A new wave guide for use with a CO2 delivery system for laparoscopic surgery.

This study assessed the performance of the Heraeus LaserSonics InfraGuide with the 250Z CO2 laser in 46 patients undergoing laparoscopic surgery. Procedures included vaporization of endometriosis, adhesiolysis, ovarian cystectomy, uterosacral ligament transection, salpingoplasty, fimbrioplasty, neosalpingostomy, ectopic linear salpingostomy and myomectomy. The InfraGuide system was effective in transmitting laser energy to the pelvis. Advantages and disadvantages of the accessory sheath, spot diameter, HeNe aiming beam and optical laser coupler are discussed. The InfraGuide offers the surgeon an additional useful laparoscopic instrument.

Somatomedins in pregnancy: a cross-sectional study of insulin-like growth factors I and II and somatomedin peptide content in normal human pregnancies.

To explore the role of the somatomedins (SM) during human pregnancy, we have measured plasma levels of insulin-like growth factor I (IGF-I), IGF-II, and SM peptide content (SMPC) in 79 women in various stages of normal pregnancies. IGF-I and IGF-II were measured by specific RIAs, and SMPC was measured by a radioreceptor assay using human placental membranes. IGF-I and SMPC rose during pregnancy, showing a significant positive correlation with the length of gestation. Plasma levels of IGF-I in the third trimester averaged 324 ng/ml, a 33% increase over the first trimester average of 243 ng/ml (P less than 0.05). Although IGF-II did not correlate with the length of gestation, the third trimester average was significantly higher than the first trimester average (780 vs. 630 ng/ml; P less than 0.05). After delivery, both IGF-I and IGF-II levels rapidly dropped to levels significantly below those seen in the third trimester. The gestational rise in SMPC and plasma levels of both IGF-I and IGF-II supports the hypothesis that SM play a role in the regulation of fetal growth.

Addressing social, political, and health issues. The student congress strategy.

The authors describe a classroom instructional strategy used to resolve current social, political, and health issues. The strategy includes participation in group work and database searching in the preparation of proposed resolutions. The class serves as a Student Congress, voting for or against the resolutions presented. This instructional model could be used in any university classroom if the goal is to enhance critical-thinking skills and to heighten a sense of social and personal responsibility.

Online critical thinking: a case study analysis.

Educating nursing students using Internet technology, in conjunction with case studies, is rewarding for both faculty and the learner The authors describe an Internet journal discussion format used as an adjunct assignment in a senior clinical nursing course. Adapting a traditional teaching strategy such as case studies proved successful online. Results showed students were able to think critically about a clinical situation and recognize the importance of peer communication.
