Differential haemodynamic effects induced by beta 1-(bisoprolol) or beta 2-(ICI 118,551) adrenoceptor blockade in man.

To characterize beta 1- and beta 2-adrenoceptor mediated effects in man the influence of the selective beta 1-adrenoceptor antagonist bisoprolol and the selective beta 2-adrenoceptor antagonist ICI 118,551 on changes in blood pressure, heart rate and lymphocyte beta 2-adrenoceptor density evoked by dynamic exercise or isoprenaline infusion was studied in 12 male normotensive volunteers. Bisoprolol administration (1 X 10 mg/day) did not affect lymphocyte beta 2-adrenoceptor density, while ICI 118,551 (3 X 25 mg/day) increased it by about 40%. Exercise as well as isoprenaline infusion caused 100% increases in lymphocyte beta 2-adrenoceptor density; these increases were completely abolished by ICI 118,551, but not affected by bisoprolol. ICI 118,551 markedly attenuated isoprenaline-induced decrease in diastolic blood pressure, but did not affect increase in systolic blood pressure; on the contrary, bisoprolol inhibited the isoprenaline-evoked increase in systolic blood pressure, but did not affect the decrease in diastolic blood pressure. ICI 118,551 antagonized the isoprenaline-induced tachycardia much more potently than bisoprolol, while bisoprolol, but not ICI 118,551, suppressed exercise-induced tachycardia. It is concluded that exercise-induced tachycardia is mediated by cardiac beta 1-adrenoceptor stimulation, whereas isoprenaline-induced tachycardia is mediated by both beta 1- and beta 2-adrenoceptor stimulation.

[Parental and childhood height development. Corrected percentile standards for German children between the age of 2 and 9].

The conventional centile standards allow a judgement of children's height according to age-groups. It is equally important, however, to take the parental height into consideration. In 1970 Tanner ascertained for the British population the correlation between children's height and the mid-parent height (i.e. the average of father's and mother's height). From these results he set up centile standards that allow the judgement of children's height at successive ages in terms of the mid-parent height. Several conditions should be fulfilled for their application within other populations: identical correlation coefficients of parents' and children's heights which were shown in Tanner's international study; the accordance of the average height and the standard deviations, respectively, of children in each of the age groups as well as the conformity of the mid-parent height. Our investigations showed that the 3 parameters which were last mentioned differed from those of the British population. Therefore, proper standards for correction were set up. The calculations were worked out for children at ages 2 to 9 years. The methodical procedure will be shown in detail. The correction standards allow a statement as to whether the height of a child can be classified as normal or not within the framework of his family. This is particularly advantageous in regards to the diagnostic clarification of short stature, since it is often due to constitutional, genetic factors; if, for example, according to age the height of a five-year-old boy as well as the mid-parent height coincide at the 3rd centile, then the child's height centile will be 32nd. in consideration of these hereditary components. So the differential diagnosis of genetic short stature is made considerably more straightforward by the use of the present charts.

[Diseases caused by atypical mycobacterial].

Infections having atypical mycobacteria are occurring more frequently for 2 main reasons. First there are the disturbances of the immunity system, whether due to the acquired immune deficiency syndrome (AIDS) or the result of therapeutic measures taken against malignant processes, other diseases or after organ transplantation. Second, travel and free-time activities increase the risk of an infection. In many cases such infections become localized on the skin, but can also develop changes in the lungs, depending on the patient's occupation. For this reason we provide a review of the new literature on the microbiology of mycobacteria as well as on the symptoms of diseases of the skin and other organs which are caused by atypical mycobacteria, such as swimming pool granuloma or Buruli ulcer.

[Microbiological degradation of glucosinolates in defatted rapeseed meal].

During the degradation of thioglucosides in defatted rape seed meal (RES) microorganisms were found, whose ability to degrade glucosinolates (GSL) and vinyl thio-oxazolidone (VTO) was not known so far. The isolated microorganisms are two strains of bacteria of the species Bacillus cereus and the yeast Trichosporon cutaneum. The degradation of GSL and VTO in the cultural broths by the Bacillus cereus strains was the more complete the more other Gram-negative bacteria from RES were additionally present. Drinking tests with Wistar rats showed that here is a relation between the watersoluble and mainly bitter toxin substances of rape and their influence on the increase of the animals body weight.

[Surgical aspects of the treatment of malignant thyroid gland tumors].

95 cases with primary malignant thyroid tumours underwent surgery from 1965 to 1977. In view of the interdisciplinary character of treatment and independent of histological differentiation, total thyroidectomy should be performed. Differentiated carcinomas should be treated by selective dissection of suspected cervical lymph nodes. Surgical intervention of undifferentiated and medullary carcinomas should be followed by unilateral or bilateral neck-dissection. The survival rate is shown to depend on histological stage and extension of the tumour.

[Mass screening for goiter under a public x-ray program].

The photofluorograms obtained during the 1976 public x-ray screening programme in the urban region of Rostock were scrutinized for dilations of the mediastinum and displacements of the trachea which might indicate strumae. Suspicious x-ray findings were verified scintigraphically in 42 of 117 cases. Tracheal constriction and displacement were found in 28 patients. Except for two suffering from hyperthyroidism, all patients were euthyroid on the basis of the in vitro diagnosis. 14 of 18 "cold" nodes and regions were clarified cytologically. Altogether, 21 patients were treated by hormone substitution, 4 by surgery and 1 (female) by radio-iodine therapy. This shows that public x-ray screening programmes are a suitable tool for identifying treatable strumae regardless of age for little additional cost.
