[Contraceptive use prevalence in a suburban zone around Sousse (Tunisia)].

A contraceptive prevalence survey was conducted among 1277 women of reproductive age, to explore knowledge of and attitudes to contraceptive use and study factors influencing contraceptive use. We found that family planning methods were generally well known. The prevalence of modern contraceptive use was 53.6%, IUDs being the most commonly used followed by sterilization and pills. The public sector was the main source for obtaining contraceptives although pills and condoms tended to be obtained from the private sector. There was a significant correlation between contraceptive use and urban residence.

[Determinant factors in the choice of site of delivery and the role of peripheral maternity units in a semi-urban environment in Tunisia].

The authors analyse, in the context of a developing country, a number of factors which can determine the choice of site of delivery in a population with access to several possibilities. The role of peripheral (or local) maternity units is considered in particular, in view of the utilisation problems which they raise. This leads to the conclusion of a degree of irrationality in the choice of health care services and the characteristics of women interacting with those of professionals when explaining the choice of site of delivery. Better management of available services is desirable in order to render peripheral maternity units more useful, and improve the medical performance at the time of delivery. Sensitization of the public and above all of health professionals nevertheless remains essential.

[Knowledge, attitude and behavior of Tunisian women apropos of gynecological cancers].

With the aim to analyze population needs in the field of cancer screening (cervical and breast cancer), attitudes, behaviors and knowledge of a Tunisian women group of health service user's were studied. Results clearly demonstrate the necessity to inform the concerned population about risk factors particularly concerning breast cancer (the most frequent cancer in Tunisia) but also early symptoms and available screening methods. This role is alloted to health professionals, more for general practitioners and midwives than for gynaecologists. Health education for women and groups, however, have to go hand in hand with training of health professionals in matter of test screening.

[Breast self-examination practice in Tunisia based on a survey of first-line patients].

As the first phase of a breast cancer screening campaign in central Tunisia, a survey aimed at evaluating the frequency of self-examination of the breasts (SEB) and identifying factors which might influence this practice took place in the Kalâa Kbira region of Tunisia, involving 300 first-line out-patients. Women answered a questionnaire presented by a resident in medicine during the course of the visit. The survey showed that the use of SEB remains slight (28%). Discriminant analysis enabled identification of parameters influencing this practice. Thus age of the patient, educational level of the woman and/or spouse and the source of information were the most significant parameters, in increasing order of importance. The results of this survey should enable definition of the content and form of information to be given to women in order to obtain their participation in a breast cancer screening campaign in central Tunisia.

[Using prenatal care in the Tunisian context: barriers and motivation factors].

Several barriers to and motivators of antenatal care are analysed in the context of a developing country. The authors found that socio-cultural factors are as important as organizational factors in motivating women to seek antenatal care. While social and economic development may improve the quality of life in the long term, it is important to respond now to the needs of pregnant women in order to encourage them to use such services. A comprehensive analysis of certain organizational factors would be useful in achieving this goal.

[Risk factors in cardiovascular diseases in a semi-urban community of the Tunisian Sahel].

Cardiovascular diseases are the leading cause of mortality in the western countries at the same time they are beginning to appear with increasing frequency in developing countries. An epidemiologic study was carried out at Kalaa Kebira (Sousse-Tunisia) to determine the prevalence of the different risk factors of cardiovascular diseases. The study was performed on a representative sample of 555 adults of 20 years and more (response rate of 76%). There was a high proportion of adult males smokers (66%). The prevalence of hypertension varied from 15% to 25%, those of diabetes was 6.5%. An hypercholesterolemia greater than 6.2 mmole/l was observed for 12% of the sample and obesity for 20%.

[Diarrheal diseases treated at the health center of Kalaa-Kebira (Sousse-Tunisia)].

Diarrheal diseases of children constitute a public health problem in Tunisia because of their frequency as well as their association with malnutrition. A national programme for diarrheal diseases control was instituted in 1980. This study focuses on a local health center (the PMI of Kalaa-Kebira in an effort to assess the magnitude of diarrheal diseases of children in 1985, and to see to what extent goals and recommendations of the national programme have been attained.

[Hereditary degenerative spinocerebellar diseases in Tunisia with manometric studies in bladder disorders (author's transl)].

During a survey conducted in Tunisia in 1978, 204 cases of hereditary degenerative spinocerebellar diseases were discovered among members of 117 families. The cases included 109 patients with Friedreich's ataxia, 28 with Piere Marie's heredo-ataxia, 20 with Strumpell-Lorrain's disease, and 47 with intermediary forms. The latter group included incomplete forms of Friedreich's and Pierre Marie's diseases. The onset or progression of the disease was linked to a febrile episode in 25 p. cent of the cases. Emphasis is placed on the presence of bladder sphincter disorders in approximately one third of the patients with Friedreich's or Pierre Marie's diseases. Manometric studies in 17 cases demonstrated the presence of normal bladders in 4 cases, hypertonicity of the bladder in 5 patients, and hypesthesic retention-type bladders in 5 other cases. In 3 patients the disorder was difficult to classify. These results show that sphincter disorders should not constitute a criterium for exclusion of the diagnosis of spinocerebellar degeneration.
