Dietary carnitine intake related to skeletal muscle and plasma carnitine concentrations in adult men and women.

The purpose of this investigation was to determine if there was any relationship between dietary carnitine intake and the concentrations of carnitine in skeletal muscle and blood plasma in healthy adult men and women. Subjects (14 men, 14 women, fasted 8 h) reported to the Biodynamics Laboratory where they completed a 24-h diet recall questionnaire. Resting muscle biopsy (vastus lateralis) and blood plasma samples were taken and assayed for free, short-chain, and long-chain acyl carnitine concentrations. Dietary carnitine intake was estimated from data on concentrations in food. There was no significant relationship between either protein or carnitine intake with skeletal muscle carnitine concentrations. There was a significant relationship between both dietary carnitine (r = 0.50) and protein (r = 0.48) intake with blood plasma total acid soluble carnitine concentrations (p less than 0.01) in all subjects.

Demographic-economic analysis in a multi-zonal region: a case study of Nordrhein-Westfalen.

"This paper develops a series of spatially-disaggregated demographic-economic models, based on an extended Leontief type input-output model. Travel between residence and place of work and residence and place of shopping is included in the models, and a detailed decomposition of a range of multipliers identified. A model explicitly involving travel to work is developed empirically for the state of Nordrhein-Westfalen in West Germany, and sets of multipliers presented which relate changes in demographic and economic variables in particular zones of the state with changes in economic output and employment-related variables in other zones."

Separation of erythropoietic stimulators in severe anaemia using high performance gel permeation chromatography.

Patients with various types of anaemia, but with comparable haemoglobin levels, show a wide range of serum erythropoietic activity. We have developed a method for the fractionation of serum by HPLC followed by bioassay of the individual fractions, using the mouse spleen cell microassay. Up to three distinct peaks of erythropoietic activity corresponding to molecular weights (MW) greater than 300,000, 250,000-300,000 and 40,000 have been found in serum from both normal and anaemic subjects. The erythropoietic profiles of the sera examined differ markedly in anaemias of different aetiology. The chemical nature and the physiological significance of the stimulators remain to be investigated.

The influence of oxidation-reduction state on the kinetic stability of pig kidney general acyl-CoA dehydrogenase and other flavoproteins.

Pig kidney general acyl-CoA dehydrogenase is markedly stabilized against loss of flavin and activity in 7.3 M-urea or at 60 degrees C upon reduction with sodium dithionite or octanoyl-CoA. Electron transferring flavoprotein is similarly stabilized, whereas egg white riboflavin-binding protein loses flavin more readily on reduction. These and other data support the anticipated correlation between the kinetic stability of the holoproteins and the oxidation-reduction potential of their bound flavins.

The clinicopathological features of metastatic superficial papillary bladder cancer.

A review of the records of more than 1,000 patients with bladder cancer has led to the identification of 9 with superficial papillary tumors and no evidence of muscle invasion in whom distant metastases developed. The majority of these patients had multiple recurrent tumors that were difficult to control by endoscopic means and failed to respond to intravesical chemotherapy, Helmstein's therapy or radiotherapy. Of the patients 4 had superficial tumors in the prostatic ducts but no stromal invasion. In 7 patients with well or moderately differentiated tumors the histological grade was the same in the metastases. The sites of metastases were bone in 5 patients, lung in 3 and liver in 1, which suggested hematogenous spread of tumor. Histological evidence of microvascular invasion was seen in 2 of the resected bladder tumors. We conclude that failure to control bladder tumors by local means should be an indication for early cystectomy, even in patients with no evidence of muscle invasion.

Effect of chemotherapy on the histological appearances of testicular teratoma metastatic to the lung: correlation with patient survival.

We report on 20 patients with teratoma of the testis metastatic to the lungs who underwent surgical enucleation of the pulmonary metastases. Seven patients showed predominantly undifferentiated tumour in the pulmonary metastases; six of these died within 26 months, while the seventh was lost to follow up. Four patients showed only scarring or total necrosis of their pulmonary lesions. These patients all received full courses of chemotherapy before surgery and are alive and well with no evidence of recurrence up to four years after diagnosis. In nine patients the pulmonary nodules consisted entirely of well differentiated tissues of benign appearance, sometimes resembling primary pulmonary adenochondromas. These patients also received full courses of chemotherapy before surgery; one died of disseminated disease but the other eight are all alive and well with survival intervals of up to 14 years after presentation. These findings show that following chemotherapy, necrosis and differentiation of teratomatous pulmonary metastases signify a good prognosis.

Acute renal failure associated with mannitol intoxication. Report of a case.

Mannitol intoxication has been reported as a potentially life-threatening complication when the agent is used unrestrictedly in patients with established renal failure. We report a case of mannitol intoxication in which acute oliguric renal failure occurred without a known predisposing factor other than the intravenous infusion of massive quantities of mannitol.

Total cholesterol and HDL-cholesterol changes during acute, moderate-intensity exercise in men and women.

Chronic endurance exercise training has been associated with decreased levels of total cholesterol and increased HDL-cholesterol. To our knowledge rapid changes in cholesterol and HDL-cholesterol during acute exercise have not been described under controlled conditions. We studied 28 subjects (14 males and 14 females) during bicycle exercise for 40 min at a work intensity of 55% of their maximal oxygen consumption. Total and HDL-cholesterol levels were measured (and LDL-cholesterol calculated) at rest, 10, 20, 30, and 40 min of exercise, and 15 min postexercise. There was a significant (p less than 0.001) increase in HDL-cholesterol levels at 10 min of exercise (58.8 +/- 13.9 mg/dl, mean +/- SD) above rest (53.1 +/- 13.4 mg/dl) for all subjects. This increase persisted (p less than 0.001) at all time points throughout the exercise session, but declined by 15 min postexercise. There was a small, insignificant decline in LDL-cholesterol. It is concluded that apparent favorable changes in lipoprotein patterns occur acutely, and are sustained during short-term, moderate intensity exercise. Analyses of these changes appears necessary if the biochemical mechanisms which underlie these metabolic alterations are to be elucidated.

Effects of acute moderate-intensity exercise on carnitine metabolism in men and women.

The purpose of this investigation was to describe the dynamics of carnitine metabolism during an acute episode of exercise. Twenty-eight subjects (14 male; 14 female) exercised for 40 min on a bicycle ergometer at 55% of their maximal aerobic capacities. Blood samples were obtained at rest, 10, 20, 30, and 40 min of exercise, and 15-min postexercise. Muscle biopsies of the vastus lateralis were performed before and after exercise. Results demonstrated that the percent of acylated plasma carnitine increased significantly (P less than 0.05) across all subjects from 17.3% at rest to 22.3% by 40 min of exercise and continued to increase to 22.8% 15-min postexercise. Total muscle carnitine levels fell significantly (P less than 0.001) across all subjects from 4.21 (1.27) (means +/- SD) mumol/g wet weight at rest to 3.29 (1.27) mumol/g wet weight after exercise. Well-trained males and females had almost identical levels of muscle carnitine [4.35(1.86) and 4.34 (0.64) mumol/g wet weight, respectively]. These levels were somewhat higher but not significantly higher than their moderately trained counterparts [3.86(1.34) and 4.28(1.18) males and females, respectively]. Carnitine palmitoyl transferase (E.C. 2.3.1.21) activity also declined significantly (P less than 0.05) across all subjects after exercise. This study is the first to demonstrate a potential loss of acylated carnitine forms from muscle to plasma during acute exercise, possibly reflecting an increase in carnitine turnover. Alterations in carnitine status may represent another metabolic adaptation to chronic exercise training.

Iron granules in plasma cells.

The curious and unusual finding of coarse iron granules in marrow plasma cells is reported in 13 patients, in whom the finding was incidental. In 10 of these patients there was known alcohol abuse and serious medical complications of that abuse. Previous reports of the finding are reviewed. Haematological data of the 13 patients are presented. A hypothesis is outlined which may account for the finding.

Demographic-economic forecasting within an activity-commodity framework: some theoretical considerations and empirical results.

"This paper presents a methodology which allows demographic and economic forecasting models to be integrated in a consistent manner. By embedding a conventional static Leontief input-output model within an activity analysis framework, a number of interesting results are obtained. First, a new series of production, income, and employment multipliers is derived which offers considerable advantages over those currently in use. Second, partitioning the framework provides fresh insights into the complex relationships between demographic and economic variables. These developments in methodology are tested in a case study of Merseyside where an operational version of the framework is applied in a forecasting context."
