A method of surgical correction of myopia by fashioning a new optically active epithelial surface: keratoepitheliomileusis.

A new form of refractive keratoplasty for the correction of myopia was developed using a new approach to the problem. The midperipheral annular area of the recipient cornea was deepithelialized, and Bowman's layer was removed using a special bore coated with diamond crumbs. A cryolathed annular graft was placed on this prepared midperipheral area of the recipient cornea concentrically with its center. As a result of subsequent reepithelialization inside the transplanted ring, a regular optically active surface was formed. The refractive effect gradually appeared eight to ten weeks after surgery This new optically active surface acted as a lens, consisting of epithelial cells on top of the host cornea. The method required neither cutting of the stroma nor any manipulation in the optical zone of the cornea. A new term "keratoepitheliomileusis" was coined for this new procedure. Eleven patients (12 eyes) with high myopia ranging from 7.5 to 22.0D underwent keratoepitheliomileusis. The refractive and visual results found after four to 16 months of follow-up are presented. Emmetropic refraction occurred in three patients. In the other nine eyes, the deviation from emmetropia ranged from 0.75 to 2.5D. In all patients, uncorrected visual acuity was improved considerably (up to 0.3 to 1.OD) without loss in their best-corrected vision.

Contact transscleral laser cyclocoagulation in glaucoma.

A new way to treat glaucoma by laser coagulation of the ciliary body was developed. The laser beam is channeled to the eye through the fiberoptic system. The tip of the fiberoptic probe is placed in direct contact with the surface of the eye to be treated. The technical equipment developed for contact transscleral laser cyclocoagulation is simple, practical, and easy to use. The method was investigated experimentally (on animals and on human eyes to be enucleated) and tried in clinical practice. Only patients facing glaucoma surgery (on whom all other methods of conservative therapy failed) were selected for this kind of treatment. The immediate hypotensive effect was approximately 13 mmHg during the first 24 hours; then there was a gradual elevation of the tension, reducing the initial effect on average by 30%. This method has definite advantages over surgical cyclocoagulation as far as safety and simplicity are concerned. The use of contact fiberoptic systems may open up new possibilities in other fields of laser treatment of glaucoma, including techniques to channel the laser beam to the structures of the anterior chamber angle.

[The clinico-morphologic foundation of the modern pathogenetically oriented surgery of hydrophthalmos simplex].

On materials of 45 biopsies dissected at the time of antiglaucomatous operations and 10 eyes of newborns, a detailed morphologic analysis of the state of the drainage apparatus of the in norm and at relatively early stages of hydrophthalmos simplex is presented, and considering results of clinical investigations (105 eyes, of them 45 eyes mentioned) a foundation of modern surgery of congenital glaucoma is given. It is shown that the basis of congenital pathology of retinocorneal angle is disturbance of anatomotopographic relationships of its structures as well as expressed trabeculodysgenesis and dysgenesis of scleral sinus. The paper describes three main variants of combinations of pathologic changes in the drainage apparatus, characterized by a common gonioscopic picture and reflecting the degree of severity of congenital pathology (3 degrees of goniodysgenesis). It is noted that the presence of a coarse congenital pathology of trabecular plexus in hydrophthalmus, incompatible with the possibility of its functioning, and at the same time a satisfactory enough state of the "scleral sinus-emissary" link allow to consider the basic element of surgical reconstruction of anterior outflow passage to be trabeculotomy from outside. Indications in the choice of adequate surgical intervention and its volume are shown (the degree of goniodysgenesis and the state of functional preservation of scleral sinus and emissaries). The approved, within 15 years, system of operations in case of hydrophthalmos simplex and their results are presented.

[Problems of the diagnosis and differential treatment procedures in congenital dacryocystitis].

The paper analyses effectiveness of treatment for dacryocystitis in 223 children, aged from 1 months up to 15 years. In 35 children the process was bilateral. In 232 cases dacryocystitis was congenital, in 26--acquired. Complex examination of lacrimal and upper respiratory tracts has revealed pathology in 53% of cases. In these cases preliminary sanation of upper respiratory tracts was made. A scheme of a staged treatment for congenital dacryocystitis is proposed, and results after treatment by retrograde catheterization in 161 children aged to 1.5 year and by endonasal dacryorhinostomy in 62 children above 2.5 years of life are analysed.

The effect of radial keratotomy on contrast sensitivity.

In the period after surgical correction of myopia and astigmatism in 30 patients (44 eyes) by radial keratotomy, contrast sensitivity deteriorated slightly. The disturbances, however, were transitory, and the general trend was toward restoration of function. Strict adherence to an adequate operative technique prevented a marked decline in contrast sensitivity. The decrease must be considered before surgery, particularly in patients who need a high degree of fine motor control.

Circling keratorraphy: a new approach to surgical correction of aphakia (preliminary communication).

A new approach to surgical correction of hyperopia (including aphakia) is advanced. A ring-shaped inlay of biologically-inert polymer is imbedded into the cornea in order to make its curvature steeper. The corneal tissue is displaced centripetally by constant traction around its optical zone. No corneal grafting or incision in the optical zone is required. The simplest way to carry out the procedure is to place a buried intracorneal suture in a circle 6 to 8 mm in diameter. The suture must be tied strongly enough to ensure sufficient bulging of the cornea inside the circle. Then it will remain buried inside the cornea permanently. The term "circling keratorraphy" was coined for the technique. The results of the first six procedures performed on patients are presented and are considered to be promising. Some improvements in the exactness and delicacy of the technique are required; this work is currently in progress.

Laser cyclotrabeculospasis in the system of laser treatment in glaucoma.

Different methods of glaucoma treatment by laser have their specific indications and taken together can be considered a kind of "system." A new method of so-called laser cyclotrabeculospasis was developed for use in open-angle simple glaucoma. The anterior surface of the ciliary body (seen gonioscopically) is coagulated by a conventional (argon-gas) laser close to the scleral spur. Tractional pull onto the trabeculum and the inner wall of Schlemm's canal is a probable explanation for hypotensive effect of the procedure and the improved aqueous outflow. There is no risk of serious complications. The laser cyclotrabeculospasis was used clinically to limit the use of surgery, but potentially the method may have some advantages over therapy by drugs as well.

Ciliary Body

Technique of implantation of extrapupillary iris lens: 8 years of clinical experience.

A new type of intraocular lens has been devised. The lens is iris-supported but, in contrast to conventional iris-clip lenses, is fixed at the iris periphery, leaving the pupillary region free. The lens may be implanted either anterior or posterior to the iris plane. Implantation into the posterior chamber is particularly beneficial when combined with an extracapsular extraction. Eight years' experience (189 operations) underlines a number of important advantages of the extrapupillary iris lens. The pupil is not distorted, so that its form and function remain normal; the danger of dislocation is avoided; and the lens may be employed in the presence of an iris coloboma. The supporting prongs of the lens are made of gold and platinum alloys, which add considerably to its tissue tolerance. So far no complication has arisen necessitating removal of such a lens. This new type of intraocular lens offers a combination of efficacy, safety, and technical simplicity.
