Diffuse lymphocyte-predominant Hodgkin's disease (diffuse paragranuloma). A variant of the B-cell-derived nodular type.

Lymph node sections from 10 cases of mixed nodular/diffuse and 10 cases of completely diffuse lymphocyte-predominant Hodgkin's disease (LPHD) were immunophenotyped. The results obtained were compared with those of nodular LPHD (nodular paragranuloma). In conventional stains, nodular/diffuse LPHD differed from diffuse LPHD in the presence of nodularity, which can be best demonstrated with silver impregnation. Immunohistologic analysis showed a correlation of the difference in nodularity with the presence or absence and pattern of follicular dendritic cell (FDC) meshwork, ie, a relatively sharply defined and large spherical meshwork was present in nodular areas of nodular/diffuse LPHD, whereas FDCs were either absent or present in a diffuse, ill-defined meshwork, usually of small size, in the diffuse zones of nodular/diffuse LPHD and in diffuse LPHD. The amount of FDC meshwork corresponded roughly to the number of reactive B cells and T cells, meaning that in diffuse areas significantly fewer B cells and more T cells were observed than in nodular areas. The immunohistologic analysis also showed that the antigen profile (positivity with the monoclonal B-cell marker L26 in the majority [14/20] of cases and negativity for CD15 in all but one of 20 cases) of the tumor cells in both nodular/diffuse LPHD and diffuse LPHD were comparable while it was different from the antigen profile (L26- and CD15+) in most cases of nodular sclerosis and mixed cellularity types of HD. This suggests that the considered subtypes of LPHD differ mainly in FDC pattern, but not in origin and nature of the tumor cells. This further justifies assignment of the above-mentioned LPHD subtypes to the category paragranuloma (LPHD).

[Small intestine transplantation--a causal therapy in short bowel syndrome].

The problems of surgical technique, graft physiology and immunological reactions in small-bowel transplantation have been investigated in extended animal experiments. In these experiments, the fundamentals of a successful clinical application of small-bowel transplantation could be laid. A successful human small-bowel transplantation could be carried out by the Kiel Group for the first time in 1988. A graft which had been removed from a related donor showed a complete adaptation after 22 months, so that the patient became completely independent from parenteral nutrition. After that, in several cases small-bowel and combined liver and small-bowel transplantation have been carried out. Thus, the clinical small-bowel transplantation represents the causal therapy of short-bowel syndrome and should be developed in further clinical trials.

Immunohistochemical differential diagnosis of granulocytic sarcomas and malignant lymphomas on formalin-fixed material.

A panel of monoclonal antibodies (anti-CD45 [common leukocyte antigen], Ki-B3, L26, MT1, UCHL1, anti-CD15 [X-hapten], anti-neutrophil granule protein elastase [NP57]), anti-lysozyme, and the naphthol-ASD-chloroacetate reaction were applied to two cases of granulocytic sarcoma (GS) for evaluation of their utility in differentiating GS from malignant lymphoma. Lysozyme and naphthol-ASD-chloroacetate esterase were found to be the most reliable markers for detection of the myeloid nature of the tumour cells. GS infiltrated solely the mucosa of the nasal cavity in one case, while in the other it involved both the nasal cavity and maxillary sinus with simultaneous eruptions on the skin of the trunk. In both cases, peripheral blood and bone marrow findings were inconspicuous at the time of diagnosis of GS.

Primary gastrointestinal lymphoma in childhood (up to 18 years of age). A morphological, immunohistochemical and clinical study.

A group of 47 patients up to 18 years of age, with primary non-Hodgkin's lymphoma of the gastrointestinal tract, were investigated. The lesions were located in the stomach (n = 2), small intestine (n = 17), the ileocecal region (n = 20), the large intestine (n = 7), and multifocally in the small and large intestines (n = 1). Of the patients, 41 were male and 6 were female; their age at presentation ranged from 2 to 18 years. All of the cases belonged to the high-grade malignancy group of the updated Kiel classification. Burkitt's lymphoma was the most frequent histological type (n = 35), followed by centroblastic lymphoma (n = 3), immunoblastic lymphoma (n = 2), lymphoblastic lymphoma (n = 1), and large-cell anaplastic lymphoma (n = 1). Five of the patients had high-grade unclassified B-cell lymphoma. Of all the lymphoma types 41 cases (87%) were positive for Ki-B3 (a B-cell marker). Expression of monoclonal immunoglobulin was demonstrated in 8 of 35 cases (23%) of Burkitt's lymphoma, in all 3 cases of centroblastic lymphoma, in both cases of immunoblastic lymphoma, in the single case of lymphoblastic lymphoma, and in all 5 cases of high-grade unclassified B-cell non-Hodgkins lymphoma. Most of the Burkitt's lymphomas showed the light chain lambda (7/8). According to the staging classification of Murphy (N Engl J Med 299:1446-1448, 1978), 12% of the 34 cases available were stage IE, 44% stage IIE, 38% stage IIIE, and 6% stage IVE. Of these 34 patients, 13 died with lymphoma within 1 year after diagnosis. The survival rate for the 19 patients in stages I and IIE at 2 years was 83%, while for the 15 patients in stages III and IVE it was 32% (P less than 0.05). A significant difference in survival was found between the 13 patients with primary involvement of the small intestine and the 20 patients with primary involvement of the large intestine and ileocecal region, the latter showing a better prognosis. Lymphoma type did not significantly influence survival. Our findings indicate that the stage at diagnosis and the primary site are important prognostic features in gastrointestinal non-Hodgkin's lymphoma.

Progressive transformation of germinal centers with and without association to Hodgkin's disease.

Progressively transformed germinal centers occurred in about 3.5% of cases of chronic nonspecific lymphadenitis. They are larger than germinal centers and are composed of follicular mantle lymphocytes, small clusters of proliferating mainly medium-sized B- and T-cells, as well as an extensive network of follicular dendritic cells. Sixty-six patients with lymph node enlargement containing progressively transformed germinal centers and staging and sequential biopsies of 213 patients with Hodgkin's disease (mixed and nodular sclerosis type) were investigated with special reference to the relationship of this lesion to Hodgkin's disease. In most cases, progressively transformed germinal centers developed without any obvious signs of illness and seemed to have no association with Hodgkin's disease. The patients could be differentiated into two groups. The larger group, Group 1 (n = 55 of 66) consisted of patients showing progressively transformed germinal centers without association to Hodgkin's disease. The smaller group, Group 2 (n = 11 of 66) showed progressively transformed germinal centers obviously with association to nodular paragranuloma (Hodgkin's disease lymphocytic predominance type). Progressively transformed germinal centers preceding (n = 3), simultaneously (n = 4), and after development of nodular paragranuloma (n = 4) were found. With regard to subtypes of Hodgkin's disease other than paragranuloma, progressively transformed germinal centers also could be found in sequential biopsies of Hodgkin's disease of mixed and nodular sclerosis type. In one case, progressively transformed germinal centers preceded, in another case they occurred simultaneously in mixed type of Hodgkin's disease, and in two cases of nodular sclerosis type progressively transformed germinal centers developed after the onset of Hodgkin's disease. These findings suggest that progressively transformed germinal centers may be a result of different processes that may be occasionally related not only to nodular paragranuloma, but also in rare cases to nodular sclerosis and mixed type of Hodgkin's disease.

Human cardiac allograft rejection. Correlation of grading with expression of different monocyte/macrophage markers.

Endomyocardial biopsies from human cardiac allografts were investigated with the use of immunohistochemical methods. A correlation between the different degrees of rejection reaction and subtypes of monocyte/macrophage infiltrates was established. Early and mild rejection is characterized by recently immigrated monocytes/macrophages positive for the monoclonal antibody Ki-M1. In contrast, the cellular infiltrates in severe and resolving rejection show strong expression of antigens recognized by Ki-M6 and Ki-M8, indicating increased phagocytotic function of monocytes/macrophages. The immunohistochemical assessment of monocytes/macrophages can thus be used, in addition to conventional histology, in the diagnosis of cardiac allograft rejection.

Primary lymphomas of the lung: morphological, immunohistochemical and clinical features.

Sixty-two cases of primary malignant lymphoma of the lung were investigated. Fifty-eight lymphomas were of B- and two of T-cell type. Two cases of high-grade lymphoma could not be further classified. The largest group (43 cases) consisted of low-grade B-cell lymphoma of the bronchus-associated lymphoid tissue. These showed features similar to low-grade B-cell lymphomas of the mucosa-associated lymphoid tissue of the stomach. The low-grade lymphomas showed a peak occurrence in the sixth decade, the high-grade lymphomas in the seventh decade. Males predominated slightly. Three-quarters of the patients with low-grade B-cell lymphoma of the bronchus-associated lymphoid tissue showed solitary or multiple sharply defined nodules of the lung. The prognosis of the B-cell-derived lung lymphomas without constitutional symptoms was relatively favourable, regardless of whether they were of low- or high-grade malignancy, whereas patients with constitutional symptoms and the two patients with T-cell lymphomas showed a bad prognosis. However, recurrences and metastases in the lung, stomach, lymph nodes and salivary glands were seen in about 46% of the cases of low-grade B-cell lymphoma of the bronchus-associated lymphoid tissue.

Monocytoid B cell lymphoma: clinical and prognostic features of 21 patients.

Twenty one patients between 34 and 83 years of age with monocytoid B cell lymphoma (MBCL) of the lymph node were studied. The histological picture characteristically showed broad strands of medium sized cells with irregularly shaped nuclei and a fairly broad rim of slightly basophilic cytoplasm. One case developed into a centroblastic polymorphic lymphoma. Bone marrow infiltration was documented in three cases and leukaemic conversion in one case of MBCL. Seven patients had enlarged spleens. Eight patients were in stage I, five in stage II/IIE, two in stage IIIs and six in stage IV at the time of diagnosis. Of 18 patients subsequently followed up, eight were in complete remission, two in partial remission, and three were undergoing treatment at completion of the study; five patients had died. Relapse occurred in nine patients and was a common feature of MBCL. The prognosis of MBCL was comparable with that of other low grade malignant lymphomas. Fourteen patients presented with primary nodal lymphoma. In seven patients with nodal MBCL, however, a concomitant low grade B cell lymphoma of the mucosa-associated lymphoid tissue (MALT) was also found in the stomach (n = 4), nasopharynx (n = 1), salivary glands (n = 1) and thyroid gland (n = 1). Two of these cases developed into high grade lymphoma. These extra-nodal manifestations were found simultaneously with MBCL in five patients. In another two patients, however, these symptoms occurred in a later phase of the disease. It is emphasised that adequate staging procedures must be carried out in any case of nodal MBCL to exclude underlying low grade B cell lymphoma of the MALT.

[A new pan-macrophage antibody Ki-M1P stains plasmacytoid cells in paraffin sections of lymph nodes].

Plasmacytoid, T cells (PTC) occurring in cases of chronic non-specific lymphadenitis were investigated using a panel of monoclonal antibodies and cryostat sections. From the typical T cell antigens only CD4 was detectable on PTC. Antibodies directed against myelo-monocytic antigens such as Ki-M2, Ki-M6 (CD68), and Ki-M7 revealed positive reaction with these cells. The recently established monoclonal antibody Ki-M1P reactive with monocyte/macrophages shows a surface and granular cytoplasmic reactivity with PTC. This observation may indicate the myelo-monocytic origin. Ki-M1P detects a formalin resistant antigen and is thus applicable to conventionally processed paraffin sections as demonstrated in twenty cases of hystiocytic necrotizing lymphadenitis (KIKUCHI).

Human monoclonal antibody Ha6D3, a candidate for treatment of leukaemia? In vitro reactivity of Ha6D3 with leukaemic cells and in vivo applications in a chimpanzee.

The human monoclonal antibody Ha6D3 of the IgM type was used to stain malignant lymphoma cells from peripheral blood in flow cytometry and from cryosections of lymph nodes using the immunoperoxidase technique. It was found to react with peripheral white blood cells of all 12 cases of leukaemia and with lymph node cells of seven out of 11 B cell lymphomas and with the one T cell lymphoma tested so far. For in vivo experiments a batch of 70 mg Ha6D3 was purified and 6 mg Ha6D3 was injected intravenously into a chimpanzee with time intervals of 10 months and 1 month. The side effects observed were shivering, some muscular spasms and variations in the heart frequency. A decrease of lymphocytes of more than 50% was documented by haematogram analysis. The flow cytometry data showed that the Ha6D3 antigen does not modulate. Even after three repeated injections applied in a time interval of several months no immune response to Ha6D3 could be detected in vivo or in vitro. Based on these data we suggest that Ha6D3 may become a candidate for the treatment of certain leukaemias in vivo.

Distribution pattern of follicular dendritic cells in low grade B-cell lymphomas of the gastrointestinal tract immunostained by Ki-FDC1p: a new paraffin-resistant monoclonal antibody.

This study surveyed 97 cases of low grade B-cell lymphoma (LGBL) of the gastrointestinal tract (GIT) by conventional morphology and immunohistochemistry, focusing on the most frequent subtype: the so-called LGBL of the mucosa-associated lymphoid tissue (MALT). Special reference was made to the follicular dendritic cells (FDCs) selectively visualized by a new paraffin-resistant monoclonal antibody Ki-FDC1p. LGBL of the MALT accounted for 83 cases. The diagnosis was based on (a) a characteristic cytology, which included centrocytoid cells, a varying degree of plasma cell differentiation, and some blasts; (b) the presence of lymphoepithelial lesions; and (c) the occurrence of two types of follicles easily detectable with Ki-FDC1p. Some were restricted to the mucosa, contained normal germinal center cells, and were indistinguishable from reactive follicles. Others consisted of small clusters of FDCs, were randomly distributed throughout the tumor, and escaped detection in conventional stainings. Such small clusters of FDCs were found to be restricted to LGBL of the MALT, not occurring in other types of LGBL, and were interpreted as tumor-associated abortive follicles discernable from residues of reactive follicles due to their cellular constituents, localization, and distribution pattern. Eight cases showed closed sheets of blasts and were classified as high grade malignant lymphoma secondary to LGBL of the MALT. In two cases the LGBL of the MALT were restricted to the mucosa, in 31 cases the submucosa was also infiltrated, and in the remaining 50 cases the infiltration also involved deeper wall layers of the GIT.(ABSTRACT TRUNCATED AT 250 WORDS)
