PET findings in a brain abscess associated with a silent atrial septal defect.

Brain abscesses are classical complications of congenital heart disease (CHD) in children and adolescents. This association is rarely observed in adults. We report a 46-year-old man presenting a fronto-parietal abscess associated with an asymptomatic atrial septal defect. Positron emission tomography (PET) study revealed high uptake of L-[methyl-11C]methionine ([11C]methionine) and 2-[18F]fluoro-2-deoxy-D-glucose (FDG) around the brain abscess. We suggest (1) to exclude a silent cardiac malformation in the presence of a cerebral abscess of unknown source occurring in adults; (2) to consider the diagnosis of brain abscess in cases of high uptake of [11C]methionine and FDG in relation to a brain lesion.

Cerebellar hypermetria is larger when the inertial load is artificially increased.

Hypermetria is a classical cerebellar symptom designating the overshoot observed when a patient is asked to make a very fast and accurate movement. The movement studied here was wrist flexion. Hypermetria was found to be increased by artificially increasing the inertia of the moving hand. A normal subject adapted himself to increasing inertia by increasing both his agonist activity (the launching force) and its antagonist activity (the braking force). In the same circumstances, a patient with symptoms of cerebellar system dysfunction was found to be able to increase his agonist activity but not his antagonist activity.

Chronic interstitial cystitis occurring during the shift between rheumatoid arthritis and lupus.

This case reports documents the progressive development of chronic interstitial cystitis during the overlapping process from rheumatoid arthritis to lupus. Concomitantly high titer antinuclear antibody with an anti-68 KD RNP pattern and other biological markers of lupus were observed in the blood. The symptoms dramatically improved under methotrexate therapy. The pathogenic mechanism is discussed.

[Acute motor axonal neuropathy with favourable outcome after Salmonella Ohio infection].

We report one patient presenting an acute weakness following an infection with Salmonella Ohio. Diagnosis was Guillain-Barré syndrome (GBS). However, preservation of reflexes and electrophysiological study showing purely axonal pattern make this case different from typical GBS. Outcome was favourable. There are probably various forms of GBS and the axonal form do not always carry a bad prognosis as previously reported.

[Reversible dementia and polyneuritis in an elderly woman: a case of late-onset disseminated lupus erythematosus].

Reversible dementia and polyneuropathy in an elderly woman: a case of late onset systemic lupus erythematosus. We report the case of a 76-year-old woman with undiagnosed systemic lupus erythematosus. She presented with severe dementia. She was given prednisolone 20 mg daily with dramatic improvement occurring within one month. Electrophysiological study showed evidence of axonal neuropathy.
