Enhanced spino-pelvic fixation for segmental spinal instrumentation: a preliminary biomechanical evaluation.

A new method for spino-pelvic fixation of Luque rods was developed and biomechanically evaluated on cadaveric specimens. The technique is surgically simpler and faster than the Galveston technique because it does not require the complex three-dimensional bending of rods to conform to the medullary cavity of the iliac wings. Spines were tested in axial compression in both bilateral and unilateral ischial weight bearing in the intact condition, instrumented with the new technique, and instrumented with the Galveston technique. Compared to the intact spine, the technique increased bilateral average stiffness by 131% and unilateral average stiffness by 192%. Compared directly to the Galveston technique, the new method exhibited increased average stiffness of 15% in bilateral and 11% in unilateral ischial weight bearing. The new method combines the advantages of a simpler and faster surgical procedure than the Galveston technique with enhanced spino-pelvic fixation with the potential to decrease migration of rods relative to each other and the pelvis.

Infected tibial pseudarthrosis. A 2-year follow up on patients treated by the Ilizarov technique.

The Ilizarov technique with a circular external fixator was used in the treatment of infected tibial nonunions. This is a report on 13 patients with a 2-year follow up after removal of the external fixation. There was no debridement of the site of nonunion performed. Union was obtained in all cases. There was no recurrence of infection at follow up. The Ilizarov technique provides the orthopedic surgeon with another alternative in the treatment of complicated tibial nonunions and their associated problems.

The structure and development of osteogenetic repair tissue according to Ilizarov Technique in man. Characterization of extracellular matrix.

Distraction osteogenesis by the method of Ilizarov has permitted the study of bone formation. This is an analysis of 64 human biopsies in patients who were undergoing tibial lengthening by the method of Ilizarov. After analysis by conventional and polarized light microscopy, four stages of bone formation were identified. Bone formation is of the direct type with no cartilaginous phase.

Pseudomalignant myositis ossificans.

PMO is characterized by the following: no history of antecedent trauma, localized expanding soft tissue mass, peripheral calcification surrounding a radiolucent center on plain radiographs and CT scans, microscopic evidence of zonation, and self limited natural history.

Current trends in the treatment of congenital dislocation of the hip.

The best results in CDH are after early diagnosis and treatment. More carefully taught examination techniques of the newborn are important. With the addition of ultrasound examination of the hip, the number of children with CDH diagnosed in the newborn period should increase. The Pavlik harness has had increased use in the child up to 6 months of age. It allows a simple reduction technique for approximately 90% of these children. If skillfully used, the risk of avascular necrosis can approach zero. The early gentle reduction will improve the results of treatment in avoiding avascular necrosis. Skin traction, the human position during the application of a cast, and femoral shortening at the time of open reduction in the child over 3 years of age also contribute to the decreased incidence of avascular necrosis. The early recognition of even the minor forms of subluxation and the treatment with proximal femoral or acetabular procedure is important. This program should increase the number of children reaching adulthood with a concentrically reduced painless hip.

Hip dysplasia, subluxation, and dislocation in cerebral palsy: an arthrographic analysis.

Fifty-nine hip arthrographic roentgenograms were performed on 31 children with cerebral palsy (CP) and plain radiographic evidence of hip deformity. Five levels of hip instability were defined by the arthrographic migration index (MI). The incidence of femoral head deformation increased with an increase in MI. A new measurement, defined by the intersection of Hilgenreiner's line with a tangent drawn from the lateral cartilaginous acetabular anlage, decreased proportionally with the increase in the MI. These findings indicate that progressive deformation of the cartilaginous acetabulum and femoral head occurs during (and, in some cases, before) evolution of hip instability in patients with CP.
