Medical students' assessments of skill development in rural primary care clinics.

INTRODUCTION: This study assessed the impact of a rural primary care preceptorship on medical students' self-perceived ability to provide acute, chronic, and preventive care, to perform procedures, to communicate with patients, and to understand the community and healthcare system. METHODS: Students were surveyed about their self-assessed skills on 11 major components (97 items) immediately before and after a 16 week preceptorship in a rural primary care clinic. Responses were analyzed for 96 medical students using a paired comparisons t-test and univariate statistics. RESULTS: Students' skills significantly increased on all components and items. The skills most highly assessed post-preceptorship were those skills related to the management of chronic problems, the provision of patient education and health maintenance, and the ability to handle undifferentiated and acute problems. Among the 11 components assessed, students ranked their skills in performing procedures the lowest. The largest cumulative gain in skills was in the areas of understanding health systems and the community. CONCLUSIONS: This study provides a unique opportunity to look at skill development before and after a rural clerkship. From the student's perspective, the 16 week preceptorship appears to be of significant educational benefit. Future studies need to examine other measures of performance and outcomes of training in rural primary care settings.

Indirect interaction of solid-state qubits via two-dimensional electron gas.

We propose a mechanism of long-range coherent coupling between nuclear spin qubits in semiconductor-heterojunction quantum information processing devices. The coupling is via localized donor electrons which interact with the two-dimensional electron gas. An effective interaction Hamiltonian is derived and the coupling strength is evaluated. We also discuss mechanisms of decoherence and consider gate control of the interaction between qubits. The resulting quantum computing scheme retains all the control and measurement aspects of earlier approaches, but allows qubit spacing at distances of the order of 100 nm, attainable with the present-day semiconductor device technologies.

The examination of physicians' awareness of dementing disorders.

Despite a significant increase in the amount of research being conducted on the etiology, diagnosis, and treatment of Alzheimer's disease and other dementing disorders, there has been little assessment of the utilization of this new wealth of information by the primary care physician. It was hypothesized that there would be a wide variation in physicians' knowledge of dementing disorders and the procedures used to diagnose these disorders. To quantitate the present status of primary care physicians' knowledge of dementing disorders and to investigate possible relationships between this knowledge and different physician and practice characteristics, 50 general internists and family practitioners from Winnebago County, Illinois, were interviewed regarding their recall of causes of dementia, procedures used in diagnosing these diseases, treatment recommendations, and comfort in making a differential diagnosis. Results revealed a wide variation in both physicians' familiarity with the causes of dementia as well as the procedures used in making the diagnosis. Although a majority (80%) of the physicians reported some degree of confidence in making a differential diagnosis of dementing disorders, a significant trend was found between physician age and comfort in making the diagnosis, with age being directly correlated with comfort. This was in spite of a significant negative association between physician age and immediate recall of causes of dementia. These data imply that younger physicians are entering the work force with at least better immediate recall of information on dementing disorders, but they have had little opportunity to become comfortable with that knowledge due to a generally smaller geriatric population in their practice.(ABSTRACT TRUNCATED AT 250 WORDS)

Progression of depression in the prenatal and postpartum periods.

Studies of birth related emotional disorders, which track a cohort of women prospectively throughout the prenatal and postpartum periods, have rarely been undertaken. We measured depression at six separate intervals in the prenatal and postpartum periods among an experimental cohort (n = 57) and at two separate intervals in a nonpregnant control cohort matched on age. Subjects with high levels of prenatal depression were three times more likely to report a past history of depression and two and one-half times more likely to have high depression levels at six weeks postpartum (indicating risk for a puerperal depression). However, they were only one-third more likely to experience transient postpartum depression. Prenatal depression may be another facet of a more encompassing maladjustment which includes past depressive episodes and increased risk for puerperal depression.

Extended Watson integrals for the cubic lattices.

The known exact expressions for extended Watson integrals relating to various cubic and modified cubic lattices are summarized. A new closed form expression for Watson's result on the simple cubic lattice is given in terms of gamma functions.

The unwanted pregnancy in adolescence.

The unwanted pregnancy in adolescence is a crisis during which the adolescent woman often seeks help from an agency or physician. Frequently the option chosen is termination by therapeutic abortion. This option should be offered in a nonjudgmental manner with an attempt to understand and monitor the adolescent's emotional status. Psychiatric consultation should be sought only under specific circumstances is described which will help the adolescent with unwanted pregnancy resolve this life crisis constructively.
