Autoimmune reactions in patients with M-component and peripheral neuropathy.

A study of 17 patients with autoimmune axonal or demyelinating peripheral neuropathy in combination with M-component is described. The M-component was associated with MGUS (monoclonal gammopathy of undetermined significance) in 12 patients, CLL in one patient, Waldenström's disease in one patient, and myeloma in three patients. Immunohistological examination with direct and indirect fluorescence showed binding of antibodies to nerve structures of the same class and light chain as seen in the M-component. In five cases of IgM M-component, the demyelinating neuropathy was caused by binding of the IgM M-protein and complement C3b to myelin-associated glycoproteins (MAG). In 12 cases with axonal neuropathy, binding of IgG to the connective tissue of the peri- and endoneurium was found in 50% of cases, IgM in five cases, and IgD in one case. None of the patients had central nervous system (CNS) symptoms. The clinical and therapeutic difficulties are discussed; only two patients with an acute course responded to immunosuppression. A marked co-expression of other autoimmune phenomena is interpreted in the light of cross-reactions between the autoantibody and similar tissue autoantigens.

Bacteremia and candidemia in hematological malignancies: clinical findings.

171 episodes of bacteremia and candidemia in 142 patients were recorded during the period 1981-1985 in patients with hematological malignancies. Overall mortality, within 1 week of onset of bacteremia, was 20%. Increased mortality was found in patients with poor disease-prognosis (39%), with granulocytopenia for more than 6 days (30%), and in those developing hypotension (49%). Compared to a similar previous study from 1970-1974, the incidence of bacteremia remained unchanged, but the mortality had decreased by 50%. This result could not be explained by changes in microbial spectrum, in patient groups, or in the initial antibiotic regimen used. An improved general condition of these patients due to better supportive treatment may be the single most important factor in improving the outcome of septicemia.

Prediction of survival in adults with acquired Bi- or pancytopenia.

In a retrospective study of 44 adults with acquired bi- or pancytopenia without evidence of any causal disorder, the survival curve suggested the existence of a subgroup of short survivors, mainly with aplastic anaemia, with death within 4 months. The initial values of 14 single clinical, blood and bone marrow variables were significantly associated with survival less than 4 months. Stepwise multiple logistic regression analyses identified 2 combinations of variables displaying significant simultaneous associations with short survival: (i) increased % of non-myeloid bone marrow cells and haemorrhagic manifestations initially; (ii) increased % of non-myeloid marrow cells, circulating erythroblasts and no history of any drug exposure. The predictive capacities of a resulting estimate of the probability of short survival and of previously introduced prognostic indices were approximately equal. The frequency of a correct prediction of a survival shorter than 4 months was in the range of 0.71-0.78 and that of longer survival in the range of 0.74-0.94.

Monoclonal immunoglobulin of CLL lymphocytes detected by radioimmunofixation with 125I-protein A of Triton X-100 solubilized lymphocyte material.

A sensitive radioimmunofixation method (detection limit 1 mg/1 for monoclonal IgM) utilizing 125I Protein A and immunofixation for demonstration of monoclonal immunoglobulin in Triton X-100 extracts of isolated chronic lymphocytic leukemia (CLL) lymphocytes is presented. Thirty-two untreated patients with clinically typical CLL were studied and the results compared to those obtained by immunofluorescence. Monoclonal IgM was detected with similar frequency by the two methods (69% and 75%), but IgD was detected less frequently by radioimmunofixation (25% and 40%). When the results obtained with the two methods were combined, all of the cases were immunoglobulin positive. Two complete IgM type M-components were demonstrated in two cases, an excess of seemingly free heavy chains was found in four cases, and an excess of seemingly free light chains in six cases.
