[The value of supportive therapy in the treatment of malignant tumors].

Supportive care are a strongly connected constituent of the curative and palliative treatment of cancer. They serve for prevention and early diagnosis, respectively, and treatment of complications during therapy and disturbances which may issue from the tumour disease itself. These are especially the results of the insufficiency of the bone marrow which in the last years increasingly moved into the centre of the therapeutic interest. A survey of the present state of the treatment of these and other general complications in patients with malignant neoplasms is given.

[Clinical aspects of the excretion of alkylating cyclophosphamide metabolites in the urine of tumor bearing patients].

The urinary excretion of alkylating CP metabolites was measured in 33 tumor patients by using NPB-test under consideration of different doses, routes of administration (i.v. and i.m.) and function of the liver and the kidney. There was no limit in activation capacity with increased doses of CP. The estimation of the upper activation limit was hindered by the host's subjective toxicity. CP was activated in a comparable manner after i.v. or i.m. administration. It is of interest that liver involvement by the disease or functionally compensated post-nephrectomy state of patients investigated so far did not diminish the excretion of alkylating CP metabolites.

[New cytostatics and their clinical application (author's transl)].

A comprehensive review of the state of development and clinical evaluation of cytostatic agents is given. Special attention is directed to agents which are known for long a time but whose clinical importance has grown only in recent years. This review takes into consideration the chemical structure, mode of action, pharmacokinetics, dosage, side effects, and results of clinical treatment of the following drugs: adriamycin, nitrosourea preparations, DTIC, dibromdulcitol. trofosfamid and ifosfamid, ftorafur, 5-azacytidin, platinum complexes, ICRF-159, cytembena, HMM, VM 26 and VP 16-213, cytostasan, estramustinphosphat, antibody-cytostatic complexes.

[The paraneoplastic syndrome].

The notion of the paraneoplastic syndrome contains a great number of signs which are not to be explained by the local growth of the tumour. The disturbances may express themselves in form of fever, anorexia and cachexia, or they may also affect selectively determined organs. As a cause for these tumour-associated changes, apart from unspecific and specific metabolic functions of the tumour cell, also the sequelae of immunological reactions between tumour and host organism are made responsible. The metabolic remote action of malignant neoformations proved as clearly manifest above all in the paraneoplastic endocrinopathies. Though most of the paraneoplastic syndromes are not strongly specific for the tumour their knowledge may be significant in the diagnostics and control of the course of neoplasias. This is also the case in emergency situations which can be evoked by such pathological processes. The most important paraneoplastic syndromes and problems of their pathogenesis are explained.

[Cytostatic chemotherapy and sensitivity tests (oncobiograms) (author's transl)].

The success of cytostatic chemotherapy depends on the numerous factors. Thus, apart from pharmacokinetic and cell kinetic factors, particular attention is being attached to the resistance at cell-biochemical level. In the past decade, a number of screening tests has been devised to avoid the use of ineffective cytostatic drugs in therapeutic schedules. The use of organ culture technique is described for cytostatic sensitivity tests in patients with operated bronchial carcinoma.

Nurse-led diabetes clinics benefit black and Asian patients.

There is strong epidemiological data that shows that prevalence of diabetes varies with ethnic origin. This article describes the establishment of diabetic clinics within general practices in an attempt to identify and meet the needs of minority ethnic groups in relation to this condition. A future article will describe opportunistic screening of the target population.
