Heightened expression of the cytotoxicity receptor NKG2D correlates with acute and chronic nephropathy after kidney transplantation.

The activating cytotoxicity receptor NKG2D binds to stress-regulated molecules encoded by the major histocompatibility complex class I chain-related (MIC) and UL-16-binding protein (ULBP)/retinoic acid early transcript (RAET) gene family. To assess whether acute allograft rejection leads to an induction of these inducible ligands and their receptor NKG2D, we examined the mRNA profiles in kidney transplant biopsies. Expression levels were correlated with the incidence of acute rejection (aRx) episodes and chronic allograft nephropathy (CAN) proven by histology. Whereas MICA, ULBP1/3 and RAET1-E did not display heightened gene expression, elevated levels of NKG2D mRNA could be associated with aRx (p < 0.001). Immunohistology of kidney biopsies diagnosed with aRx revealed NKG2D+ cells in tubulointerstitial areas positive for CD8+ cells. Most importantly, elevated levels of NKG2D mRNA were associated with restricted long-term graft function assessed by the glomerular filtration rate at 6, 12 and 18 months posttransplantation. Induced NKG2D mRNA expression was still observable in biopsies diagnosed with CAN (p < 0.001), demonstrating a higher sensitivity and specificity compared to CD3, granzyme B and granulysin mRNA measurement. Significant elevated levels of NKG2D mRNA could be further detected in urine sediment prior to aRx, suggesting this receptor as a new candidate marker for the diagnosis of acute and chronic allograft rejection.

Early post-transplant urinary IP-10 expression after kidney transplantation is predictive of short- and long-term graft function.

The early identification of renal transplant recipients at enhanced risk of developing acute and subclinical rejection would allow individualized adjustment of immunosuppression before functional graft injury occurs and would exclude these patients from drug-weaning studies. Protein and reverse transcriptase-polymerase chain reaction-based analyses of candidate markers in urine open the opportunity to closely monitor kidney-transplanted patients non-invasively. The chemokine interferon-inducible protein 10 (IP-10; CXCL10) might be an interesting candidate to uncover ongoing immune processes within the graft. Urine samples from kidney-transplanted recipients were retrospectively analyzed for IP-10 mRNA and protein expression. IP-10 levels were correlated with the incidence of acute rejection episodes proven by histology and long-term graft function assessed by the glomerular filtration rate 6 months post transplantation. IP-10 expression in urine identified patients with ongoing acute rejection episodes several days before a biopsy was indicated by rising serum creatinine levels. Most importantly, elevated levels of urinary IP-10 protein within the first four postoperative weeks were predictive of graft function at 6 months even in the absence of acute rejection. These data reveal a correlation between elevated IP-10 expression in urine at early time points post-transplantation and intragraft immune activation that leads to acute rejection and compromised long-term graft function.

"Fluorescent timer": protein that changes color with time.

We generated a mutant of the red fluorescent protein drFP583. The mutant (E5) changes its fluorescence from green to red over time. The rate of color conversion is independent of protein concentration and therefore can be used to trace time-dependent expression. We used in vivo labeling with E5 to measure expression from the heat shock-dependent promoter in Caenorhabditis elegans and from the Otx-2 promoter in developing Xenopus embryos. Thus, E5 is a "fluorescent timer" that can be used to monitor both activation and down-regulation of target promoters on the whole-organism scale.

Amplification of cDNA ends based on template-switching effect and step-out PCR.

A new method for amplifying cDNA ends is described which requires only first-strand cDNA synthesis and a single PCR to generate a correct product with very low or no background. The method can be successfully applied to total RNA as well as poly A+ RNA. The same first-strand cDNA can be used to amplify flanking sequences of any cDNA species present in the sample.

Inductive interactions regulating body patterning in planarian, revealed by analysis of expression of novel gene scarf.

Subtractive hybridization was used to search for the early difference in gene expression between anteriorly and posteriorly regenerating tissues of the same region of the planarian body. A sequence (named scarf) specific for posteriorly regenerating tissue was isolated, encoding a soluble C-type lectin consisting of two slightly different carbohydrate-recognition domains. Such an unusual bivalent structure allows attribution of the protein to a novel subfamily of C-type lectins. scarf expression in intact planarian is rather abundant and follows a characteristic pattern not linked to any known morphological structure. We performed a series of experiments using scarf as a molecular marker. Its expression was monitored during different types of regeneration by whole-mount in situ hybridization and reverse-transcription polymerase chain reaction. The obtained data suggest that scarf expression is positively regulated by anterior tissues closely adjacent to the scarf-expressing region, so that their surgical removal results in rapid scarf switch-off. In turn, tissues expressing scarf seem to inhibit its activation anteriorly. This indicates that at least some of the body patterning events in planarians are based on a system of reciprocal inductive interactions rather than on a global morphogen gradient.

Ordered differential display: a simple method for systematic comparison of gene expression profiles.

A method for display of 3'-end restriction fragments of cDNAs is proposed, extending the idea reported recently. First, representative pools of such fragments are selectively amplified using PCR suppression effect. Then, simplified subsets of these fragments suitable for comparison by PAGE are amplified by adapter-specific primers extended by two randomly picked bases at their 3'ends. By testing all possible combinations of extended primers the whole mRNA pool may be systematically investigated. The method was applied to search for molecular regional markers of freshwater planarian Dugesia tigrina .

Construction of cDNA libraries from small amounts of total RNA using the suppression PCR effect.

Here we describe a method for preparing high-quality cDNA libraries from total RNA. By this method, double-stranded (ds) cDNA ligated with a specially designed ds adaptor is amplified by PCR using a modified T-primer and another primer corresponding to the outer part of the adaptor. The suppression PCR effect strongly inhibits the amplification of poly(A) RNA, thereby reducing background. This method leads to amplification of high-quality cDNA, facilitating the construction of representative cDNA libraries from as little as 10-100 ng of total RNA.

[Considerations on the emergency of testicular torsion].

Testicular torsion is an emergency with priority in childhood and adolescence. The ambulatory pediatrist therefore is confronted frequently with this disease. From his current viewpoint complemented by expert opinions the clinical urologist wants to have an influence on reducing the still high testicular loss rate by giving suitable measures respectively recommendations.

[Lesion of the ureteral wall caused by gynecologic surgeries from the urologic viewpoint].

It is reported on 31 ureteral strictures in 29 female patients after abdominal or vaginal, simple or complex hysterectomy. In 21 cases the ureteral obstruction was combined with an external urinary fistula. In 11 cases it was noted a spontaneous receding of the ureteral obstruction respectively a sanitation of the urinary fistula without stenosis. The general renal protection in form of a nephrostomy as an important component of therapy is recommended. Directions for the medical care of intraoperatively diagnosed ureteral lesions are given.

[Prognosis-oriented diagnosis, treatment protocol and after care in uretero-vaginal fistula following gynecological surgery from the urological viewpoint].

It is reported on 20 cases of ureterovaginal fistula after hysterectomy. In all cases renal protection (nephrostomy) was compulsory. Spontaneous healing without stenosis occurred in 6 cases, in 10 cases urethral reimplantation was successful, in 4 cases nephrectomy had to be carried out. Tactics of the therapy are described from the urologic viewpoint. Pre- and postoperative marks with relevance for prognosis and the special following-up are especially emphasized in this connection.

[Clinical aspects and treatment of penis fracture].

Our experiences in the treatment of 5 patients suffering from penile fracture are presented. The clinical picture is determined by defect size, localization and injury of the urethra. The functional results of early operative management are good and the literature reports are confirmed.

[Lymphogenous metastasis of urinary bladder cancer].

In 83 patients with locally curable bladder cancer of stage T1-T3 an exploratory pelvic lymphadenectomy was performed. The rate of metastases in all patients was 15.66%. In the analysis of lymphogenous metastasizing routes an obligatory direct deposition primarily in the lymph nodes of the obturatorius and/or external group with constantly by-passing of the viscero-vesical nodes was found. From this some conclusion to a modified technical-tactical approach of the exploratory pelvic lymphadenectomy have been done.

[Iatrogenic inguinal injury of the spermatic cord from a treatment and forensic viewpoint].

Iatrogenic obstruction of the spermatic duct or iatrogenic occulusive azospermia, in a wider context, is more frequently diagnosed today than it used to be in the past. This is attributable to general improvement of the system of andrological care. Hints are given on avoidance of that lesion by surgical tactics as well as on provisional treatment and on compulsory notification of injuries that occurred to the spermatic duct. A rating of 20 to 30 per cent of disablement is proposed for irreversible iatrogenic occlusive azospermia. Reference is made to possible modes of financial compensation in the context of mVGE or EmU procedures.

[Varicocele and fertility--spermiographic and histologic findings].

The results of the treatment of 251 patients with varicocele are demonstrated. By means of spermiographic and histologic investigations the fertility-disturbing influence of this vascular disease of the testis shall be proved and the effect of the operative sanation shall be estimated. For many years the high ligature of the testicular vein has been favoured method of operation; since 1984 exclusively the percutaneous transfemoral occlusion of the testicular vein has been performed, when a left-sided varicocele is present. In 61% of the patients examined an improvement of the spermiogram is to be proved, in which case the most conspicuous rate of increase lies at the motility of spermatozoa with 58%. After removal if the varicocele the investigations of the testicular tissue reveal in 42% of the cases an improvement of the findings by decrease of the tubular atrophy and increase of the tubular cell number. Among the number of our patients the rate of conception is 49%. The treatment of varicocele is a fertility-protective measure and therefore should be performed in all adolescents and males at fertile age.
