[The time of maintainance of positive serologic results among patients operated on echinococcal liver cysts].

The purpose of this work was to analyse results of serologie examinations and complaints reported by patients who were operated on echinococcal liver cysts. The analysis was conducted on patients who were qualified to surgical removal of echionococcal liver cysts. Persons who were analysed answered the questionaires and 11 of them send sera for serologie examination. The questionaires consisted of questions about time since operation, complaints before and after operation abdomen pains, distensions, vomits, exanthema, weakness, headaches). Results showed that patient were from 1 to 6 years after operation. Among 11 who send sera 18% results were negative (2 and 6 years after operation) and 82% was positive. Analysis of 15 questionnaires showed that majority of patients still had the complaints that were present before operation.

[Tuberculous lymphadenitis--diagnostic difficulties].

Three cases of tuberculous lymphadenitis hospitalized in Department of Parasitology and Neuroinfection are presented. In all patients tuberculin test was positive. In 2 patients minimal lesions in lungs were present. In non patient BK in sputum was found. In microscopic picture of enlarged lymph nodes non specific inflammation granulomatosis was found but microbiological examinations were not done. In all patients the results of smears of lymph nodes pus were negative. In 2 patients culture was positive. Persistent enlarged peripheral lymphatic nodes, especially with fistula should be suspected of tuberculous etiology. Material obtained from node's biopsy should be taken to microbiological and histological examination.

[Concentrations of tumor necrosis factor alpha and interleukin-1 beta in cerebrospinal fluid in the course of tick-borne encephalitis].

CSF concentrations of TNF-alpha and Il-1 beta were detected in patients with TBE. The cytokines were detected by immunometric assay by MEDGENIX kit. CSF Concentrations of TNF-alpha and IL-1 beta in patients with TBE were significantly higher than in control group before as well as after treatment and normalization of CSF parameters. These concentrations were lower comparing to one obtained in group of bacterial meningitis. There was no correlation between concentration of cytokines and other CSF parameters (cytosis, protein, glucose concentration). Concentrations of analysed cytokines did not change significantly before and after treatment. Detection of CSF concentrations of TNF-alpha and Il-1 beta in patients with tick-borne encephalitis can be used to evaluate efficacy of treatment and retreat of infection.

[Detection of antibodies to Borrelia burgdorferi among forestry workers in North-Eastern Poland].

The purpose of the study was to assess the incidence of Borrelia burgdorferi antibodies and clinical forms of Lyme borreliosis among forestry workers in north-eastern Poland. The group studied consisted of 1,466 persons (297 women and 1,169 men), aged 20-70 years. In 439 (23.81%) persons the presence of B. burgdorferi antibodies was detected in serum, in 271 (18.49%) persons active Lyme borreliosis was diagnosed and 78 (5.32%) persons were carriers of B. burgdorferi antibodies. Arthritis (43.84%) and neuroborreliosis (32.95%) were found to be the most common forms of borreliosis.

[Changes in granulocytic receptors for FcR IgG and CR with circulating immune complexes in patients with lyme borreliosis].

This study aimed to estimate some PMN functions, involving phagocytic activity in patients with Lyme borreliosis. Decreased percentage PMN with FcR and CR receptors was observed. Increased immune complexes levels in the serum of patients before, and their normalization after treatment were found. These results indicate a depression of non-specific cellular response, which can influence the general immune system in patients with Lyme borreliosis.

[Analysis of selected parameters of damage to the liver during the course of tick-borne encephalitis].

In 1993, 80 patients (43 female and 37 male) with tick-borne encephalitis were hospitalized in Clinic of Neuroinfections and Parasitic Diseases of Medical School in Białystok. 6 (7.5%) patients were jaundiced, 16 (20%) had hepatomegaly. We noticed increased enzymatic activity of aspartate aminotransferase (x = 74.5 U/l) in 9 (11.25%) cases, alanine aminotransferase (x = 89.5 U/l) in 11 (13.75%) cases, increased level of bilirubin in blood (from 1.4 to 3.5 mg/dl) in 7 (8.75%) cases. Protein level in blood was average x = 5.95 g/dl, percentage of albumin was average x = 48.61%, globulin--x = 14.62%. Prothrombin index and ammonia blood level were in normal range. Observed pathological changes were transient and referred mainly to patients with severe clinical course of disease.

[A comparison of serologic ELISA tests from BIOMEDICA Ges. M.B.H. and VIRO-Immun Labor Diagnostica GMBH in the diagnosis of Lyme disease (introduction)].

The purpose of this work was to evaluate detection of antibodies IgM and IgG against B. burgdorferi. We examined presence of IgM in 96 serum samples from patients suspected of Lyme disease. We looked for IgG in serum samples of 48 forest workers who submitted frequent tick bites in last two years. Among 96 patients whose sera were examined we had positive results in both tests in 5 cases and in 25 cases positive results were achieved only in test II. In 48 serum samples which we examined for presence of IgG in 2 cases positive results were in both tests, in 1 case positive in test II and negative in test I in 3 cases positive result in test I and negative in test II. According to these results we drew following initial conclusions: 1) obtained results testify better sensitivity and specificity of test II which consists of recombinant antigens of Borrelia burgdorferi, 2) no essential difference was shown in detection IgG among two used tests.

[Mental disorders in the course of neuroborreliosis: own observation].

Three cases of psychic disorders in the course of Lyme borreliosis were described. Two cases were connected with Lyme meningitis, one of them with chronic Lyme arthritis. These cases were described to discuss the possible occurrence of mental disturbances in the course of Lyme borreliosis, directly or indirectly connected with the central nervous system.

Borrelia burgdorferi Group↗

Role of reactive oxygen species (ROS) in patients with erythema migrans, an early manifestation of Lyme borreliosis.

BACKGROUND: Lyme borreliosis is a tick-transmitted, chronic, zoogenous disease caused by Borrelia burgdorferi spirochete. The clinical picture of Lyme disease is characterized by the variety of tissue and organ involvement and differing severity of symptoms. One of the pathogenic symptoms of early Lyme disease is a skin lesion called erythema migrans. MATERIAL AND METHODS: The purpose of our research was to estimate the parameters of the antioxidant system and the concentration of lipid peroxidation products in the plasma of patients with erythema migrans (EM). The parameters measured included the activity levels of superoxide dismutase (SOD) according to Sykes, gluthatione reductase (GSSG-R) according to Mize and Langdon, glutathione peroxidase (GSH-Px) according to Paglia and Valentine; the concentrations of malondialdehyde (MDA) were examined by means of a Bioxytech LPO-586 kit. The total sulphydryl groups (-SH) according to Ellman and reduced glutathione (GSH) were measured using a Bioxytech GSH-400 test in plasma samples collected from 20 patients with EM aged from 19 to 50, taken before (examination 1) and after (examination 2) therapy with amoxycycline. The control group consisted of 8 healthy people. RESULTS: The results of our examinations prove that beta-lactamase antibiotic therapy brings non-enzymatic antioxidant parameters to control values, though the treatment causes no change in enzymatic antioxidant parameters, resulting in the further activation of free radicals. CONCLUSIONS: In patients with Erythema migrans, the decreased capability to reduce lipid superoxidants leads to maintaining a high concentration of membrane lipid peroxidation products.

[The assessment of tumor necrosis factor (TNF) alpha and interleukin (IL)-1 beta levels in cerebrospinal fluid (CSF) and serum in patients with purulent meningitis].

The purpose of this work was to detect the concentration of cytokines--tumor necrosis factor (TNF)-alpha and Interleukin (IL)-1 beta in cerebrospinal fluid (CSF) and serum, their diagnostic and prognostic usefulness among patients with purulent meningitis. 25 patients of Centre of Parasitic Diseases and Neuroinfections hospitalized with diagnosis or suspicion of purulent meningitis were studied. The patients were divided into two groups: I-12 persons with diagnosis of purulent meningitis and group II-13 controls where suspicion of meningitis was excluded during clinical procedure. The cytokines were detected by immunometric assay. This examination was performed twice--before and after treatment. In group I-patients with purulent meningitis, TNF-alpha and IL-1 beta CSF concentrations were significantly higher in comparison with group II. After treatment the concentrations of cytokines did not show significant differences between group I and II. In acute phase of meningitis correlations between CSF concentration of TNF-alpha and IL-1 beta and other indexes of inflammation in CSF: pleocytosis, concentration of protein and glucose were found. The serum concentrations of the cytokines were many times lower than in CSF. The detection of TNF-alpha and IL-1 beta CSF concentration can be used to estimate the efficacy of therapy and regression of inflammation in the central nervous system.
