Immunosuppressive acidic protein (IAP) in gastric cancer patients.

Immunosuppressive acidic protein (IAP) in the serum from peripheral, splenic and regional venous blood of gastric cancer patients was assayed. Peripheral venous blood was collected before, and for several months after the surgery. The IAP values of peripheral venous blood, increased with advancing cancer stage; in patients at the same cancer stage, the IAP values of venous blood draining from the cancer lesion were the highest. In curatively operated patients, the IAP values increased during the first postoperative month and decreased thereafter. In patients with non-resectable cancer and in patients subjected to palliative surgery, these values remained high; they increased sharply just before death. Determination of the serum IAP values in gastric cancer patients may be useful in estimating the immunological state of gastric cancer patients and for monitoring the postoperative immunological status.

Gastrectomy↗

Esophageal cancer associated with primary early gastric cancer.

We treated 114 Japanese patients with primary esophageal cancer during a period between 1965 and 1981; and 5 (4.4 per cent) were associated with simultaneous primary gastric cancer. Four of the 5 patients were treated surgically and all of the coexisting gastric cancers were in the early stage. The presence of gastric lesions was correctly diagnosed preoperatively in two of four patients. Surgical reconstruction was done to prepare a new route for food ingestion using a segment of ileo-ascending colon. In the other two patients, gastric tube was prepared, because gastric cancers were detected by chance in the resected specimens in these patients. Based on our experiences, we wish to stress the importance of detailed preoperative gastric examination and of careful intraoperative inspection of the gastric mucosa in patients with esophageal cancer whose preoperative gastric examination provides inconclusive evidence due to the presence of esophageal stenosis.

[Application of heat combined with antineoplastic agent administration in the treatment of cancer (with special reference to malignancy of the digestive system)].

Recently, there is a renewed interest in the efficacy of hyperthermia in human cancer therapy. Based on the results of our experimental studies, we treated far-advanced cancer patients with extracorporeally induced total-body hyperthermia (TBHT) or continuous hyperthermic peritoneal perfusion (CHPP). In the TBHT combined with anticancer chemotherapy, a partial remission was observed in 4 (25.0%) of 16 far-advanced cancer patients. However, the improvement of survival time was unsatisfactory. The CHPP combined with anticancer chemotherapy was performed in 23 gastric cancer patients with peritoneal dissemination. In 6 of them, subjective complaints were improved, but the remarkable prolongation of survival time was not obtained. If in the future hyperthermochemotherapy is performed for cancer recurrence in early stage, more favourable results may be expected.

[Short-term intensive chemotherapy (STIC) combined with intravenous hyperalimentation in advanced stomach cancer].

A short-term intensive chemotherapy (STIC) consisting of large dose of mitomycin C (MMC), tegafur (FT-207), streptococcus preparation picibanil (OK-432) and Krestin (PSK), combined with IVH was performed in 72 patients with advanced gastric cancer. Thirty-three patients were unresectable and 39 patients were recurrent. In this modality, an intensive chemotherapy was able to carry out, maintaining, or increasing in host immunologic parameters and body weight in spite of their disturbed condition in oral food take. Partial response was obtained in 10 of 51 evaluable cases (19.6%) and the survival period on STIC was superior to that on controls. However, no significant difference was observed between the two groups. Four patients had become to be able to take full oral food again by STIC and resulted in the removal of IVH. Hyperglycemia and septicaemia induced by IVH were observed in 9 of 72 patients (12.5%), one died of septicaemia. Full attention should be paid in the management of IVH for patients with advanced cancer.

[Immunological significance of splenectomy in gastric cancer surgery. Production of immunosuppressive factor in the spleen].

We studied immunological significance of splenectomy in gastric cancer surgery with special reference to serum suppressor factor. The results showed that the sera from splenic venous blood in advanced gastric cancer was more suppressive to PHA blastogenesis of normal lymphocytes and also higher in the values of circulating immune complexes, as compared to the sera from peripheral blood. Furthermore, the media from cultures of spleen cells of patients with advanced gastric cancer indicated to suppress PHA blastogenesis of normal lymphocytes, as compared to the media from cultures of peripheral lymphocytes of the same patients. The suppressive activity was more increased in the media from cultures of nonadherent cell population obtained from the spleen cells. From these data, it was suggested that the spleen of patients with advanced gastric cancer was capable of producing serum suppressor factor. Therefore, gastrectomy combined with splenectomy may be reasonable in patients with advanced gastric cancer.

[Clinical practice of systemic hyperthermia therapy and immune reactions of the host].

The effect of extracorporeally induced total-body hyperthermia (TBHT) on host immunocompetence was described. 1) Partial response (more than 50% of tumor regression) was observed in 4 of 13 evaluable patients and pain relief was obtained in 3 of 6 patients who had been suffered from pain before receiving TBHT. 2) In vitro hyperthermia (39 degrees, 42 degrees C) depressed the cytotoxic activity of murine lymphocytes against MH-134 cells as well as PHA-induced blastogenesis. In vitro hyperthermia (42 degrees C) also resulted in a marked decrease in the capability of E-rosetting and PHA-induced blastogenesis of human lymphocytes. However, human lymphocytes blastogenesis was enhanced after in vitro heating at 39 degrees C. 3) E-rosetting capability and PHA-induced blastogenesis of lymphocytes were depressed just immediately after the treatment in patients treated with TBHT, which returned to pretreatment levels within a week. 4) Decreased PHA and PPD skin reactions remained up to 3 weeks after the last TBHT. 5) Serum levels of immunoglobulin, complement and alpha 2-macroglobulin were slightly affected by TBHT, and serum haptoglobin showed a tendency to decrease.

Effect of regional or distant lymph node removal and of splenectomy on immunologic responses in Ehrlich tumor-bearing mice.

To study the role of regional (RLN) and distant (DLN) lymph nodes and of the spleen in the regulation of tumor immunity, we monitored tumor growth in mice subjected to lymph node removal or splenectomy. We found that tumor growth was facilitated when RLN were removed before or soon after tumor inoculation and that RLN removal after extirpation of the immunizing tumor decreased the host resistance to a subsequent challenge. When splenectomy preceded tumor inoculation, tumor growth was not affected, however, marked growth inhibition was observed when splenectomy was performed 5 days after tumor inoculation. Furthermore, splenectomy combined with extirpation of the immunizing tumor increased the host resistance to a subsequent challenge. We conclude from these studies that RLN or the spleen may be of immunologic importance in the host's tumor resistance.

[Combination chemotherapy with neocarzinostatin (NCS) and other antitumor agents for advanced carcinoma of the digestive organs--improved clinical effect with NFO therapy].

We have previously reported the clinical effect of NF therapy (NCS 5,000 units and 5-FU 500 mg intravenously; twice a week) on patients with advanced carcinoma of the digestive system. In the present study, NFO therapy (NCS 2,000 units and 5-FU 500 mg intravenously, Picibanil 1-2K. E. intramuscularly; twice a week) was applied for those patients to take advantage over NF modality. Treated were 62 patients with NFO and 48 were evaluated for its clinical effects. In comparison of NFO and NF, the antitumor effects were noted in 9 of 48 patients (18.8%) for NFO, and 2 of 27 (7.4%) for NF therapy judged by Koyama & Saito's criterion. If the Karnofsky's criterion was applied, I-A category or more were obtained in 10 of 48 patients (20.8%) for NFO and in 2 of 27 (7.4%) for NF therapy. In particular, NFO therapy resulted in the advantageous clinical effects on patients with hepatic and pancreatic carcinomas irrespective of primary or metastatic. The adverse effects of NFO were not more frequent than those of NF therapy. From these results, a new combination regimen, NFO, is thought to bring about a considerable benefit in the treatment of advanced carcinoma of the digestive organs.

Flow cytometric analysis of the DNA content of tumor cells in cases of gastric cancer in the upper third of the stomach and in the remnant stomach.

Thirteen samples of gastric tumors that had developed in the remnant stomach (remnant gastric cancer) were compared with 63 samples of primary gastric tumors located in the upper third of the stomach (primary gastric cancer) by both clinicopathologic and flow cytometric analysis. The depths of invasion of all these tumors corresponded to the mucosa, submucosa, or muscularis propria layers and the histological stages were stages I, II, or III. There was no significant difference between the two groups of samples either histopathologically or clinically (the 5-year survival rates were 74.6% for patients with remnant gastric cancer and 90.4% for patients with primary gastric cancer). DNA aneuploidy was encountered in 23.1% of the cases of remnant gastric cancer and in 33.3% of the cases of primary gastric cancer. Little difference was found in the S-phase fractions between tumors and normal gastric mucosa of the upper third of the stomach and the remnant stomach. Thus, while the environments in the upper third of the stomach and in the remnant stomach are very different, histopathological and biological characteristics of adenocarcinomas that developed in the remnant stomach are very similar to those of adenocarcinomas in the upper third of the stomach.

Correlation between the lymphocytic infiltration of tumors and the proliferative activity of cancer cells from surgically treated esophageal carcinoma.

To evaluate the correlation among the biological behavior of cancer cells, the local immune response of the host, and survival of patients with esophageal carcinoma, we investigated the proliferative activity of cancer cells by immunostaining with the Ki-67 monoclonal antibody in 95 cases of surgically resected esophageal squamous cell carcinoma and compared them with the extent of lymphocytic infiltration of the tumor (LI) in the same specimens. The mean Ki-67 score of 43 tumors with low-grade LI was 46.7% and it was not different from that of 52 tumors with high-grade LI (49.4%, p = 0.441). A significant correlation between the Ki-67 score and the clinicopathological characteristics of tumors (depth of tumor invasion, lymph node metastasis, and stage) was detected. However, in the same stage, the survival of patients with a low Ki-67 score was not different from that of patients with a high Ki-67 score. On the other hand, even no significant correlation between the extent of LI and the clinicopathological characteristics of tumors was found; the 5-year survival rate of 17 patients with high-grade LI (13.9%) was significantly better than that of 15 patients with low-grade LI (0%, p = 0.07) in stage III. These facts indicate that proliferative activity might correlate with the tumor progression, however, when the survival is compared in the same stage, the local immune response such as the extent of lymphocytic infiltration of the tumor might be the better prognostic factor than proliferative activity in patients with esophageal cancer.

The effect of angiotensin II on blood flow in tumours during localized hyperthermia.

The selective enhancement of drug delivery to tumours is an important factor in the effectiveness of thermochemotherapy as well as in standard normothermal chemotherapy. We have attempted to clarify experimentally using AH 100B tumour-bearing rats whether or not a selective increase in blood flow in tumours can be produced under specific conditions of local hyperthermia by administration of angiotensin (AGT II). AGT II (2 micrograms/kg/min) produced an elevation of blood pressure (ca. 150 mm Hg) when local hyperthermia, at 41, 43, and 45 degrees C, was induced. Furthermore, at 41 and 43 degrees C a selective increase in blood flow in tumours resulted from the AGT II-induced hypertension. By contrast, a decrease in blood flow was observed at 45 degrees C both in tumour and in muscle. These results indicate that AGT II-induced hypertension and the resultant selective increase in drug delivery to tumours during the initial phase of heating may result in an augmentation of the anticancer effects of thermochemotherapy.

Attempt to induce total-body hyperthermia by whole-abdominal hyperthermia using a radiofrequency capacitive-heating system: an experimental study in dogs.

The effects of whole-abdominal hyperthermia (WAH), using an 8 MHz radiofrequency capacitive-heating system, on the intraperitoneal and extraperitoneal distribution of heat and on the functions of visceral organs were studied. Eight female mongrel dogs were divided into two groups. Five were infused and three were not infused intraperitoneally with physiological saline. The mean increase in systemic temperature during WAH was 6.4 degrees C for the saline-infused group and 5.1 degrees C for the non-infused group. The systemic temperature reached 40 degrees C about 50 min after the start of heating in four of five animals in the saline-infused group; three of the four animals died, and in these the maximum temperature of the liver exceeded 43.5 degrees C. The infusion of saline into the peritoneal cavity may facilitate the elevation of systemic temperature. The effects on the liver of WAH were very marked, as analysed by biochemical and histological techniques. Visceral organs tolerated heating to less than 43 degrees C by WAH alone. It appears that elevation of the systemic temperature to 40.5 degrees C or more can be safely achieved under conditions where the temperature in the peritoneal cavity is kept below 43.5 degrees C during WAH.

Effects of hyperthermia on the metabolism of 5-fluorouracil in vitro.

The effects of hyperthermia (38-42 degrees C) on the metabolism of 5-fluorouracil (5-FU) were examined using Ehrlich tumour cells (E-cells) and Sarcoma-180 cells (S-180 cells) in vitro. A temperature-dependent elevation of the intracellular concentration of 5-FU was observed in both types of tumour cell after incubation with 5-FU. The levels of 5-fluorouridine (FUR) and 5-fluoro-2'-deoxyuridine (FdUR), which are active metabolites of 5-FU, increased significantly after treatment of cells with 5-FU and hyperthermia. The highest concentrations of these active metabolites were found when the cells were incubated at 39 degrees C. The levels of alpha-fluoro-beta-ureido-propionic acid (FUPA) and F-beta-alanine, which are inactive catabolic metabolites of 5-FU, also increased when the cells were incubated at 39 degrees C. The percentage inhibition of thymidilate synthetase (TS) activity remained high (about 60-70%) at 39 degrees C for 240 min. These results suggest that the optimal temperature for potentiating the intracellular metabolism of 5-FU, in terms of both activation and inactivation, is 39 degrees C in vitro.
