LET, track structure and models. A review.

Swift heavy ions when penetrating through matter strip off those electrons having a smaller orbital velocity than the ion velocity. The remaining electrons screen the nuclear charge yielding an effective charge. The effective charge of the ions interacts predominantly with the target electrons causing excitation and ionizations of the target atoms. Using the Bethe Bloch formula for the energy loss combined with the Barkas formula for effective charge, the energy loss values as well as unrestricted and restricted linear transfer can be calculated within a few percent of accuracy. From the primary energy loss only a small fraction of 10% or less is transformed into excitation. The major part of the energy loss is used for the ionization of the target atoms and the emission of the corresponding electrons with a high kinetic energy. These electrons form the track around the trajectory of the primary ion in which two thirds of the primary energy is deposited by collisions of primary, secondary and later generations of electrons with the target molecules. In the electron diffusion process the energy is transported from the center of the track into the halo. The radial dose decreases with the square of the radial distance from the center. The diameter of the track is determined by the maximum range of the emitted electrons, i.e. by the maximum energy electrons. All ions having the same velocity i.e. the same specific energy produce electrons of the same energy and therefore tracks of the same diameters independent of the effective charge. But the dose inside the track increases with the square of the effective charge. Track structure models using this continuous dose distributions produce a better agreement with the experiment than models based on microdosimetry. The critical volume as used in microdosimetry is too large compared to the size of the DNA as critical structure inside the biological objects. Track structure models yield better results because the gross-structure of the track i.e. its lateral extension and the thin down toward the end of the track is included in these calculations. In a recent refinement the repair capacity of the cell has been included in a track structure model by using the complete shouldered x-ray survival curve as a template for the local damage produced by the particle tracks. This improved model yields presently the best agreement with the experiment.

Energy Transfer

[Multiple trauma: preclinical needs, transportation, time sequences].

Compared to the magnitude of the trauma problem few precise data exist on prehospital care of trauma patients. The aim of the study was the collection and careful evaluation of prehospital data on trauma patients concerning time sequences, patients' status and prehospital interventions. From 1. 1. 1987 to 31. 12. 1990 all 49,045 prehospital emergencies in Cologne were prospectively registered. 8792 trauma patients were treated by an emergency physician in the field. 9.5% were severely injured, defined by a trauma score less than or equal to 12. 9.9% of the trauma patients were intubated, 54.9% received an i.v. line, and 20.6% were triaged to a trauma center. Our data form a valid base for analysis of the effectiveness of prehospital trauma care.

[Single-stage exposure of the fistula in surgical treatment of the peri-proctitis abscess].

117 anorectal abscesses has been operated by one operateur. 68x a fistulectomy was done. 2x there has been a high fistula. Of the remaining 47 patients 44 have had a followup for 15 months. In this time 37 showed a fistula-in-ano. Procedure and the position of the fistulous tracks are shown. The assumptions and dangers of the single-stage approach are demonstrated.

Radiation-induced activation of transcription factors in mammalian cells.

In mammalian cells radiation induces the enhanced transcription of several genes. The cis acting elements in the control region of inducible genes have been delimited by site directed mutagenesis. Several different elements have been found in different genes. They do not only activate gene transcription in response to radiation but also in response to growth factors and to tumor promoter phorbol esters. The transcription factors binding to these elements are present also in non-irradiated cells, but their DNA binding activity and their transactivating capability is increased upon irradiation. The signal chain linking the primary radiation-induced signal (damaged DNA) to the activation of transcription factors involves the action of (a) protein kinase(s).

Brain stem auditory evoked potentials (BAEP) monitoring in brain death.

In our intensive care unit, brain stem auditory evoked potentials (BAEP) are measured continuously in comatose patients. For this study we examined 15 patients who fulfilled the criteria of brain death (2). Loss of BAEP in these patients inevitably means loss of brain stem function and consecutive death. Our investigation shows that continuous BAEP monitoring can be helpful for the assessment of brain death and for the earlier decision of organ explantation.

UV-induced transcription from the human immunodeficiency virus type 1 (HIV-1) long terminal repeat and UV-induced secretion of an extracellular factor that induces HIV-1 transcription in nonirradiated cells.

UV irradiation, but not visible sunlight, induces the transcription of human immunodeficiency virus type 1 (HIV-1). Chimeric constructs carrying all or parts of the HIV-1 long terminal repeat linked to an indicator gene were transfected into HeLa cells or murine and human T-cell lines, and their response to irradiation was tested. The cis-acting element conferring UV responsiveness is identical to the sequence binding transcription factor NF kappa B. UV irradiation enhances NF kappa B binding activity as assayed by gel retardation experiments. Interestingly, the requirement for UV irradiation can be replaced by cocultivation of transfected cells with UV-irradiated nontransfected (HIV-1-negative) cells. A UV-induced extracellular protein factor is detected in the culture medium conditioned by UV-treated cells. The factor is produced upon UV irradiation by several murine and human cell lines, including HeLa, Molt-4, and Jurkat, and acts on several cells. These data suggest that the UV response of keratinocytes in human skin can be magnified and spread to deeper layers that are more shielded, including the Langerhans cells, and that this indirect UV response may contribute to the activation of HIV-1 in humans.

[Glutamic acid group poisoning. So-called Chinese restaurant syndrome].

After eating a soup 10 persons (out of 100) fell sick; within 10 minutes they suffered from nervous muscle convulsions, trembling, mouth desiccation and dilatation of the pupils. The soup contained glutamate as flavour enhancer in an unusually high concentration of 31 grams per litre.

Effects of SO2 exposure on allergic sensitization in the guinea pig.

The effect of sulfur dioxide (SO2) exposure on local bronchial sensitization to inhaled antigen was studied in the guinea pig. Exposure to SO2 (0.1 to 16.6 ppm) was performed in a 20 L exposure chamber for 8 hours on 5 consecutive days, while temperature, moisture, and concentration of SO2 were monitored and kept constant. SO2 concentrations were measured hourly by Schiff's reaction. On the last 3 days, SO2 exposure was followed by inhalation of nebulized ovalbumin (OA) for 45 minutes. One week later, specific bronchial provocation with inhaled OA (0.1%) followed by plethysmographic measurements of airway obstruction were performed every 2 days during a 2-week period. Specific antibodies against OA were measured in serum and bronchoalveolar fluid by a direct enzyme immunoassay. The SO2-exposed group (N = 17) demonstrated 67% to 100% positive bronchial reactions to inhaled OA, depending on the concentration of SO2, whereas the control group without previous SO2 exposure (N = 14) demonstrated bronchial reactions in only one animal (7%: p less than 0.05). The degree of bronchial obstruction was significantly higher in the exposed group, compared to the control group, for all SO2 concentrations (p less than 0.05). OA-specific antibodies in serum and bronchoalveolar fluid increased in SO2-exposed groups significantly, compared to the control group (p less than 0.05). It is concluded from these results that exposure to SO2 in low and medium concentrations can facilitate local allergic sensitization in the guinea pig.

Size and composition of liver vitamin A reserves of human beings who died of various causes.

Postmortem livers from 77 "normal" persons, 37 patients with neoplastic disease, 10 subjects with liver insufficiency, and 7 infants were analysed for free and esterified retinol. The average concentrations of total vitamin A for the members of each group were 597, 551, 289, and 162 micrograms/g wet liver, respectively. Compared with corresponding control values, both cancer victims and patients with liver disease had significantly lower hepatic vitamin A levels. With regard to the composition of the liver vitamin A reserves, our results show that approximately 97% of this vitamin was present as retinyl ester. Additionally, minute amounts of retinol were also found in most of the liver specimens analysed. In "normal" subjects the major ester fraction recovered was palmitate/oleate followed by stearate and myristate/linoleate. By contrast, the second most abundant fatty acid in the retinyl ester fraction of cancer victims was myristic/linoleic acid together with significantly smaller quantities of stearic acid. In tissue samples obtained from patients with liver disorders, however, the myristate/linoleate fraction was increased and therefore nearly equal amounts of both retinyl stearate and myristate/linoleate were present.

Cause of Death

[Continuous registration of intracranial pressure using disposable transducers].

The authors report the results of physical tests (baseline drift, temperature drift, absolute accuracy) and clinical experiences in intraventricular intracranial pressure monitoring with disposable transducers. Physical tests gave excellent results concerning baseline drift (less than 1%), temperature drift (0.35 mmHg/degrees C), and absolute accuracy (+/- 2 mmHg) in the range of intracranial pressure of both transducers tested (Gould DTX and Cobe 41-500). Clinical testing gave corresponding values compared with routine methods of ventricular pressure monitoring via a Bentley-Trantec-Transducer. As a result of the study disposable pressure transducers are recommended for routine use in clinical practice of intraventricular intracranial pressure monitoring.

Brain Injuries

[Sudden cardiac death following psychoemotional stress].

Sudden death from cardiac arrest is described, which occurred after a minor accident. Morphologically, a coronary change was found in the sense of "juvenile coronary atherosclerosis" in connection with intramural bleeding postmortem. As there were no injuries in connection with the accident but psychoemotional stress had occurred, it is concluded that affective resonance resulting from the accident initiated the death. The pathological and pathophysiological relationships are discussed.

[Morphometric and densitometric studies on the ultrastructure of endothelial cells in capillaries in tumor bearing rats].

The permeability of tumor capillaries to higher molecular substances like albumin and beta-globulin is increased. The comparison of permeability is associated with a modification of the ultrastructure of the capillary wall, especially of endothelial cells and their nuclei. On micrographs of blood capillaries Zajdela-Ascites-Hepatoma pregnanted rats we've studied a) morphometrically the surface and the volume of endothelial cell nuclei in relation to the time after tumor implantation, b) densitometrically the Hetero-Euchromatin-ratio in the nuclei of the same cells in relation to tumor time too. We've got the following results: The surface of the endothelial cell nuclei is increasing in the first four days, then it's decreasing to the 8th day. The volume of the same nuclei is increasing to the 4th experimental day too, then it's relatively steady. At the 6th experimental day the largest part of Heterochromatin is "euchromatinized". It will be possible to distinguish the functionally and the degeneratively nucleus' swelling with morphometrically methods. The reasons of modifications in the ultrastructure of blood capillary endothelial cell nuclei might be a) the increased demandings to the permeability of capillary wall, b) the increased activity of the MMS/RHS and c) local immunity reactions against the tumor.
