Ultrasonographic features of normalization of the pylorus after pyloromyotomy for hypertrophic pyloric stenosis.

PURPOSE: The purpose of this study was to describe the time course, early postoperative changes, and morphologic features of normalization of the pylorus after pyloromyotomy for hypertrophic pyloric stenosis. METHODS: The subjects were 17 infants (9 boys, 8 girls) who underwent umbilical incision Ramstedt pyloromyotomy. The pyloric muscle mass was measured immediately before the operation and then at intervals from 3 days to 6 months after the operation using a 7.5-MHz ultrasound probe. RESULTS: In longitudinal section, the dorsal part of the pyloric muscle thickened transiently and then thinned to normal values by 5 months after the operation. It was 5.1 +/- 0.8 mm (mean +/- SD) preoperatively, increased to 6.0 +/- 0.3 mm by day 3 after the operation (P <.05), and thinned to 2.8 +/- 0.2 mm by 5 months after the operation. Concomitantly, the length of the pylorus gradually decreased (from 20.1 +/- 2.9 mm preoperatively to 16.9 +/- 2.7 mm by 3 days postoperatively [P <.05] and to less than 15 mm, by 4 months). In transverse section, the muscle normalized as in the longitudinal section. At the site of the incision it was 4.3 +/- 0.4 mm thick preoperatively, thickened to 4.6 +/- 0.4 mm by 3 days after the operation (P <.05), thinned to 2.1 +/- 0.9 mm by 7 days (P <.05), and then increased slightly, but always was less than 3.0 mm. Morphologically, in transverse section, the incised area looked like a wedge by 3 days after the operation. CONCLUSIONS: After pyloromyotomy for hypertrophic pyloric stenosis, there is an early transient increase in muscle thickness within the first few postoperative days followed by a slow decrease that reaches normal thickness (<3 mm) by 5 months. This decrease in thickness is accompanied by a gradual decrease in length to 75% of the preoperative value by 5 months. The morphologic features in this normalization are first a wedge (day 3), then a flat tire (days 7 and 14), and finally an elongated ring (5 months). J Pediatr Surg 36:582-586.

Analysis of 1,992 patients with anorectal malformations over the past two decades in Japan. Steering Committee of Japanese Study Group of Anorectal Anomalies.

BACKGROUND/PURPOSE: This report describes the results of a group study of the Japanese Study Group of Anorectal Anomolies (JSGA) to determine the relative incidence of specific types of anorectal anomaly in Japan, and includes discussion of rectourethral fistula regarding the relationship between the levels of the fistula and blind end of the rectum, low type deformity, rare types, and associated anomalies. METHODS: A total of 1,992 patients (1,183 boys and 809 girls) registered from 1976 to 1995 were analyzed. RESULTS: High-type deformities accounted for 26.0% of cases, intermediate 10.7%, low 57.2%, miscellaneous 4.5% and unclassified 1.8%. The most frequent deformity was male anocutaneous fistula (n = 364), followed by male rectourethral fistula (n = 333), and female anovestibular fistula (n = 241). There were 42 rectovesical fistulas in boys and 93 rectocloacal fistulas in girls. Covered anus complete occurred at the same frequency (10.1% of low deformities) as covered anal stenosis. In rectourethral fistula, the blind end of the rectum lay at or above the level of the P-C line in 40.3% of cases, at or above the M line in 39.6% and at the vicinity of the I line in 20.2%, respectively. There was no parallel relationship between the site of the fistula opening and the level of the rectal pouch. The overall incidence of patients having one or more associated anomalies was 45.2%: 70.6% in high deformity, 60.7% in intermediate, and 31.3% in low. The rate of association of Down's syndrome with deformities without fistula (40.3%) was significantly higher than with deformities with fistula (0.3%). CONCLUSIONS: Rectovesical fistula and covered anus complete were not infrequent deformities in this series. We consider that at least 20% of rectourethral fistula should be categorized as intermediate or low deformity from the viewpoint of the position of the rectal pouch. A significant preponderance of Down's syndrome in the deformities without fistula suggests that further investigation of associated anomalies in comparision with other congenital diseases may provide insights into the pathogenesis of anorectal malformation in the field of molecular genetics.

Bone marrow metastasis of neuroblastoma analyzed by MRI and its influence on prognosis.

Bone marrow metastases (BM mets) of neuroblastoma were analyzed by magnetic resonance imaging (MRI). BM mets were demonstrated in 45 femurs of 23 patients with neuroblastoma by MRI. They were classified into two patterns, nodular (N) type and diffuse (D) type, on MRI, with an incidence of 16% (7/45) and 84% (38/45), respectively. No bone metastases (B mets) were accompanied by N type BM mets, whereas all of them were accompanied by D type BM mets. The rate of disappearance of N type BM mets after chemotherapy was 100% (7/7) and was significantly higher than that (26%, 10/38) of D type BM mets. The rate of disappearance (58%, 11/19) of BM mets in bones without associated B mets was also significantly higher than that (23%, 6/26) of BM mets in bones with associated B mets. BM mets present in bones with B mets tended to persist after chemotherapy. The survival rate of patients with residual BM mets after chemotherapy was significantly lower than that of patients without residual BM mets. This suggests that powerful systematic chemotherapy and additional targeted therapy to lesions remaining after chemotherapy are needed to improve the cure rate of patients with advanced neuroblastoma.

[Two cases of methicillin-resistant Staphylococcus aureus infection].

We have experienced 2 cases of MRSA infection. Case 1: A 16 month-old girl, whose underlying disease was VAHS, had chronic sinusitis. MRSA was isolated from the blood and rhinorrhea. Her sinusitis was very intractable and it was difficult to eradicate MRSA from the nare. Case 2: A 6-months-old girl was admitted to our hospital with urinary tract infection. She had an ectopic uretelocele. Partial nephrectomy and uretectomy was performed. Retroperitoneal and intraperitoneal abscess had recurred three times during 17 months after the first operation. This abscess was slowly progressive and MRSA was very difficult to eradicate. These 2 cases showed characteristics of MRSA infection.

Cytogenetic characteristics of a malignant rhabdoid tumor arising from the paravertebral region. A case report.

The pathological and cytogenetic features of an extrarenal malignant rhabdoid tumor (MRT) arising from the paravertebral region in an infant were investigated. The patient died 4 months after diagnosis, due to aggressive tumor progression. The tumor was composed of medium-sized round cells with cytoplasm containing eosinophilic inclusions, which ultrastructurally were composed of densely packed whorled intermediate filaments. Flow-cytometric analysis of the tumor cells revealed a diploid pattern. Amplification of the N-myc oncogene was not identified. Immunohistologically, the inclusion bodies showed a positive reaction with antiserum against vimentin. The tumor cells were not reactive with antiserum against epithelial membrane antigen, anti-keratin (polyclonal) or cytokeratin (monoclonal, CK1), but did react with 5H10, an antiserum established from human sarcomatous Wilms' tumor. This case is discussed with reference to the literature on extrarenal MRT, placing stress on the histogenesis of this tumor.

Management of defecation in spina bifida.

The main presenting complaint in defecational disorders in spina bifida is constipation. The principal cause of this complaint is hypofunction or paralysis of muscles in the pelvic floor as a result of the spinal cord injuries. To evaluate the present status of defecation, anorectal manometry with special reference to the anorectal pressure profile has been useful. The possibility of using anorectal manometry for the research of anorectal movement and participation of spinal cord was discussed.

Tumor imaging by antineuroblastoma monoclonal antibody and its application to treatment.

Antineuroblastoma monoclonal antibody (MoAb) was labeled with iodine-131 (131I) and injected into transplantable human neuroblastoma-bearing nude mice in vivo. Imaging of the tumor by gamma camera was then attempted. At the fourth day after injection of the antibody, relatively heavy labeling was observed in the tumor. Principal organs were removed and their 131I uptake was determined. A high radioactivity count was detected in the tumor, indicating efficacy of this antibody in tumor imaging of neuroblastoma. This antibody was labeled with 131I and injected into transplantable human neuroblastoma-bearing mice. Apparent inhibition of tumor growth was observed in animals treated as such compared with animals of the control group, MoAb alone group, and 131I alone group (P less than 0.05). In addition, pathohistologic study showed binding of 131I to tumor cells and necrotic changes, suggesting the possibility of application of this MoAb to the treatment of neuroblastoma.

[Diagnosis of anterior perineal anus (APA)--significance of electromyographic evaluation of the external anal sphincter location].

APA is a common cause of constipation, and is the mild case of the imperforated anus. On diagnosing APA, anterior displacement of the anus and normal distribution of the external anal sphincter to the anus are essential. To determine the location of the anus in the perineum simple clinical technique was developed. In 61 normal cases, the result of measurement was almost same in each sex, but in 3 APA cases the location were anteriorly dislocated than normal cases. Distribution of the external anal sphincter was evaluated with electromyographic technique, and location map of the sphincter was made. In 3 APA cases, the anus was totally surrounded by the external anal sphincter, but in 17 ano-cutaneous fistula cases, the opening was anteriorly dislocated to the sphincter distribution map. As a conclusion, newly proposed simple clinical technique to determine anal location and electromyographic examination of the external and sphincter distribution are very useful in objective diagnosis of APA.

[Delayed primary operation of solid tumors in children].

Delayed primary operation has been started since 1982 in our department and 19 cases of solid tumor in childhood were treated with this method. They included 11 cases of advanced neuroblastoma, 5 cases of hepatoblastoma and 3 cases of yolk sac tumor in sacrococcygeal area. We discussed effectiveness of preoperative chemotherapy to the primary tumor and metastatic lesions, the best timing of operation after chemotherapy and the operative technique in delayed primary operation. Preoperative chemotherapy were useful in almost all cases of delayed primary operation, so the primary lesion and lymph nodes metastases were removed easily. We recommended it was the best timing of operation when 2 or 3 courses of preoperative chemotherapy were finished. CUSA (Cavitron Ultrasonic Surgical Aspirator) was the useful instrument to remove the primary tumor and lymph node metastases completely in advanced neuroblastoma preserving both kidneys.

Radioimmunodetection of neuroblastoma with antineuroblastoma monoclonal antibody.

The Antineuroblastoma monoclonal antibody CNM-5 was submitted to preclinical studies and then used for tumor imaging with following results. 1. CNM-5 is an antibody which belongs to the IgM fraction and has a molecular weight of 900 kilodaltons. Its protein concentration was found to be 17.99 mg/dl. 2. The 131I-labeled antibody was injected into nude mice transplanted with human neuroblastoma, and imaging was performed by a gamma camera. Accumulation of 131I in the tumor was relatively clearly seen. Various organs were removed and examined for 131I uptake. Radioactivity was easily detected in the tumor, suggesting that CNM-5 is effective for use in imaging diagnosis of neuroblastoma.

Kinetics of cytotoxic T lymphocytes in stage IV-S neuroblastoma.

The kinetics of cytotoxic T lymphocytes (CTL) in a patient with stage IV-S neuroblastoma were examined for 2 months. CTL were detectable during the preoperative and postoperative periods in this stage IV-S neuroblastoma patient, in spite of the tumor-bearing condition. Furthermore, the patient's mother showed the same level of CTL. This experiment suggests that patients with stage IV-S neuroblastoma have a constantly high level of CTL, and that this immunologic response is predominantly hereditary.

[Effectiveness of cis-dichlorodiammine-platinum in the treatment of advanced neuroblastoma].

Twenty cases of advanced neuroblastoma treated at the Pediatric Surgical Department of Chiba University from 1975 through 1985 were discussed. Cis-dichlorodiammineplatinum (CDDP) and VM-26 were administered to 7 patients with disseminated neuroblastoma resistant to treatment with cyclophosphamide, doxorubicin and vincristine. One complete remission, 4 partial remissions, and 2 minor responses were observed in our series. These results were far better than in our previous study, in which patients were treated with regimens containing cyclophosphamide, doxorubicin and vincristine. Several kinds of side effects were observed in children treated with CDDP. Vomiting occurred in all of them, and bone marrow toxicity was found in 5 among 7 patients. There was one case of severe nephrotoxicity caused by severe proximal tubular dysfunction with increased urinary loss of sodium, leading convulsion.

[Bowel function of spina bifida].

Bowel function of the spina bifida was studied by clinical assessment and anorectal manometric studies. Anorectal pressure profiles were well correlated with clinical assessment but not with recto-anal inhibitory reflexes. Early training of the defecation is emphasized to obtain good bowel function.

[Influence of the catheter diameter on the manometric measurements of the lower esophageal sphincter (LES); with special reference to the application for children].

In order to study the influence of the catheter diameter for measurement of the LES pressure, the LES pressure of rats was measured by using 6 kinds of catheters of different diameters. The results showed that the pressure of LES was 13.8 +/- 3.7 cmH2O, 15.2 +/- 3.2 cmH2O, 18.1 +/- 4.4 cmH2O, 22.3 +/- 6.7 cmH2O, 30.3 +/- 10.0 cmH2O and 42.0 +/- 11.9 cmH2O, respectively with the catheter diameter of 3, 4, 5, 6, 8 and 10 Fr. The larger the diameter of the esophageal tube, the higher the pressure of LES, in the range of more than a 5 Fr. tube. (P less than 0.05) Even in the LES model made of the isolated esophagus of rat, the diameter-pressure curve was similar with the LES measurement of the anesthetized rats. The increased pressure of LES was caused by an increase in tension from the esophageal wall to its proper pressure. For measurement of LES pressure of rats, the catheter of 3 Fr. or 4 Fr. in the outer diameter was more suitable since there was no tension from the esophageal wall. In order to search an appropriate catheter diameter for children, the LES pressure of 11 neonates and 12 infants were either measured with various kinds of tubes. The pressure of LES of neonates was 34.4 +/- 8.4 cmH2O, 36.4 +/- 10.7 cmH2O, 39.6 +/- 10 cmH2O, 46.0 +/- 8.7 cmH2O and 61.8 +/- 22.9 cmH2O, respectively with the catheter diameter of 4, 6, 8, 10 and 12 Fr.(ABSTRACT TRUNCATED AT 250 WORDS)

[Etiological studies of manometric asymmetry in the lower esophageal sphincter (LES)].

In order to study the reason for manometric asymmetry of LES, the manometric profile of LES was measured at 3 different points (at the same level) of 20 normal infants and children by using 5 kinds of 3-lumen tubes with different rigidities. The results were; (1) the pressure was highest at 8 degrees (posterolateral wall) in LES by using all of the 5 esophageal tubes (p less than 0.001), and the length was shortest at 8 degrees in LES with 4 esophageal tubes, except for one 8 Fr. silicone tube. With this tube, LES had no asymmetry in the length. (2) The lower the rigidity of the esophageal tube, the less the asymmetry of the LES. In exact terms, the average coefficient of variations of LES pressure and length were; (in order of rigidity) 28.9% and 24.2% with a 12 Fr. vinyl-chloride tube, 25.7% and 23.4% with a 10 Fr. vinyl-chloride tube, 27.2% and 23.6% with a 8 Fr. evatate tube, 24.5% and 22.0% with a 8 Fr. vinyl-chloride tube, and 12.0% and 7.0% with a 8 Fr. silicone tube respectively. The variation in asymmetry of LES pressure and length were smollest by using the latter tube with the lowest rigidity. Therefore, the results suggest that the asymmetrical profile of LES was caused mainly by the rigidity of the esophageal tube rather than by the anatomical specialty of the esophago-cardiac junction.
