[Use of walking in the evaluation of aerobic fitness].

The reliability and validity of assessment of the maximal oxygen consumption as a criterion of aerobic fitness by the 2000 m walking test was established in 38 students of the military secondary school (mean age 17.8 +/- 0.4 years). The reliability of the 2000 m walking test is very high, during repeated estimations in the course of one week we find a correlation coefficient of r = 0.82 (p < 0.001). There is also a close correlation of 2000 m walking time and 2000 m running time (r = 0.67, p < 0.001). A group of 18 students selected at random from the above group was examined in the laboratory on a treadmill up to vita maxima. A close correlation was found between the results during walking and the maximal oxygen consumption (VO2max.kg-1) (r = -0.59 and -0.60) and also the 2000 m running time and VO2max.kg-1 (r = 0.59), p < 0.01 in all instances. The standard error of the mean of the maximal oxygen consumption estimate from the 2000 m walk is 3.44 ml.kg-1 x min-1 (5.7%) and from the running time of the same distance 3.49 ml.kg-1 x min-1 (5.8%). These results justify the conclusion that the 2000 m rapid walking test (normal style) is a simple alternative way for estimating the maximal oxygen consumption and thus also for evaluating the level of aerobic fitness with an accuracy which matches that, when running is used for the test.

[The effect of hormone therapy on the lipid spectrum in postmenopausal women].

In a group of 92 postmenopausal women with climacteric and postcastration syndrome the relationship between different types of hormonal therapy and changes of indicators of the lipid metabolism was investigated. Before onset of treatment and after three weeks of treatment the authors assessed total cholesterol, beta-lipoproteins, triglycerides, HDL, LDL and plasma concentrations of oestrone and 17-beta-oestradiol. A significant rise of both oestrogens was recorded in a group treated with Presomen tablets and Folovirin inj. In the lipoprotein fractions only beta-lipoproteins rose significantly during administration of Mestranol tabl. and Provera tabl. The investigation is the beginning of a prospective study concerned with the possibility to influence one of the risk factors participating in the development of arteriosclerosis.

[Morphologic basis of hemorrhage during hormonal therapy in the climacteric syndrome].

In a group of 133 postmenopausal women in the course of nine years the relationship between two types of hormonal treatment and one of the side-effects--metrorrhagia--was investigated. This symptom was found in 37 patients. The authors followed two aims. To evaluate the relationship between treatment of the climacteric syndrome and the morphological state of the uterine mucosa and to assess the optimal therapeutic pattern under local conditions. The insignificant frequency of proliferating endometrial changes supports some data in the literature which draw attention also to other causes of haemorrhage. With regard to therapeutic regimes, the authors consider continual administration of conjugated oestrogens--0.625 mg and medroxyprogesterone acetate 5 mg per day as suitable. They emphasize the necessity to follow up the patients in a special clinic with the opportunity to make hormonal examinations and to ensure interdisciplinary collaboration.

Clinical experience with the testing of serum proteinase activity by cathepsin B-like sensitive amino acid derivatives of 7-amino-4-methylcoumarine.

The proteolytic activity of serum against two synthetic polypeptide substrates conjugated with 7-amino-4-methylcoumarine which are known to be highly specific substrates for lysosomal cathepsin B-like cysteine proteinase, was studied in normal individuals and in patients with breast and colorectal cancer. The results showed that both substrates are cleaved in different ways and their hydrolysis is very poorly sensitive to inhibitors of cysteine proteinases. On the other hand, cleavage is intensively inhibited by disodium-EDTA which is known as an activator of cysteine proteinases. This indicates that lysosomal conditions are quite different from serum which is an unstable mixture of various enzymes and their substrates. It was also demonstrated that hydrolysis of the tested substrates is not probably realized by cysteine proteinases but it can be performed by cooperation of other serum proteinases. Moreover, our results confirmed that serum activity of these enzymes can be significantly higher in patients with breast cancer, particularly with metastasis, than in healthy women. A slight insignificant increase was observed also in patients with colorectal cancer.
