Expression of hemin receptor molecule ChuA is influenced by RfaH in uropathogenic Escherichia coli strain 536.

The outer membrane protein ChuA responsible for hemin utilization has been recently identified in several pathogenic Escherichia coli strains. We report that the regulatory protein RfaH influences ChuA expression in the uropathogenic E. coli strain 536. In an rfaH mutant, the chuA transcript as well as the ChuA protein levels were significantly decreased in comparison with those in the wild-type strain. Within the chuA gene, a consensus motif known as the JUMPStart (just upstream of many polysaccharide associated gene starts) sequence was found, which is shared by RfaH-affected operons. Furthermore, the presence of two different subclasses of the chuA determinant and their distribution in E. coli pathogroups are described.

Effect of long-term angiotensin-converting enzyme inhibition on vascular function in patients with chronic congestive heart failure.

The present study demonstrates that peripheral vasodilatory capacity is impaired in patients with chronic congestive heart failure not treated with aspirin, but preserved in patients taking aspirin. This decreased peripheral vasodilatory capacity can be restored by chronic angiotensin-converting enzyme inhibition, indicating that locally acting cyclooxygenase-dependent factors contribute to peripheral vasoconstriction in chronic congestive heart failure.

[Roentgen diagnostic staging program and treatment of subluxation of the talar supinator].

In case of 380 patients (286 operated), who were followed up, we will prove the usefulness of an already some years ago by us recommended step-program with timely, personal and economically defensible expense for opportune and complete diagnosis of frequent fibular ligamentous injuries in the ankle joint. Measurements of x-rays in stress are demonstrated, also principles of treatment including operative-technical details. Only consistent measures offer good prerequisites for healing with restored function.

Acute pancreatitis: secondary findings on hepatobiliary scintigraphy.

Review of hepatobiliary scintigrams in patients with serologically documented pancreatitis revealed scintigraphic abnormalities in 19 of 21 studies (90%) in 19 patients. Abnormalities included duodenal loop widening (14/21 or 65%) and duodenogastric reflux (10/21 or 48%). Total biliary obstruction was seen in five studies, thereby precluding evaluation of the gastrointestinal phase in these patients. Excluding these, duodenal loop widening and duodenogastric reflux were seen in 88% and 63% of patients respectively. We evaluated three patients in whom initial scans showed obstruction, but repeat examination showed resolution of obstruction following passage of common duct stone, with duodenal loop widening and duodenogastric reflux suggestive of acute pancreatitis. Duodenal loop widening as demonstrated by hepatobiliary scintigraphy is a sign of pancreatic enlargement in acute pancreatitis, whereas duodenogastric reflux appears to be an indirect manifestation of an adjacent inflammatory process.

[The importance of early diagnosis of Wegener's granulomatosis: a typical problem case for the otolaryngologist (author's transl)].

Although the etiology and pathogenesis of Wegener's granulomatosis is still obscure, the pathological process is assumed to belong to the group of immunologic diseases. Destructive lesions commonly appear in the midface, particularly in the nose, and are seen microscopically as necrotizing vasculitis. We report a 19 year old woman with undetected disease who was finally diagnosed by repeated biopsies through collaboration between an otolaryngologist and a pathologist. This emphasizes the importance of an early diagnosis for therapeutic reasons since cooperation with an internist prevented the patient from developing gross mid-facial lesions. Treatment involved corticosteroids, immune suppression, and temporary low-dose heparin. During a seven-year period of follow-up, limited tissue changes occurred. Wegener's granulomatosis without treatment will proceed to death, while early recognition and proper treatment can induce long-lasting remissions. Thus, it becomes necessary for head and neck clinicians to become familiar with symptoms of the initial stage of the disease since they must help to secure a proper diagnosis (particularly through biopsies when indicated).
