[Attempts of orbit irradiation after enucleation of the eye with malignant choroidal melanoma. Part I].

PURPOSE: The problem of orbit irradiation after enucleation of the eye with choroidal melanoma is controversial. We have decided to analyse our own material in order to estimate the effectiveness of this method. MATERIAL AND METHODS: The clinical material comprised 202 patients, 97 women and 105 men, in the age of 15-84 years, whose eyeballs were enucleated because of choroidal melanoma. In 72 patients the orbit was irradiated after enucleation with 60Co applicator (CKA4). The dose was about 50 Gy, 5 mm deep. The height of tumour, its location, histological type, infiltration of the sclera or beyond the eyeball and the treatment of tumour before enucleation were analysed. The follow-up time was 5-20 years. RESULTS AND CONCLUSIONS: The survival time of patients in the age below 30 years (p < 0.05) and of patients with choroidal melanoma of the height above 3 mm (p < 0.01) was significantly longer when the orbit was irradiated. Also the survival time of patients with scleral infiltration and with spindle-cell type of tumour was longer (but statistically not significantly) in those, whose orbits were irradiated after enucleation. Exenteration of the orbit was necessary in 4 cases not irradiated after enucleation, only in 1 case after irradiation. The probability of survival after irradiation of the orbit was significantly higher than in cases not irradiated (0.6971 vs. 0.6219). The estimated mean survival time (in months) was longer, but not significantly, in patients after irradiation of the orbit (197.017 vs. 181.409). We conclude that irradiation of the orbit after enucleation of the eye with choroidal melanoma should be recommended. Further investigations will be continued with collaboration of Institute of Oncology in Cracow.

[Survival rate of young patients with malignant choroidal melanoma after conservative treatment].

PURPOSE: The purpose of this study was estimation of the results of treatment and probability of survival in patients with malignant choroidal melanoma at the age under 40 at the time of diagnosis. MATERIAL AND METHODS: 110 patients were treated, including 61 women and 49 men. The follow-up time was 5-25 years, in the diagnosis several different methods were used: ophthalmoscopy, ultrasonography, transillumination, and in some cases additionally computer tomography, magnetic resonance imaging, and fluorescein angiography. All patients were also periodically tested for metastases. The following methods of treatment of melanoma were applied: xenon-arc photocoagulation (21 eyes), brachytherapy (33 eyes), combined treatment (45 eyes). In 58 patients enucleation, and in 2 exenteration were necessary. RESULTS AND CONCLUSIONS: Positive results of treatment were obtained in 48 patients (43.7%), 24 patients died because of metastases. The chance of survival was smaller in women, in cases of greatest dimension of the tumours, when the tumours were located in peripheral areas of the eye fundus, when the tumours infiltrated the optic nerve, and in epithelioid cell type of tumour.

[Genetic markers and congenital cataract].

In this work an attempt has been made to analyze the relationship between genetic markers and the occurrence of congenital cataract in children. The study included 32 families with 66 children in whom various clinical forms of congenital cataract had been diagnosed. In all examined patients, genetic markers such as ABO, MN, Rh systems, Gm1 factor, acid phosphatase (ACP1), esterase D and haptoglobin group were determined. The results were compared with the control population. It was found that the frequency of occurrence of heterozygote phenotype Hp 2-1 is higher in families with congenital cataract with simultaneous decrease of the frequency of occurrence of homozygote Hp 2-2. The obtained data were compared with those of other authors.

[Analysis of complications in patients with malignant choroidal melanoma treated with Co60 and xenon-arc photocoagulation].

The authors observed for 5-18 years 157 patients with malignant choroidal melanoma, age 19-80. Among them there were 57.96% men and 42.04% women. The treatment consisted in irradiation of the tumours with Co60 plaques according to Stallard's methods, and additionally, in the majority of patients, in xenon-arc photocoagulation. Complications developed after treatment were diagnosed. The most common complication was cataract, which occurred in 118 eyes (75.2%), average 2.5 years after irradiation, most commonly in the eyes where the tumour was located between the optic disc and the equator. Second most common complication were various lesions of vessels in retina and hemorrhages, which occurred in 105 eyes (66.9), average one year after irradiation, most often in the eyes with tumours of medium size. Another type of complications, retinopathy or maculopathy, was found in 99 eyes (63%), average 14 months after the beginning of the treatment. These lesions occurred more often in younger patients. The fourth complication was consecutive glaucoma. It was diagnosed in 71 eyes (45.2%), most often 1 to 5 years after irradiation. In many cases glaucoma was combined with other complications, which resulted in enucleation of 62% of ill eyes. The fifth complication were hemorrhages to vitreous. They occurred in 35 eyes (22.29%), much more often in the cases where photocoagulation was performed. The authors found glaucoma to be the most dangerous complication of the conservative treatment of choroidal melanoma, often leading to the loss of the eye. Other complications have negative influence on visual acuity. One should expect that application of other methods of irradiation and lower doses will limit the number of complications.

Studies on absorption and elimination of drugs. Part VI: Fitting of a double exponential curve to the observed drug concentration after single extravascular administration.

A simple program was written in Algol 1204 for the Polish digital computer ODRA 1204. It permits the fitting of the Bateman equation C equals const [e-KEt -- e-KAt] or the equation C equals Be-KEt -- Ae-KAt to the observed plasma or serum concentration data after single extravascular administration of a drug. The results obtained with this M-31 program, compared with the calculations of the pharmacokinetic parameters for tetracycline HCl obtained with the Wagner's Nonline or Lag Time program, show a good conformity.

Bone and joint pathology in fluoride-exposed workers.

Clinical and radiological investigations were performed for 2,258 aluminum workers exposed to fluoride for an average of 17.6 yr (standard deviation = 7.6). Changes in bone and joints were presented in detail in three groups: (1) exposed up to 5 yr (135 cases), (2) exposed from 6-32 yr (1,463 cases), and (3) retired workers (660 cases). A semiquantitative assessment of early fluorosis was introduced. A 20.2% incidence of fluorosis was found, but, according to Roholm, only 1.05% was in stage I. The disease was mainly in the pre-stages of O and OI. A close relationship between the occurrence of fluorosis and the time and degree of fluoride exposure was found. The difficulties in diagnosing skeletal fluorosis result from the questionable sensitivity of the x-ray techniques and from the nonspecificity of the associated symptoms. A quantitative method to assess osteosclerosis and bone structure alteration is needed.
