Bitemporal compression injury caused by static loading mechanism. Report of two cases.

The authors report two cases of bitemporal compression injury caused by a static loading mechanism. These head injuries resulted from gradual bitemporal compression of the head. Plain skull films showed multiple skull fractures, and carotid angiography revealed internal carotid artery obstruction at the base of the skull. Neurological examination disclosed a slight disturbance of consciousness, hemiparesis, multiple cranial nerve injuries, and Horner's syndrome. In comparison with impact head injury, the energy from this type of trauma tends to be transmitted to the foramina and hiati of the middle cranial fossa and results in multiple injuries to the cranial nerves, sympathetic nerves, and blood vessels.

[Functional recovery following surgical removal of traumatic epidural hematoma--factor analysis].

UNLABELLED: Factors contributing to functional recovery following evacuation of epidural hematoma were analyzed in 53 subjects. Subjects were limited to the cases with "pure" epidural hematoma. Fifty-three cases were classified into 3 groups based on presence or absence, and duration of preaggravation period (PAP) following occurrence of head trauma (Fig. 1). Level of consciousness at operation and at PAP is summarised in Fig. 2. Major neurological signs at operation are summarized in Table 1. Gradings of functional status were divided to 6 (Table 2). Gradings of 53 subjects which were judged 1 month after removal of epidural hematoma are summarized in Fig. 3. RESULTS: 1) As to outcome in relation to duration of preaggravation period (PAP) and consciousness level at operation (Fig. 4): The patients whose PAP was shorter and whose consciousness level at operation was more severe, took outcome of lower (worse) gradings. 2) As to outcome in relation to interval from the end of PAP to operation and PAP (Fig. 5): The patients whose PAP are within 3 hours, took outcome of relatively good recovery only when epidural hematoma was removed within 5.5 hours after the end of PAP. 3) As to outcome in relation to interval from the end of PAP to operation and consciousness level at operation (Fig. 6): The patients whose consciousness level at operation was better than semicoma took good recovery when epidural hematoma was evacuated within 5.5 hours after the end of PAP. This was right even in the patients who presented with decerebrate posture.(ABSTRACT TRUNCATED AT 250 WORDS)

[Treatment of carotid-cavernous fistula using a detachable balloon catheter--a case report and review].

A case of post-traumatic carotid cavernous fistula, successfully treated by a detachable balloon catheter, is reported. A 55-year-old housewife was admitted to the author's department on 25th August, 1980. Three months prior to admission, she complained of right tinnitus following a traffic accident and then developed right chemosis and exophthalmus. A selective right internal carotid angiography revealed rapid filling of the carotid cavernous fistula. Under general anesthesia, the patient was treated by the maneuver after Debrun. Since it appeared necessary to employ the second balloon, the first one was released gently being inflated only with the contrast medium. During the procedure, the neck of the balloon was incidentally snapped upward and, then, the fistula was eventually occluded preserving the carotid flow as was verified by a control angiography. The bruit was abolished immediately after the procedure and all ocular symptoms disappeared during the following a few weeks. A skull film showed a deflation of the balloon at 4 weeks postoperatively, but clinical and angiographic follow-up examinations at 9 months thereafter revealed no signs or symptoms of recurrence. This Debrun's procedure is being widely accepted for a carotid cavernous fistula as a relatively simpler, safer, and more effective one comparing with the conventional methods. Our experiment on dogs, however, indicated that rupture of the balloon is not always avoidable even with a smaller volume than its maximum capacity and some resistance was felt during introduction of the co-axial catheter. Thus, it appeared mandatory to improve and quality control the balloon and the catheter. In the case presented here, a follow-up angiography showed no recurrence of the fistula except for an asymptomatic pseud-aneurysmal pouch, even though the balloon was inflated only with the contrast medium and was deflated within 4 weeks. These findings suggest possibility of successful, and long-lasting outcome without employing silicone.

Use of milk progesterone enzyme immunoassay for early pregnancy diagnosis in cows.

An enzyme immunoassay using beta-galactosidase as the tracer was applied to determine milk progesterone level in cows. The novel method was reliable and practicable and required only a spectrophotometer and a centrifuge as major equipment. The milk progesterone enzyme immunoassay successfully diagnosed early pregnancy in cows. Milk samples were collected from 268 Holstein-Friesian cows in commercial dairy herds on 20, 21 or 22 days(usually 21 days) after insemination. Progesterone level in skim milk higher than 1.0 ng/ml indi-cated pregnancy. Pregnancy was confirmed by rectal palpation on 60 to 120 days after insemination. The accuracy of the milk progesterone test was 60.0 % (132 220 ) for a positive diagnosis and 100 % (48 48 ) for a negative result. A high incidence of embryonic death, 27.9 % (51 183 ), may have reduced accuracy for a positive test result. The enzyme immunoassay may be an alternative to radioimmunoassay in milk progesterone analysis for pregnancy diagnosis.

Anterior decompression for ossification of the posterior longitudinal ligament of the cervical spine.

Anterior decompression and fusion for treating ossification of the posterior longitudinal ligament of the cervical spine was performed in 12 patients. The central part of the vertebral body and the ossified area of the posterior longitudinal ligament were removed by means of a microrongeur and an air drill. The defect was filled with a long bone graft taken from the ilium. The operative results were excellent. Marked improvement of radicular and spinal cord signs was seen in all 12 cases. Three vertebral bodies were fused in one case, four in nine cases, and five in two cases. The highest level of fusion was C-2 and the lowest was T-1. It is considered that any ossification of the ligament below the C-2 level can be removed via an anterior approach as long as no more than five vertebral bodies are involved. Spinal computerized tomography was valuable in providing more detailed information about the stenotic spinal canal and the shape of the ossified ligament.

[A case of cervical extradural neurinoma with vertebral artery occlusion (author's transl)]?1.H.

A case of cervical extradural neurinoma with total occlusion of the right vertebral artery is reported. The patient was a 22-year-old man who had suffered slowly progressive tetraparesis for 8 years. Neurological examination demonstrated spastic tetraparesis, muscle atrophy of the trunk and four extremities, and hypalgesia corresponding to C2, 3 and 4 dermatomes. Plain radiogram of the cervical spine revealed a large bony defect on the right side of C2 and C3 vertebrae and enlargement of the intervertebral foramen of the right C2-3. Vertebral angiogram showed a reticular tumor stain in the area of the bony defect mentioned above and an occlusion of the right vertebral artery at the level of the inferior border of the C2 vertebra. It's feeding arteries were radicular arteries. The tumor was located extradurally and excised successfully. Histology showed a typical neurinoma. Referring to the reported cases, vertebral angiography has been rarely performed and there were only three cases including our cases in which total occlusion of the vertebral artery was demonstrated.

Neuroepithelial (colloid) cyst of the cerebellar vermis containing a xanthogranuloma.

A case of a neuroepithelial cyst of the cerebellar vermis is presented. The cyst contained a viscous fluid and a xanthogranulomatous nodule. The fluid showed high signal on T1- and low signal on T2-weighted MR images. The xanthogranuloma showed mixed intensities with partial contrast enhancement. The correlation of the cyst contents and MR signals is discussed.
