Closed septal osteotomy.

A technique is described that permits realignment of the bony septum (perpendicular plate of the ethmoid, vomer, and vomerine ridge). It involves a closed fracture of the bones by means of a large, long nasal speculum. Mere spreading of the septum fractures the bony septum, permitting reasonable, midline realignment in most patients. Spreading with the speculum also infractures the turbinates in many instances and thereby helps open up the airway even further. Of the 32 patients reviewed, it was found that the perpendicular plate and vomer could be fractured in 100% of patients. The vomerine ridge (premaxilla area), on the other hand, could only be fractured in 69%. Of the patients in whom fracture was possible, the bony septum was straightened dramatically in 82% of patients and improved greatly in 18% of patients.

Correlation of compound action potential and electromyography with facial muscle tension.

Functional electric stimulation is a new method for dynamic rehabilitation of paralyzed muscles. The output of such prosthetic devices needs to be modulated by some index of the muscle movement. In facial paralysis a measure of the muscle contractions of the normal contralateral side seems to be an appropriate input. In the rabbit, we simultaneously measured the compound action potential of the buccal branch of the facial nerve, the electromyogram of the zygomaticus major muscle, and the muscle twitch tension through strain gauge. The compound action potential, electromyogram, and strain gauge each had a sigmoidal relationship to stimulus intensity. The compound action potential peak-to-peak amplitude was found to have a linear correlation to the peak twitch tension of the corresponding facial muscle. The electromyogram response, although more variable, also had a linear correlation with muscle contraction. The possibility of predicting the contraction of facial muscles before they actually occur is discussed in the context of available and future functional electric rehabilitation models.

Conservative treatment of cutis aplasia.

Three cases of cutis aplasia are presented. The defects involved include full-thickness defects of scalp and cranium as well as full-thickness skin defects of the abdomen and thigh. All patients were treated conservatively with the use of Silvadene cream dressings. Healing was obtained in all patients.

Treatment of major wound necrosis following total knee arthroplasty.

Following total knee arthoplasty, seven patients developed significant wound necrosis and dehiscence, requiring wound coverage with soft tissue flaps. Three patients had rheumatoid arthritis, three had degenerative arthritis, and one had osteosarcoma of the distal femur. Five different prostheses were used and the wound problems were discovered on average 21 days after arthroplasty. The average wound size was 6.0 cm2. Five were infected, four with Staphylococcus epidermidis and one with Staphylococcus aureus. All patients were treated with antibiotics and local debridement for an average of 10 days prior to the flap procedure. Medical gastrocnemius muscle flaps were used in two patients, unipedicle flaps transposed from the lateral thigh in three, and bipedicle flaps shifted from the medial thigh in two. Flaps were done an average of 56 days after arthroplasty, and knee rehabilitation was delayed an average of 76 days after arthroplasty. Patients were followed an average of 48 months after the flap procedure. Six patients had mild or no knee pain and one who remained infected had moderate constant pain. Three of the patients had greater than 90 degrees of knee motion and one had 75 degrees of motion. The remaining three had only 35 degrees of motion, due in part to significant preoperative contractures, infection, local radiation, and chemotherapy. Late infection developed in two patients at 20 and 45 months following the flap procedure. There was one excellent, three good, two fair, and one poor result using the Hospital for Special Surgery knee rating system at final follow-up examination.
