[Pediatric "floating elbow"--case reports].

Floating elbow is an injury that is characterized by simultaneous fracture of humerus and forearm of the same extremity. The injury is caused by high energy (falls, car accidents) and is often associated with extensive soft tissue contusion of affected limb and there may be present lesion of neurocircular bundle. Operative treatment (minimally invasive percutaneous pinning) should be performed as soon as possible after patient's admission and this treatment should be preferred before circular casting, which may cause compartment syndrome. In the paper we present two cases of pediatric floating elbow treated at our clinic with different functional outcomes.

[Simultaneous double interphalangeal dislocation on a single finger].

The authors report a rare case of the simultaneous double interphalangeal dislocation in the little finger of the left hand. A 35-year-old man, playing football as a goalkeeper, was injured when another player trod on his little finger. The dislocation was reduced by traction without anesthesia and the finger was immobilized on a splint. Although the patient removed the splint only a week later and failed to come for a check-up, at 1 year, his little finger showed no deformity and regained a full range of painless, active motion.

[Isolated complete pancreatic rupture].

Isolated complete pancreatic rupture caused by blunt trauma is not a common injury. Such injury due to the retroperitoneal location of the pancreas and slow presentation of clinical signs of pancreatic trauma is unsuitable for an early diagnosis and this may lead to deterioration of the general condition of the patient. Early diagnosis in blunt pancreatic trauma may not lead to a smooth uncomplicated course of treatment. In our report, we present the case of 35-year-old man hit by a horse who suffered isolated complete laceration of the pancreatic tail.

[Dislocation of the humerus--diagnosis and the Arlt method of reduction].

Authors present 248 shoulder dislocations included recurrent dislocations in a 3-year period. All of dislocations were of anterior type except 3 posterior dislocations and 1 inferior or luxatio erecta. They describe and recommend method of reduction without local or general anesthesia according to Arlt, which was used in 234 cases with effectivity of 96.6%. Posterior dislocation can have a very subtle signs and is generally initially missed in more than 50% cases. Two X-ray views in posterior dislocation are necessary to confirm diagnosis. Authors used in addition to anteroposterior transthoracic view with no posterior dislocation missed.

[The closed method of intramedullary fixation of forearm fractures].

The authors demonstrate a group of 27 patients subjected to intramedullary osteosynthesis of the forearm by means of Kirschner wires, using closed surgery. They prefer the described method in patients suffering from multiple injuries who have devastated soft parts of the forearm and in unstable fractures in children. The authors draw attention to the fact that they have achieved very good results by the simple surgical method they described.

[Pediatric injuries in the lower extremities caused by insertion of the foot into the spokes of a bicycle].

The authors describe the injury of the lower extremity caused by putting the child's leg between the spokes of bicycle wheel during ride. The result is contusion of soft tissues of the leg and ankle or the fracture of the shin in a typical place. The injury requires a through treatment of soft tissues and sometimes also reduction of the fracture under total anaesthesia with subsequent fixation by plaster of Paris. On the basis of the group of 52 patients it has been proved that the mentioned injuries but for exceptional cases heal without sequelea. The authors emphasize the importance of the prevention of this injury by the installation of a plastic cover over the bicycle frame.
