[Sturge-Weber syndrome with atypical calcifications].

INTRODUCTION: The syndrome, or disease, or Sturge Weber (SSW) is a neuro-ectodermic disorder of unknown incidence, sporadic presentation and specific sex incidence. It is characterized by the presence of a flat, facial angioma which affects at least the first branch of the trigeminal nerve, association with ipsilateral leptomeningeal vascular anomalies, one or more symptoms (epilepsy, hemiparesia, hemiplegia or mental retardation) and ipsilateral vascular lesions of the choroid which lead to glaucoma. As a consequence of lepto-meningeal involvement, homolateral cerebral hemi-atrophy develops together with cortico-subcortical calcifications with a characteristic "railway line" appearance. CLINICAL CASE: We present the case of a six month old girl with a flat port wine angioma on the left half of her face, including three branches of the trigeminal nerve and the left half of her body. She had partial motor crises of the right leg. On the cranial CT there were left periventricular calcifications and calcifications of the choroid plexus. Gadolinium-MR showed signs of left cerebral hemi-atrophy, which was confirmed on the cerebral SPECT (left temporal hypoperfusion). CONCLUSION: This case is interesting on account of the presence of atypical calcifications, both with regard to the sites and age of presentation. We emphasize the need for cranial CT to rule out the presence of calcifications, (as in this case) not seen on Xray of the skull or on MR. We favour the use of cerebral SPECT as a complementary diagnostic technique.

[Mucin-hypersecretory pancreatic tumors: a pathology with defined characteristics].

We report a case of a mucin-producing tumor of the pancreas, a rare neoplasm of which only a few cases have been described in Europe. The finding of progressive hyperamylasemia without abdominal pain suggests the presence of this type of tumor. At endoscopic retrograde cholangio-pancreatography (ERCP), mucinous excretion and ductal dilation were found. Its prognosis is better than adenocarcinoma.

Extrahepatic bile duct carcinoma: US characteristics and accuracy in demonstration of tumors.

PURPOSE: To assess the accuracy of ultrasonography (US) in the demonstration of extrahepatic biliary tree cholangiocarcinoma and to analyze US features of the different morphologic types of this tumor. MATERIALS AND METHODS: US findings were reviewed retrospectively in 49 patients with pathologically proved cholangiocarcinoma. Findings from percutaneous transhepatic cholangiography (n = 47) and endoscopic retrograde cholangiopancreatography (ERCP) (n = 2) were compared with those from US. RESULTS: US demonstrated the tumor in 47 (96%) of the 49 patients; a mass was demonstrated in 44 patients, and a focal or diffuse thickening of the bile duct wall was seen in three patients. CONCLUSION: The accuracy of US in depicting cholangiocarcinoma was attributable to the skill of the radiologists who performed the study and evaluated the findings and to the high frequency of tumor at the hilar level, where the liver and gallbladder allowed a clear acoustic window that facilitated detection of tumors.

Cystic teratoma of the mesentery.

Cystic teratoma is an unusual cause of mesenteric/omental cysts. A case of cystic teratoma of the mesentery in a 5-year-old girl is reported.

Percutaneous nephrostomy in pyonephrosis.

A series of 76 pyonephrotic kidneys in 73 patients were drained by percutaneous nephrostomy (PN) tube and examined to evaluate the contribution of this technique to the treatment of pyonephrosis. In 71 patients, clinical symptoms disappeared 24-48 h after the procedure. Two patients died from sepsis subsequent to anuria and underlying malignancy. Once the acute phase had remitted, interventional procedures were carried out in 39 cases, and constituted the definitive therapy in 36. In 32 cases, elective surgery was the definitive therapy, including the 3 cases not resolved after interventional procedures. Three patients in whom the obstruction cleared spontaneously following PN needed no additional treatment. Major complications included 6 cases of sepsis, all of which resolved satisfactorily with proper medical therapy.

Carcinoma of the hepaticopancreatic ampullar region: role of US.

Hepaticopancreatic ampullar tumors are so called because they are located at the confluence of the bile duct, pancreatic duct, and duodenum. Jaundice is an early sign of the disease and often leads to early diagnosis and favorable prognosis compared with other tumors that occur in this area. Of eight patients who underwent ultrasound (US) in the past 5 years, six (75%) were found to have tumor. The sizes of the tumors ranged from 1.6 to 2 cm. An intraluminal, polypoid mass in the distal part of the common bile duct was seen in four patients. In the other two patients, a sharply delineated mass gave rise to abrupt termination of the distal duct. Improved US resolution, more experience with this modality, and accurate diagnosis of these tumors with US will contribute to improved detection and prompt treatment.
