[Circular intestinal and esophageal suture with universal suture device (experimental trial)].

A universal suture device (USD) able to make simultaneous two-line and cross-shaped circular clips stitches on various tube-shaped organs (vessels, intestines, esophagus, stomach, ets), creating end-to-end and end-to-side anastomosis with the external cuff is proposed. The results of experimental trials of USD and its clinical use in anastomosing main vessels were published before (8, 9, 11, 12). In this article the results of experiments with the use of USD in intestinal and esophageal sutures in 35 dogs are analysed. There were 4 series of experiments with 45 anastomoses (31 intestinal and 14 esophageal anastomoses). The anastomosed segments of the intestines and esophagus healed at the level of the lower side of the cuff. The cuff completely resumes its shape in 2.5-5 months, and the clips come off by month 7-8 after the surgery. The mucous layer of the esophagus straightened by the 2-3d month, while adventitial-muscular layer does not straigheten at all, and the clips from it could be found till the 10th month after the surgery. There were no cases of anastomotic incompetence.

Development of autonomous paracorporeal and implanted artificial heart and circulatory assist systems.

The development of autonomous systems for the artificial heart (AH) and for circulatory assistance (CA) has taken several directions. Studies relating to one direction--the development of paracorporeal and implanted systems based on thermal or electric motors--have been conducted at our institute in two stages, moving from stationary systems to totally implanted systems for long-term use. The development of these systems is described. Improvements in the linear solenoid motor, used in some of the first paracorporeal systems, enabled progress toward the development of implanted CA systems based on a reversing electrohydraulic actuator. Modifications were also made in the control system. Early animal testing of these systems indicated that they could be effective in unburdening the heart cavities during left and right bypasses. Full implantation, however, demanded that the devices have improved forms and dimensions and less weight. Smaller electromechanical systems are being developed, and we are experimenting with ways to increase the overall autonomy of the system. An orthotopic model of an implanted AH is promising. Experiments continue on the use of the body's own skeletal muscles to create a blood pump.

[Method of devising a muscle pump for assisted circulation].

The authors have proposed a model of the blood pump from the skeletal automuscles of the body for carrying out aortal counterpulsation under experimental conditions. The pump is run by means of direct electric stimulation of the muscle. Have demonstrated that it is possible in principle to design a completely implanted small system of aortal counterpulsation.
