[The influence of Daclizumab (Zenapaxe) on the postoperative period in allogenic kidney recipients].

The subjects of the study were 48 patients who were administered Daclizumab (Zenapaxe) immediately after transplantation (main group), and 17 controls. The number of infusions depended on the dynamics of the absolute CD-25 number in the peripheral blood, and varied between 1 and 11. The following variables were measured in order to evaluate the influence of Zenapaxe on the course of the postoperative period: 24-hour proteinuria, blood creatinine, steroid pulse number, blood pressure, and the doses of oral corticosteroids. All the patients were examined before discharge and 1 year after transplantation. Comparative analysis revealed the following: 1. In the main group proteinuria was 1.5 times lower 1 year after surgery vs. its level before discharge, while in the controls proteinuria grew up; 2. Steroid pulses in the controls were 4 times more frequent than this parameter in the main group; 3. Blood pressure and blood creatinine in the main group decreased during the first year after surgery, while in the control group the situation was the opposite. Thus, Zenapaxe (Daclizumab) is an effective immunosuppressive agent, displaying positive influence on the course of the posttransplantation period.

[Reparative regeneration of the human liver grafted from a cadaver and a relative donor].

The morphological features and dynamics of regeneration of the grafted human liver were studied by using data on 609 needle biopsies obtained for morphological monitoring in 38 recipients during transplantation of the liver from cadavers and relative donors. The biopsy specimens of donor organs taken prior to grafting served as a control. Irrespective of the type of grafting, regenerative processes in the grafted liver were shown to have common regularities and to run by using the mechanisms of both intracellular regeneration and proliferation. Their maximum rate was seen within a month after grafting of a cadaverous organ and within 3 months after grafting a part of the liver from a relative donor. In cadaverous transplantation, regenerative processes depended mainly on the degree of ischemic lesions; in relative donor organ transplantation, that depended on the fitness of graft mass (the index K being close to 0.5), proliferative processes and the time of higher hepatocytic ploidity increased. An adequate evaluation of the status of a cadaverous organ, estimation of the optimum mass of a grafted hepatic part from a relative donor, and effective immunosuppressive therapy ensure required reparation rates and a complete recovery of the structure of a graft.

[Orthotopic liver transplantation].

Orthotopic liver transplantation is the only method of choice for many severe liver diseases with poor prognosis. The development of liver transplantation programmes is based on medical science achievements and high technology in surgery, anesthesiology and perfusiology. The experience of the Surgery Research Center, Russian Academy of Medical Sciences, made it possible to obtain positive results of liver transplantation and posttransplantation management. This in turn provided longer survival for 80% of recipients. At the same time the mortality of potential recipients was 60.4%, which is due to the greater shortage of donor organs and which is the main problem in the development of organ grafting in Russia. The fact that there is a high proportion of children who are recipients for the donor's liver requires that liver transplantation from living related donors should be developed.

[Heart valve dysplasia as a cause of valvular insufficiency in cardiac surgery (clinico-morphological correlations)].

Dysplasias underlying the mitral and aortic valve diseases in the presence of valvular incompetence were identified in 110 patients with a history of valve replacement. The morphological and electron microscopy evidence together with combinations of valvular changes with Marfan's syndrome and Erdheim's disease, and in some cases with minimal changes in the aorta and peripheral vessels, which followed the type of idiopathic cystic medionecrosis without aneurysm formation support the idea of the dysplastic etiology of the detected abnormalities. The occurrence of a heart disease in the presence of dysplasia is determined by the extent of a valvular process and attending complications, such as chordal rupture or septic endocarditis.

[Reparative regeneration of microvascular anastomosis].

Dynamics of reparative regeneration of the sutural anastomosis of the abdominal aorta has been studied in 90 white mice during the time from 1 day up to 1 year. The operation has been performed by means of microsurgical technique. Histological, histochemical, electron microscopic and radioautographic (3H-thymidine and 3H-uridine labelling) methods have been used. By the 7th day fibrin is organized on the adventitia, the scar along the resection line is formed, endothelial lining and middle layer of the vascular wall are restored. As sources of regeneration in the anastomosis serve cellular elements preserved in the lesion margins. Further maturation of the formed structures and formation of the initial thickening by migrating smooth muscle cells of the middle layer take place. Completeness and typicity depend on preservation of the elastic carcass in the middle layer and adventitia.

[Differential diagnosis of the rejection crisis and toxic and immune-induced nephropathy].

The work is based on the analysis of materials concerning 26 patients with transplanted allogeneic kidneys. All the patients received immunodepressive therapy with a combination of prednisolone and sandimmune. The posttransplantation period was complicated by sandimmune nephropathy in 11 and by a rejection crisis in 15 patients. The following tests were used in the differential diagnosis of these two complications: clinical (firmness of the allogeneic kidney determined by palpation, arterial hypertension, hyperthermia), laboratory (level of creatinine in blood and its clearance, potassium and sodium in urine), and instrumental-functional (ultrasonic examination, electrothermometry, rheography). With the use of these tests, nephrotoxicity of sandimmune origin and rejection crisis were differentiated successfully in all cases.

[Evaluation of the condition of an allotransplanted kidney during the treatment with Sandimmune].

Instrumental-functional examinations (echography, rheography, electrothermometry) were performed in 22 patients with the allotransplanted kidney treated by Sandimmune and in 20 patients treated by standard immunodepression (Azathioprine and corticosteroids). An analysis of the examinations performed has shown reliable differences between the complications which involved the transplant size, cortical layer, skin-temperature gradient, the degree of the venous wave.

[Morphological manifestations of graft functional insufficiency in the 1st 2 months after a human cadaver kidney transplant].

The autopsy and biopsy materials were used for clinico-anatomical comparisons, and three types of changes were distinguished in transplants of cadaver kidneys of man in insufficiency of the function developing in the first 2 months after transplantation: I type--necrotic changes, II--changes resembling the picture of acute renal insufficiency in circulatory shock with retention of the signs of cortical bloodflow and filtration; III--osmotic nephrosis. The nature of the initial function of the transplant reflects not only the features of the rejection reaction but also the degree of ischemia of nonimmune character suffered by the kidney which determines the rate of restoration of its function as well as sensitivity to the rejection reaction and the effect of some therapeutic factors.
