[Effect of music on fetal behaviour].

Antenatal music stimulation shown to elicit fetal heart rate and body movement responses, indicating that prenatal experience with music influences auditory functional development. The slower tempo resulted in less movement variation.

[Therapeutic effects of music on preterm infants in neonatal intensive care units].

The hospital care of premature and low-birth infants requires expensive technology and experienced care. Many studies have looked at the institution of developmental care in the NICU. Significant increases in oxygen saturation as well as decreased levels of agitation and heart rate were found with the use of music. Other studies have shown a doubled daily weight gain when premature babies in the NICU were exposed to music therapy.

[Oxidative stress in preeclampsia].

Preeclampsia is the main case of maternal and perinatal mortality. In women with preeclampsia antioxidant activity of plasma is decreased. Increased levels of lipid peroxidation products may case peroxidative damage of vascular endotelium and result in clinical symptoms of preeclampsia.

[Hemostatic profile of women using low-dose oral contraceptive Mercilon. (Preliminary report)].

The authors have investigated the hemostatic profile at 20 women using low-dosed oral contraceptive Mercilon (Organon) in continuation of six months. Statistically significant changes are found for factor X of hemocoagulation, plasminogen, protein C, protein S and antithrombin III. It is concluded that Mercilon causes activation of the coagulation system, which is balanced by the activation of the fibrinolytic system.

[Our experience in the treatment of climacteric complaints with Livial (Organon)].

The authors are sharing their experience in using a new medicine Livial (Organon) for the treatment of climacteric complaints. Twenty women have been included in the study. They have received one tablet of 2.5 mg Livial daily throughout four cycles of 28 days. All women report decrease of climacteric symptoms, especially hot flushes and night sweats. Only one of them had genital bleeding while using the drug. Twenty percent of the women reported adverse effects (breast tenderness and nausea), which turned out to be transitional. Conclusions concerning the usage of Livial in the treatment of climacteric complaints have been made.

[The anthropometric indices, morbidity and mortality of newborn infants with diabetic fetopathy].

UNLABELLED: A study was carried out to examine the anthropometric characteristics, neonatal morbidity and mortality in newborn infants with diabetic fetopathy. MATERIAL AND METHODS: Infant anthropometric measurements were obtained, neonatal morbidity and early neonatal mortality were determined in 15 neonates with diabetic fetopathy over 6 years. RESULTS: Among the study population, 33.3% of children had macrosomia. The incidence of aspiration syndrome and meconium aspiration syndrome was 40%, transient tachypnea was 33% and the observed incidence of hyaline membrane disease was 26%. The cardiologic pathology was result of patent ductus arteriosus, myocardial hypertrophy and persistent pulmonary hypertension. The most frequent biochemical changes were transient neonatal hypoglycemia and hyperbilirubinemia. The perinatal mortality rate was 26.7%, with cases of hemorrhagic disease of newborn and the incidence of neonatal thromboembolism. CONCLUSION: Despite the current improvements of diabetes care in pregnancy, newborn infants with diabetic fetopathy have a high level of morbidity and early neonatal mortality.

[The use of prostaglandins in prolonged pregnancy and their effect on the newborn infant].

OBJECTIVE: The purpose of this study was to evaluate the neonatal outcome in newborns from mothers with post-term pregnancy and vaginal prostaglandin (Prostin E2) for cervical ripening and labor induction. STUDY DESIGN: This prospective study included 82 newborns, w.g. 41-42, divided into three groups according to the management of post-term pregnancy: group I--preinduction with Prostin E2 before induction of labor with oxytocin; group II--induction with oxytocin only; group III--with expectant management. In each newborn group weight, size, head circumference, Apgar score and neonatal morbidity characteristics were noted. RESULTS: The cesarean delivery rate was 14.2% in group I, 12.9% in the group II and 3.3% in the III-d group. There were no significant differences between groups in the frequency of meconial amniotic fluid, neonatal meconium aspiration. The incidences of severe asphyxia, neonatal injury and pathologic neonatal icter were higher in the group I. CONCLUSIONS: Prostin E2, used for the cervical ripening and labor induction with oxytocin increased the rate of surgical interventions at delivery and neonatal morbidity.

[Human recombinant erythropoietin in the treatment of the severe anemia following labor (a preliminary report)].

UNLABELLED: BUT: The present clinical study was undertaken to evaluate the efficacy, safety and tolerability of r-HuEPO in the treatment of severe anaemia post partum. MATERIALS AND METHODS: Five women received Eprex (Cilag), for five days, and iron and folic acid twice daily. Haematological indices were investigated on the 2d, 5th, 14th and 30th day. RESULTS: On the 30th day the mean haemoglobin concentration was 110 g/l. A rapid haematopoietic effect was shown by an increase in the number of reticulocytes. The peak reticulocytes counts was achieved on the fifth day. The blood pressure at the course of the puerperium was normal. There was no changes in serum electrolytes during treatment with Eprex. The women noted no side effect from rHuEPO. CONCLUSIONS: The use of rHuEPO may decrease the need for red cell transfusions in severe anaemia postpartum.

[The treatment of hirsutism in adolescent girls with spironolactone].

OBJECTIVE: The purpose of this study was to determine the antiandrogenic properties of spironolactone on hirsute adolescents. MATERIAL AND METHODS: Thirty-four adolescents between 15-18 years were studied. The hirsute adolescents were treated with spironolactone from 4th to the 22d day of the menstrual cycle for 8 months. RESULTS: Clinical improvement became evident usually 3 months after initiation of treatment. After 8 months, hirsutism has improved in 24 (70.6%) of 34 adolescents. A decrease in testosterone levels occurred in all patients, even those with initially normal values. CONCLUSIONS: The encouraging results obtained with spironolactone suggest that it may proved to be the appropriate drug for treatment of hirsute adolescents.

[Changes in the concentration of individual proteins in women using current peroral contraceptives].

UNLABELLED: BUT: The aim of the present study was to establish the influence of the moderate combined oral contraceptives on the concentration of the individual proteins in serum. MATERIAL AND METHODS: The study included 70 women at the age from 16 to 30, separated into two groups: control group, including 30 women who have not used OC basic group, including 40 women using Cilest and Marvelon in the period of one to three years. There have been examined the following individual proteins in serum: prealbumin; orosomucoid; haptoglobin; TRA; C3; C4; CRP; IgA; IgG and IgM. Their level in serum was defined with immunoturbidimetric method with monoclonal antibodies of the firm Kone (Fin). RESULTS: The data showed reduced concentrations of Orosomucoid and increased concentrations of TRA in the basic group. CONCLUSIONS: The continuous use of Cilest and Marvelon do not influence the concentrations of the examined individual proteins in the serum. They lead to reduced concentrations of orosomucoid and increased concentrations of TRA.

[Hysterosalpingosonography by the vaginal route--a new method for assessing uterine tube patency].

UNLABELLED: BUT: Introducing a new sonographic method for evaluation the patency of fallopian tubes. MATERIAL AND METHODS: Transvaginal hysterosalpingosonography (HSSG), using chlorocid as a contrast material was performed in 52 infertile women, 12 of with after ectopic pregnancy. RESULTS: Transvaginal hysterosalpingosonography showed as patent 24 fallopian tubes and 68 obturated. LSC and/or HSG pointed 30 patent and 62 obturated. Compared to LSC and HSG, transvaginal HSSG showed 100% sensitivity and 88% specificity. CONCLUSIONS: This investigation revealed the good diagnostic value of HSSG with chlorocid as an easy and not expensive method for the evaluation of the patency of fallopian tubes.

[Ultrasonographic hysteroscopy--new approach for the investigation of the uterine cavity in women with postmenopausal bleeding].

PURPOSE: To assess the value of Transvaginal sonohysterography (SHG) in the detection of endometrial pathology in patients with postmenopausal uterine bleeding, without visualization of the endometrial on the Transvaginal ultrasonography (TVS). STUDY DESIGN: Sixty postmenopausal patients with abnormal uterine bleeding, without sonographic visualization of the endometrium on the Transvaginal ultrasonography underwent sonohysterography, diagnostic hysteroscopy and curettage. RESULTS: After comparing the hysteroscopic and sonohysterographic results, Sonohysterography showed a sensitivity of 100%, specificity of 94, accuracy of 95%, positive predictive value of 85% and negative predictive value of 100%. Hysteroscopy showed a sensitivity of 85%, specificity of 100%, accuracy of 95%, positive predictive value of 100% and negative predictive value of 94%. CONCLUSION: Transvaginal Sonohysterography is an easy, save and the most accurate technique for detecting endometrial pathology and endoluminal masses.

Progesterone challenge test for evaluation of endometrial hyperplasia in postmenopausal women.

One hundred and ninety asymptomatic postmenopausal women were examined by transvaginal sonography. Progesterone challenge test (PCT) was performed in 46 of the women with measured endometrial thickness equal to or greater than 6 mm. PCT positive were 32 of these women and they underwent curettage. Endometrial hyperplasia was detected in 18 women and in 2 of the atypical hyperplasia was verified. The authors conclude that PCT is a convenient and safe screening method for women at high risk for endometrial cancer.

[The choice of the uterine-contraction agent in managing the placental period of labor based on our data].

Every obstetrician is acquainted with the existing risk during the placental stage. The danger of a greater blood loss is always present. The use of uterotonic medicines reduces the post-delivery bleeding. The authors have carried out observations over 422 delivery cases. In 153 of them ergotamine has been applied in the placental stage, in 121 oxytocine, and in 148 delivery cases the placental stage has passed without uterotonic drugs. The authors have concluded that the application of contracting medicines reduces the blood loss, but their usage has to be selective and after delivery of the fore shoulder of the foetus.
