Results of pulsed electromagnetic fields (PEMFs) in ununited fractures after external skeletal fixation.

Of 147 patients with fractures of the tibia, femur and humerus, in whom an average of 3.3 operations had failed to produce union, all were treated with external skeletal fixation in situ and pulsed electromagnetic fields (PEMFs). Of the 147, 107 patients united for an overall success rate of 73%. Union of the femur occurred in 81% and the tibia in 75%. Only five of 13 humeri united. Failure to achieve union with PEMFs was most closely associated with very wide fracture gaps and insecure skeletal fixation devices.

[Completely interchangeable joint prosthesis for the hip].

The Authors present a total hip arthroprosthesis studied at Verona Orthopaedic Clinic. This prosthesis presents a complete interchangeability of wear components, not only socket or head. Advantages and premises of this solution are examined and clinical evaluation is referred.

[Use of intraosseous implants in joint prosthesis of the hip].

The authors examine actual problems in total hip endoprostheses fixation and dwell upon solutions used up to date. The authors propose a new technique of prosthethic socket fixation based on endosseous implantology. This method should increase long term stability utilizing at the same time the immediate stability offered by methyl-methacrylate cement.

Girdlestone arthroplasty for loosening of the total hip prosthesis: evaluation and results.

A total of 72 patients (73 hips) who had been submitted to Girdlestone arthroplasty to treat aseptic (63%) and septic (37%) loosening of total hip replacement, and who had been followed-up after an average of 6.8 years, were reviewed. Good pain control was obtained in nearly 60% of the patients. Functional results were poor, but 52% of the patients said that they were satisfied. Mean score based on the Harris Hip Score was 54. Infection was dominated in 92.6% of the cases. The best results were obtained in younger patients, while we did not observe any differences based on the cause of loosening (aseptic or septic). Despite the limits of the method, Girdlestone arthroplasty continues to constitute an effective indication for the treatment of loosening in total hip replacement, when the choice of reimplantation exposes the patient to a high risk of further failure.
