[Decision-making for lymph node excision in surgery of thyroid cancer. Extemporaneous examination of the external supraclavicular lymph nodes].

UNLABELLED: Modified neck dissection (MND) is not recommended for surgery of thyroid carcinoma (TC) in the absence of grossly involved nodes, except for medullary thyroid carcinoma, and clinical node recurrence in uncommon at follow-up (3% for us). But several authors report metastatic cancer in non-palpable nodes up to 70% on MND specimens. The fear of overlooking occult metastatic nodes prompted us to sample even normal appearing nodes and to rely on frozen sections (FS) to make a decision whether or not a MND should be done. PATIENTS AND METHODS: 130 among 300 consecutive patients operated for TC were submitted to supraclavicular node sampling with FS. All pathological varieties were covered. In 170 cases, sampling was not done purposely (lack of intraoperative diagnosis of carcinoma: 75) or for other reasons (absence of obvious nodes: 77; unavailable pathologist: 14; miscellaneous: 4). All specimens were reviewed by paraffin sections (PS). RESULTS: Among the 130 patients; 25 had gross metastatic node involvement, confirmed by FS+ and PS+; 1 had grossly equivocal nodes with FS- and PS+; 104 had grossly normal nodes. In 101 (97%) this was confirmed by FS- and PS-. In 3 (3%) FS was +, leading to MND, and PS confirmed metastatic involvement in 2. All 32 specimens of routine node sampling done in 1988 have been reviewed by serial cross sections, one each millimeter (331 sections). One only disclosed one occult metastatic invasion. CONCLUSION: No more than 3% of the grossly normal supraclavicular nodes are metastatic at the time of surgery.(ABSTRACT TRUNCATED AT 250 WORDS)

[Precocious pseudo-puberty in granulosa cell tumor in children less than 1 year old. 2 cases].

Two cases of isosexual precocity in relation to granulosa cell tumor in 2 girls aged 6 and 9 months respectively are described. In the two cases clinical and biological signs of hyperoestrogenemia were found but in one, acute signs of tumoral rupture were the first symptom. Histologically, the two classical subtypes were found: macrofollicular ("adult" type) and "juvenile". These two children are alive without any sign of recurrence with a follow up of 12 and 18 months. The occurrence of granulosa cell tumors in such young girls seems extremely rare.

[Intravascular malignant lymphoma (malignant angioendotheliomatosis). Apropos of a case with histologic, immunohistochemical and ultrastructural studies].

We report a new case of intravascular malignant lymphoma that arose in an 81-year-old woman. The most prominent symptoms were impressive dermatologic anomalies including painful, diffuse edema over which arborescent telengiectasic lesions could be seen. This clinical picture was associated with the presence, in deep skin biopsy specimens, of mononucleate tumor cells located within the lumen of dermal vessels. Complementary immunohistochemical and ultrastructural studies confirmed that the intravascular tumor growth was lymphomatous in nature. Clinical manifestations of this disease are recalled, and histopathologic features as well as the characteristic location of this malignant lymphoma are analyzed and discussed. Thus, we agree with Félix [3] and Witschi [6] that the cords of cells that will give rise to the follicular cells surrounding the ovocytes originate only from the coelomic epithelium; the primary mesenchyma forms the cortical stroma, and this embryonic type tissue, characterized by enourmous physiologic plasticity, differentiates into endocrine tissue to form the inner theca. the mesonephros plays no part whatsoever in the formation of the ovary.

Paraneoplastic intestinal pseudo-obstruction as the presenting feature of small-cell lung cancer.

A previously healthy 50-year old man presented with acute small bowel obstruction. No etiology was found at laparotomy. Postoperatively, the patient remained symptomatic with nausea, vomiting and severe constipation. Gastroscopy revealed retained food in the stomach. Gastric emptying of solids and liquids was dramatically decreased at scintigraphy. The colon was dilated on X-ray study. Chest X-ray revealed a pneumopathy and a small-cell lung cancer was discovered at bronchoscopy. The patient died 5 months after onset. Histologic study of the gut showed widespread degeneration of the myenteric plexus with plasma cell infiltration, Schwann cell proliferation and a reduced number of neurons of which many were abnormal. Intestinal pseudo-obstruction can reveal a small-cell lung cancer; the mechanism of neuronal impairment leading to pseudo-obstruction remains unknown, but could be related to the pathophysiology of paraneoplastic syndromes.

[Byler's disease (familial fibrogenic cholestasis in children). Apropos of 7 cases].

Seven cases presenting with the features of Byler's disease (BD) are reported. The diagnosis of BD can only be made after exclusion of other causes of cholestasis. It seems likely that BD is a syndrome and that more accurate biological and histological investigations will allow to differentiate several entities within this affection. At the present, liver transplantation appears to be the only rational treatment.

[Adenoid cystic carcinoma of the salivary glands. Apropos of 52 cases].

52 cases of adenoid cystic carcinoma of the salivary glands are reported according to the age of the patient, to the localisation near an osseous structure or in soft tissues, to the histologic grade, to the therapeutic management, viz the surgical or radiotherapy treatment, correlated to the survival. It is worthy to notice that the localisations to the palate have a better prognosis than the localisations in soft tissues. Cribriform histologic grade in the same way have a better involvement than basaloid grade, but worse than tubular grade. Effectiveness of radiotherapy does not appear clearly in our study, unlike the observations of some authors.

[Twin pregnancy combining a hydatidiform mole and a normal ovum].

It is rare for a twin pregnancy to consist of a normally growing fetus and a hydatidiform mole. A differential diagnosis between a partial mole and a degenerating fibroid together with a normal pregnancy has to be made. This can be made using ultrasound pictures but also using cultures from trophoblast samples, or cultures from amniotic cells obtained by amniocentesis. These will eliminate the possibility of the triploidy by showing that there is a normal caryotype present. The complications and the risk of degeneration are the same as with a plain hydatiform mole. The uterus has to be evacuated and a follow-up has to be carried out to screen for trophoblastic proliferation.
