In the Intensive Care Unit of our General Hospital, where an important obstetric-pediatric unit exists, 392 infants have been treated from August 1974 to 1976. Although the results of all age groups are shown, the neonatal one is specially considered. All those patients who recovered and were discharged from the hospital were called for revision and the results of those whom attended are shown. The convinience of the regionalisation and hierarchisation of these intensive care units and the necessity of adequately organise the communication and transport of these patients from the peripheral small units to the regional one, is emphasized.
OBJECTIVE: To analyse the association between housework, burden, health status, and health services use in women (employed and homemakers) in Cornellà de Llobregat (Spain). METHODS: We included for analysis 520 women (employed and homemakers) aged 25 to 64 years old who were interviewed within the 1994 Cornellà Health Interview Survey. RESULTS: Living with elderly persons and being more than 4 members at home was associated with a poorer health status in employed women, who also had a less frequent use of health services when they lived with elderly persons. CONCLUSION: Employed women have a higher probability of declaring a poor health status when their domestic workload is high.
OBJECTIVE: To assess the relationship between socio-demographic factors and smoking status in a population of Barcelona metropolitan area. METHODS: The study included 2,074 questionnaires from the health interview survey carried out in a representative sample of Cornellà during december 1993-november 1994. Only questionnaires of respondents older than 15 were included. A politomic logistic regression was used. Smoking status, classified as never smoker, actual smoker and former smoker, was considered the dependent variable and the sociodemographic factors, the independent variables. Socio-economic status was measured by educational level. RESULTS: The percentage of smokers in men was 46% while in women was only 22%. In men, the probability of smoking was higher in those men whose educational levels were lower (OR: 2.97); in women, no association was found between smoking and education. Unemployment increased the likelihood of being a smoker in both men and women (OR: 1.80). Women did smoke less than men in all age groups and educational levels except for women aged 15-44 with EGB or higher educational level. Educational level was not a significant factor in quitting smoking in either men or women. For both sexes, the greater the age the more likelihood there was of being an ex-smoker. Quitting was not associated with gender in any age or educational group. CONCLUSION: A similar prevalence of smoking in younger women and men of higher educational level suggests that gender differences are diminishing. In men, smoking was associated with educational level. Age is the principal determinant of quitting and neither educational achievement nor gender play a significant role in our study.
INTRODUCTION: The development of secondary lesions in traumatic head injuries seems to be a determinative factor for the survival of these patients. Endothelium damage of cortical microvessels could be fundamental in the main secondary lesions as cerebral ischemia and intracranial hypertension. OBJECTIVES: To investigate which are the main morphological changes that can be observed in cortical microvessels from these patients. MATERIAL AND METHODS: We have studied 15 fresh human brains from subjects died after a severe head injury. The study has been carried out by scanning electron microscopy of vascular corrosion casts and confocal microscopy of histological sections after immunocytochemistry, as well as detection of apoptosis by TUNEL technique. RESULTS AND CONCLUSIONS: The most significant structural alterations were observed mainly on arterioles and capillaries of the middle and deep vascular zones of the cerebral cortex. Corrosion casts showed vessels with longitudinal folds, sunken surface with craters and flattened vessels with reduced lumen. Histological sections immunostained with MAS-336 also showed vessels with longitudinal folds and thinning of their vascular lumen, the presence of cytoplasmic round bodies and a thickening of endothelial cell membrane. TUNEL method revealed a positive staining of some endothelial cells. The structural alterations observed seem to reveal a situation of cellular damage of endothelium in the human cortical microvessels from these patients. It can be thought that this kind of lesions, as well as the secondary functional injury of the blood brain barrier, could play an important role in the development of secondary damage.
We report a case of frontal sinus osteoma in a 66-year-old woman, with headache and deformation of the upper-inner orbit wall; it was satisfactorily solved by means of osteoplastic technique with the aid of surgical microscope. We emphasize the extrasinusal extension, and specially when the osteoma grows towards the orbit or the endocranium, in order to settle on the surgical technique.

We describe a case of generalized muscle rigidity in a 2-year-old patient with Freeman-Sheldon syndrome undergoing surgery for eventration of the diaphragm after anesthetic induction with halothane and succinylcholine. Anesthetic induction was by mask with oxygen, nitrous oxide and halothane, with succinylcholine as a muscle relaxant. Approximately 10 minutes after start of induction, muscular rigidity appeared and developed rapidly, becoming severe and compromising ventilation. Tracheal intubation was attempted without success, owing to stiffness of the masseter muscles. Sodium dantrolene 2.5 mg/kg was administered and relaxation was achieved immediately for both masseter and peripheral muscles, such that the patient recovered spontaneous breathing. We conclude that there is risk of association between Freeman-Sheldon syndrome and malignant hyperthermia.
