Primary carcinoma of the fallopian tube in an adolescent.

Only three cases of adenocarcinoma of the fallopian tube in an adolescent have been reported previously in the literature, and we report the fourth such case. The main presenting symptom in those patients was lower abdominal pain. Type of therapy and outcome of these four cases is reviewed. The small number of patients precludes definitive conclusion on optimal therapy. For this reason, we urge continued reporting of cases to insure the formation of concrete therapeutic recommendations.

False-positive transbronchial needle aspiration in bronchogenic carcinoma.

In a patient with bronchogenic carcinoma, a transbronchial needle aspiration of subcarinal and paratracheal lymph nodes was performed for purposes of staging. On cytologic examination of the specimen, a clump of malignant cells was found; however, subsequent mediastinoscopy and thoracotomy showed no evidence of metastases to mediastinal lymph nodes. We postulate that specimens from transbronchial needle aspiration can be contaminated with tumor cells located on the airway surface epithelium.

Acquired renal cysts and multiple renal cell and urothelial tumors.

This paper reports two cases of acquired renal cysts and multiple renal cell tumors in end-stage kidneys with a discussion of the pathogenesis of these lesions. The first patient had been on maintenance dialysis for approximately three years when he was found to have multiple renal cysts and renal cell tumors. He subsequently developed multifocal urothelial carcinomas of the lower urinary tract. The second patient had progressive renal failure due to hypertensive vascular disease when he underwent left nephrectomy because of multiple renal cysts and renal cell carcinoma. He later developed terminal renal failure and was maintained on chronic dialysis. He expired three years later. At autopsy, the right kidney also demonstrated multiple cysts and renal cell tumors.

Chronic expanding hematomas. A clinicopathologic entity.

Chronic exapnding hematomas occur in many locations, often simulating neoplasms. All have the same structure with a central mass of blood, a wall of granulation tissue, and dense, fibrous tissue at the periphery. The self-perpetuating exapnding nature of the lesion appears to be due to the irritant effects of blood and its breakdown products, causing repeated exudation or bleeding from capillaries in the granulation tissue.

Ovarian pregnancy with delivery of a live baby.

A case of a primary ovarian pregnancy culminating in abdominal delivery of a live baby is presented. The absolute necessity of endometrial tissue for development of the fertilized ovum is discussed. Although in advanced ovarian pregnancy there is no profound difference from normal intrauterine gestation, it may mask the presence of an abruptio placentae.
