Hyperthermic isolation-perfusion with melphalan, a preliminary appraisal of local and general effects in malignant melanoma.

Ten hyperthermic isolation-perfusions with melphalan were performed for invasive malignant malenoma of the limbs. Four cases at stage II and with in transit metastases (up to 15 cm in diameter) had a complete regression. However, no improvement of survival was noticed: 4 out of 5 stage II patients who died, within 1 year after perfusion, developed a generalized disease. Systemic chemotherapy was administered in 3 patients after perfusion, but they died from distant metastases. The need for an adequate adjuvant treatment added to isolation perfusion is discussed.

PACAP in developing sensory and peripheral organs of the zebrafish, Danio rerio.

The anatomical distribution of PACAP-like immunoreactivity was investigated in sensory and peripheral organs of the zebrafish, Danio rerio, during the pharyngula, hatching and larval periods, by using indirect immunofluorescence methods. First PACAP-like immunoreactive (ir) elements appeared during the pharyngula period, at 24 hours post fertilization (hpf), within the most superficial layer of the retina and the dorsal aorta. At 48 hpf, additional ir cells were found in the olfactory placode and esophagus. At 72 hpf (hatching period), PACAP-like immunoreactivity was first detected in the ganglion cell layer of the retina, the otic sensory epithelium, pharyngeal arches, swim bladder and pancreatic progenitor cells. During day 5 of larval development, new groups of ir cells appeared in the liver, whereas no ir elements were observed in the olfactory placode. Subsequently, at day 13 of larval development, additional ir elements were found for the first time in some gut epithelial cells while those previously observed in the retina and otic sensory epithelium were absent. The transient expression of PACAP-like ir material in sensory organs suggests that the peptide could be implicated in neurotrophic activities and neurosensorial connections in the migration and/or differentiation processes. The appearance of PACAP-like ir elements in peripheral organs at different developmental stages, indicates that this peptide could be involved in the control of more specific functions as soon as these peripheral structures begin to operate.

[Results of a national therapeutic trial in 10,000 hypertensive patients conducted by 2000 general practitioners (author's transl)].

Multicenter study in 10294 hypertensive patients conducted by 2000 G.P.'s throughout France. This trial permitted to compare the effectiveness and tolerance of 3 types of antihypertensive therapy: a beta-blocker, a diuretic and a central antihypertensive agent. It also permitted to study a certain number of socio-epidemiological characteristics in that patient population, in particular with regard to cardio-vascular risk factors.

Evaluation of the end-expiratory lung volume as an indirect index of bronchial constriction in asthma.

The purpose of this study was to evaluate the merits of the end expiratory lung volume as an indirect ventilatory index of bronchial obstruction and to show an application of continuous monitoring of lung volume in asthmatic patients. The accuracy of the external measurements (IS) of functional residual capacity (FRC) was controlled by comparing them with the helium measurements (DS) obtained during nine methacholine tests (IS = 0.06 + 1.065 DS in litres: R2 = 0.99). Seven asthmatics (18-48 yr) were monitored by measuring rib cage and abdominal perimeter variations. This was done in basal condition, after methacholine-induced bronchoconstriction and after bronchodilation by either salbutamol or oxytropium bromide inhalation. All the subjects were investigated on two separate days and were their own control. Bronchoconstriction produced a significant increase (p less than 0.01) of tidal volume (VT: + 67%), external minute ventilation (VE: + 58%), mean inspiratory flow (VT/TI: + 78%) and FRC (+ 26.5%) while frequency (f) and fractional inspiratory time (TI/TT) fluctuated non significantly. In the group of seven tested subjects, there was a significant correlation (p less than 0.01) between forced expiratory volume in one second (FEV1) and VE, FEV1 and VT/TI, FEV1 and FRC. However, the individual regression line showed a significant relationship only between FEV1 and FRC (R2 = 0.80 +/- 0.04). We therefore conclude that the variation of the end expiratory level can be chosen as an indirect index of bronchoconstriction.(ABSTRACT TRUNCATED AT 250 WORDS)

Evaluation of differential linear transformers as a non-invasive ventilatory method in supine subjects.

We present a new method to measure thoraco-abdominal perimeter variations using differential linear transformers inserted in belts. The high sensitivity (250 mV/mm), the low time drift (1 mV/24 h compared to a 1 V output for a tidal volume), the good frequency response (up to 10 Hz) and low cost (12,000 FF) are the major advantages over other devices. After calibration (using change contribution of thorax and abdomen and the least square method for calculation of volume-motion coefficients), estimated tidal volumes in 23 adults in supine position are within 0.06 +/- 5.17% and changes in functional residual capacity (43-453 ml) are within 3.4 +/- 15.3% when compared with direct spirometry. These results show that the method can be used to monitor ventilation accurately in supine healthy subjects.
