[Clinical evaluation of the arterial system in hemophiliacs as a model for long-term anticoagulation].

Issuing from the question of the value of a prophylactic anticoagulation for the purpose of prevention of arteriosclerosis 25 patients with haemophilia were examined with the aim to detect possible connections between coagulation potential and arteriosclerosis taking into consideration factors of risk and age. Indeed, also in these patients who were "anticoagulated" from birth factors of risk and references to an existing arteriosclerosis were found. Correlation analyses, however, indicated tendencies, which suggest a protective influence of the hypocoagulability concerning factors of risk and arteriosclerosis.

[Results of in vivo analysis of injected fibrinogen degradation products and factor VIII in patients with hemophilia A].

Seven patients with haemophilia A CP were injected in a dosage of 1 to 3 double units, which were stored for 127 days partly deep-frozen, partly lyophilized and after this time contained fibrinogen fission products (16--256 microgram/ml). Though in two patients after the transfusion positive WA of the injected fibrinogen fission products were established, a relevant influence on the fibrinogen fission product levels of the patients is to be expected only after administration of larger quantities of such preparations. The therapeutic efficacy of the stored CP was insufficient, since despite good in-vitro-activities of factor VIII the increase of factor VIII in four patients was slight and retarding and did not take place in three patients.

[Physiotherapeutic early mobilization of patients with myocardial infarct].

It is reported on the experiences of the programme of the physiotherapeutic treatment of the patients with myocardial infarction in the phase of hospitalisation, which was used in our clinic in 128 patients since 1972. Under careful control the period of mobilisation could be shortened without larger endangering for the patient and a new temporary step scheme could be given as guiding principle, the use of which, however, always needs the individual adaptation by physician and physiotherapist. The physiological and psycho-social advantages of an early mobilisation are clear.

[Assessment of granulopoietic function reserve of the bone marrow by means of the Pyrecol-test in granulocytopenias, immature-cell leukoses and patients under antineoplastic agent therapy].

By injection of the irritating substance Pyrecol the granulopoietic reserve of the function of the bone marrow can be judged. In normal test persons the result is after 8 hours an increase of leucocytes to 176%, when the granulocytes increase to 216%. On the other hand the rate of increase in patients with undifferentiated cell leucosis is statistically highly significantly lower (2alpha = 0.01) with 115 or 132%, respectively. A higher ejection function of the bone marrow correlating with the clinical improvement is to be found after a successful therapy with cytostatics. By this the test informatively contributes to dosage and effectiveness of the therapy with cytostatics and to the course of the disease. In 6 patients with granulocytopenia of unclear genesis examined in the result of the Pyrecol-test 5 passager granulocytopenias with favourable clinical course unambiguously distinguished themselves from a pan-myelophthisis with exitus letalis. The test is easily to be performed and has no essential concomitant a-pearances. By the integral functional evidence it represents a rational supplementation of the locally demarcated morphological substrate of the sternal puncture or bone marrow biopsy, respectively.

[Problems of idiopathic congestive cardiomyopathy].

It is reported on the course of an idiopathic congestive cardiomyopathy in a younger man which could be observed during three years. After a longer period free of symptoms acutely appeared the signs of a myocardial failure, in which case disturbances of cardiac rhythm at length caused death. In detail the clinical fingings, pathologo-anatomical changes as well as the differential-diagnostic considerations are discussed, which are important for the demarcation of the other forms of the idiopathic and secondary cardiomyopathies.

[Cardiomypathies].

Heart insufficiency, cardiomegaly and pathological changes of the ECG after exclusion of coronary heart disease, congenital or acquired vitia cordis and hypertension of the greater or lesser circulatory system result in the diagnosis cardiomyopathy with the liability of further differential-diagnostic demarcation. If causes of a secondary cardiomyopathy can be detected, a causal therapy is often possible, as to the remaining group of the idiopathic cardiomyopathies one must for the time being limit to an exclusively symptomatic treatment (protective therapy, limitation of the fluid supply, glycosides, diuretics, medicamentous and electric therapy of disturbances of the rhythm).

[Differential diagnosis of essential adiposity and Cushings syndrome].

On account of the increasing number of patients with essential obesity the delimitation of the Cushing-syndrome is of actual importance. Two observations of hypercortisolism in hyperplasia of the adrenal glands are reported on and the differential-diagnostically important parameters are demonstrated. According to our opinion anamnesis and clinical findings give the possibility of making a diagnosis already on a large scale. Analyses of the hormones and special X-ray examinations only serve for the preoperative ascertainment and localisation of the endocrine defective function. With the help of literary data causes and possibilities of treatment of Cushing's syndrome are entered.
