, plasma concentrations of Epo were similar in blood withdrawn at all vascular sites. pO2 and pCO2, as well as blood H+ and HCO3- concentrations in venous blood of the ischaemic kidney were of the same magnitude as of the normally perfused kidney. From the results presented in this paper it follows that (i) in contrast to plasma renin activity and ANP, unilateral renal 'ischaemia' does not influence plasma concentrations of Epo in renal venous blood, and (ii) chronic haemodynamic alterations do not seem to influence Epo secretion by the kidneys.

On the mechanisms of impaired phagocytosis in phosphate depletion.

Phosphate depletion (PD) impairs the phagocytic ability of polymorphonuclear leukocytes (PMNL). This derangement has been attributed to the low ATP content of PMNL in PD. The mechanisms responsible for the low ATP content are not well defined. Phosphorus deficiency, per se, and/or other cellular metabolic consequences of PD such as a rise in cytosolic calcium ([Ca2+]i) could be responsible. Indeed, PD is associated with a rise in [Ca2+]i in other cells, and such an event may inhibit mitochondrial ATP production. It is also not evident whether the impaired phagocytosis in PD is due to low ATP content and/or a rise in the [Ca2+]i of PMNL. The study presented here examined levels of [Ca2+]i, ATP content, and the phagocytic ability of PMNL from PD and pair-weighed (PW) rats and evaluated the potential beneficial effect of treatment with verapamil (V), which may prevent a rise in [Ca2+]i and the consequent effects on ATP content and the phagocytosis of PMNL. The resting levels of [Ca2+]i of PMNL from PD rats (148 +/- 3.9 nM) were significantly (P less than 0.01) higher, and the ATP contents (4.8 +/- 0.2 nmol/5 x 10(6) PMNL) were significantly (P less than 0.01) lower than in PW (111 +/- 2.8 nM and 9.3 +/- 0.3 nmol/5 x 10(6) PMNL), PW-V (114 +/- 2.2 nM and 9.3 +/- 0.28 nmol/5 x 10(6) PMNL), and PD-V (112 +/- 1.8 nM and 6.6 +/- 0.19 nmol/5 x 10(6) PMNL) animals. Despite the normal [Ca2+]i in the PMNL of PD-V rats, their ATP contents were still significantly (P less than 0.01) lower than those of PW or PW-V rats.(ABSTRACT TRUNCATED AT 250 WORDS)

Mechanism of organ dysfunction in phosphate depletion: a critical role for a rise in cytosolic calcium.

Phosphate depletion (PD) is associated with multiorgan dysfunction. It has been proposed that this phenomenon is due to two metabolic derangements: (a) there is a reduction in ATP content of cells and hence reduced availability of energy-rich phosphate compounds, and (b) there is a decrease in 2,3-diphosphoglycerate in red blood cells, a change that would increase the affinity of hemoglobin to oxygen resulting in tissue hypoxia. Recent studies have demonstrated that PD is associated with a significant elevation in basal levels of cytosolic calcium [Ca2+]i in many cells. This is due to an increased entry into and decreased extrusion of calcium out of cells. This rise in [Ca2+]i plays a major role in organ dysfunction in PD and in the genesis of decreased ATP content of cells.

Plasma level of atrial natriuretic peptide in renal venous blood--marker of kidney ischemia?

In 12 patients with unilateral significant renal ischemia plasma levels of atrial natriuretic peptide (ANP) were estimated in renal vein blood of the ischemic (IK) and contralateral kidney (NK) and in arterial blood under supine and upright conditions. Plasma ANP levels in renal vein blood were compared with plasma renin activity (PRA) of the same blood samples. Plasma ANP levels in renal vein blood of the contralateral kidney (87 +/- 9 pg/ml) were significantly lower than in arterial blood (131 +/- 11 pg/ml) and renal vein blood of the ischemic kidney (139 +/- 16 pg/ml). In contrast plasma ANP concentrations in renal vein blood of the ischemic kidney were slightly or markedly higher than in arterial blood. A positive correlation was found between the ratio of plasma ANP in renal vein blood of the IK to that of the NK under supine condition and the respective ratio of PRA. Data presented in this paper suggest the presence of abnormal handling of ANP by an ischemic kidney and that plasma ANP levels in renal vein blood may be a marker of renal ischemia.

[Behavior of prolactin secretion and 25-hydroxycholecalciferol level in patients with active kidney calculi].

In 76 patients with urolithiasis the behaviour of the level of 25-OH-D and prolactin, respectively, and the metabolism of calcium and phosphate were investigated after an intravenous calcium load. In these patients a significant lower 25-OH-D level and a significant lower increase of the serum phosphate level after calcium load was observed in comparison with healthy adults. However, in contrast to healthy probands, the prolactin level was suppressed significantly. A participation of these endocrine changes in the pathogenesis of pathogenesis of urolithiasis could be suggested.

[Calcitonin and glucagon secretion in active nephrolithiasis].

In 76 patients with active nephrolithiasis and in 28 normal subjects the influence of an Ca-load on the calcitonin and glucagon secretion and on the serum calcium, phosphate and magnesium levels was examined. In the patients with active nephrolithiasis a significant suppression of Ca-induced calcitonin secretion and absence of glucagon secretion was found. Simultaneously the patients showed a lower decrease of serum Mg and reduced increase of serum phosphate levels. The authors suggest participation of the above mentioned biochemical and endocrine abnormalities in the pathogenesis of the active nephrolithiasis.
