Influence of selected variables on adhesion testing.

Three variables associated with adhesion testing were examined: (1) the test mode, shear and tension; (2) design of the tensile apparatus; and (3) tooth substrate, human and bovine. Findings revealed no significant differences in bond strength by one tensile test and a shear test. However, the shear test produced more true adhesive failures; thus it may be preferable to use a shear test for adhesion testing. Bond strengths with some adhesive systems differed significantly when tested by two different tensile apparatus. Design of the test apparatus appeared to affect values. Bond strength measurements obtained with human and bovine enamel were essentially comparable. Values with dentin varied. There appeared to be a trend for higher bond strength values with bovine than with human dentin.

NuLYTELY (PEG 3350, Sodium Chloride, Sodium Bicarbonate, and Potassium Chloride for Oral Solution), a product from Braintree Laboratories, Inc. is a modification of GoLYTELY (PEG 3350 and Electrolytes for Oral Solution) that has been found to have the same therapeutic advantages in terms of safety, efficacy, speed and patient acceptance. This product was developed to improve upon the taste of GoLYTELY. NuLYTELY represents an effective alternative for bowel cleansing prior to colonoscopy that may be more acceptable to some patients.

Dipentum (olsalazine sodium)

Dipentum (olsalazine sodium), a product by Pharmacia, represents the first oral delivery of sulfa-free 5-aminosalicylic acid therapy available for remission maintenance in sulfasalazineintolerant patients with ulcerative colitis. It provides a safe and well-tolerated treatment regimen that has been shown to be as effective as sulfasalazine in maintaining remission of ulcerative colitis.

Engerix-B hepatitis B vaccine (recombinant)/Smith, Kline & French.

Engerix-B [Hepatitis B Vaccine (Recombinant)], a product by SmithKline Biologicals, is a noninfectious recombinant DNA hepatitis B vaccine for immunization against infection caused by all known subtypes of hepatitis B virus. It provides a safe and well-tolerated vaccination regimen shown to confer immunity on the basis of anti-HBs antibody responses comparable with those of plasma-derived vaccines.

Intron A (interferon-alpha-2b recombinant) for injection/Schering-Plough Corporation.

Intron A (Interferon-alpha-2b Recombinant) for injection, a product by Schering-Plough Corporation, is a recombinant DNA, genetically engineered copy of a naturally occurring protein which acts as an antiviral agent. This product is now approved in the United States for the treatment of chronic non-A, non-B hepatitis, also known as hepatitis C. Intron A represents an important advance in the treatment of chronic hepatitis non-A, non-B/C because until now, although many other drugs have been tried, there has been no effective treatment.

Actigall (ursodiol) capsules/CIBA.

Actigall (Urodiol) capsules, a product by CIBA-GEIGY Pharmaceuticals, is an agent intended to dissolve cholesterol (radiolucent, noncalcified) gallstones. It represents a treatment option for nonemergency patients with small-to-medium sized cholesterol gallstones who refuse to undergo surgical removal of the gallbladder or are high-risk surgical candidates.

Sandostatin (octreotide acetate) injection/Sandoz.

The potential benefits of Sandostatin represent an improvement in the quality of living for patients suffering from the symptoms of VIPomas and carcinoid tumors. These patients may have no other therapeutic alternative. Sandostatin may provide these patients with a well-tolerated treatment program that allows them to live a more natural lifestyle. Nurses must be aware of this medication and its administration in order to provide patients requiring Sandostatin therapy with the necessary instruction regarding its use.

Losec (omeprazole/MSD).

The treatment of patients with Zollinger-Ellison syndrome has represented a challenge in the past. Losec Delayed-Release Capsules may provide these patients with a well-tolerated alternative, allowing them a more natural lifestyle. In addition, Losec may also offer benefits to the patient suffering from severe gastroesophageal reflux disease in whom traditional therapy is not providing adequate results. Knowledge of this newer medication and its advantages and disadvantages will allow the nurse to take an active role in patient instruction regarding this therapy.

Cytotec (misoprostol).

NSAIDs have been associated with gastric ulcers and life-threatening complications, including hemorrhage and perforation. Cytotec (misoprostol) is the only drug approved for the prevention of NSAID-induced gastric ulcers. The administration of Cytotec represents benefits, particularly to an elderly population in which the usage trend of NSAIDs is increasing and the induced gastric injury is often silent. The nurse must have an adequate knowledge of this medication in order to provide patients receiving this drug with appropriate patient instruction.

Color stability of composite resins.

The color stability of seven visible light-cured and three chemically-cured composite resins was investigated while being subjected to UV light irradiation and storage in an aqueous environment at elevated temperatures. Color shift was evaluated visually and by colorimetric measurements. Significant correlation was found between visual scoring and colorimetric readings. When subjected to UV light, a wide deviation in color change existed from brand to brand in photocured composite resins. The color shift of chemically-cured composite resins was less than but fell within the range of photocured composite resins. When stored in water at elevated temperatures, photocured resins exhibited better color stability than the chemically-cured composite resins.

Narcan (naloxone HCl).

The gastroenterology nurse must be familiar with the use of Narcan. Narcan is frequently administered in the endoscopy suite after the procedure for the reversal of narcotic depression induced by pre-procedure intravenous sedation. Knowledge of Narcan will allow the nurse to safely administer the medication as well as adequately assess the patient's response to it.

In vitro degradation of cements: a comparison of three test methods.

Three in vitro tests were evaluated on the basis of their ability to predict the relative resistance of various types of luting cements to disintegration in the oral cavity. The tests were (1) weight loss, (2) reduction in size of cement films held between glass optical flats, and (3) reduction in transverse strength. Storage media evaluated were H2O and 0.1, 0.01, and 0.001 M HAc. These data were compared with previously obtained measurements on the in vivo disintegration rates of the same cements. The best correlation with in vivo disintegration was obtained by test No. 2 when the specimens were subjected to 0.01 M HAc.

Beyond the scope: a nursing view of the extraintestinal manifestations of inflammatory bowel disease.

There are approximately 100 systemic complications of inflammatory bowel disease involving most organ systems of the body. A review of the nursing literature demonstrates a wealth of information on inflammatory bowel disease. Most of this literature, however, contains only a brief description of the extraintestinal manifestations of inflammatory bowel disease. It is the purpose of this manuscript to familiarize gastroenterology nurses with the major categories of extraintestinal manifestations seen in association with Crohn's disease and ulcerative colitis. An awareness of these manifestations will allow nursing to play a role in the assessment and care of a patient who demonstrates these conditions. The focus of this paper will be on the clinical presentation and medical management of these conditions along with implications for nursing and possible nursing interventions.

Citrucel (methylcellulose/bulk-forming laxative).

Poor compliance from patients who require the use of a bulk laxative for chronic constipation is cited as a clinical problem. Many patients are dissatisfied with the taste and/or grittiness of psyllium seed products. Citrucel, made from methylcellulose, is completely soluble in water and may result in improved compliance from improved palatability. As with any medication, nursing should take an active role in patient instruction and teaching regarding the use of laxatives. Currently indicated in the treatment of constipation associated with a number of conditions, Citrucel may also be found to have a beneficial effect on serum cholesterol levels.

Write, right now: a guide for the novice nurse author.

The purpose of this paper is to provide the novice nurse author with guidelines to follow for preparing and writing a manuscript. Nurses are responsible for the education of other nurses, and writing and publishing play a major role in providing current knowledge within the profession. By illustrating the mechanics of the writing process, it is hoped that more nurses will approach the task of sharing their ideas on paper.
