[Transplantation of the kidneys from newborn donors, using the Mini-Lewe minipig as a model].

The importance of kidneys from newborn donors in transplantation has not been clearly defined. The purpose of this study was to investigate the degree of morphological and functional adaptation of the newborn kidney in an adult recipient. 7 MLS-matched kidneys from Lewe minipig puppies (age: 3-4 weeks) had been transplanted en bloc to their mothers with immediately bilateral nephrectomy. Preliminary results suggested that arterial vasculature and blood supply are the limiting factors in adaptation of newborn donor kidneys in adult recipients.

Circulating immune complexes after cadaver kidney transplantation.

351 sera from 27 human recipients of renal allografts and 21 healthy blood donors were assayed for circulating immune complexes by the Clq solid-phase radioimmune assay. Increased Clq-binding activity (Clq-BA) was detected in pretransplant sera from 5 patients with chronic pyelonephritis (PN) and 3 patients with chronic glomerulonephritis (GN). A significant decrease of Clq-BA immediately after transplantation could not be found. 6 weeks after transplantation only 2 patients of the PN group showed increased Clq-BA. Serial studies in 17 patients with rejection crises did not show any correlation between the level of serum Clq-BA and the occurrence of rejections. Furthermore, no correlation could be found between the occurrence of complement-dependent lymphocytotoxic antibodies measured by the 51Cr release technique and the level of serum Clq-BA. In contrast, our results show that the probability of graftectomy or graft failure is significantly higher, at least in the early phase after transplantation, when the serum Clq-BA is lowered for several weeks.

[Diagnosis and therapy of malignant parenchymal kidney tumors].

We think that the present paper insistently demonstrates the fact which is acknowledged by all but followed only by few how important is a unitary classification of the renal tumours in order to perform scientifically proved comparisons and valuations in all branches of the treatment of renal tumours including the results of the therapy. On the basis of the classification of our patients according to the TNM-system we could prove that the best results in the operative removal of the kidney with following irradiation were achieved in the categories T1 and T2, particularly when there were not yet present any metastases distant from the primary tumour.

89Strontium therapy of bone metastases of carcinoma of the prostatic gland.

Following a firm diagnostic and therapeutic schedule for patients with prostatic carcinoma. 89Strontium therapy was introduced for multiple metastases. Positive skeletal scintigraphy with 99mTc-EHDP induced check for affinity to Sr using 85Sr scintigraphy. Of 80 patients, multiple metastases were found in 26. Therapy with 1 mCi of 89Sr-chloride was started in 20 cases. In 8 patients, relief from severe pain appeared shortly afterward, and a further 8 it was possible to prevent the development of pain. Moderate success in 3 cases and a failure to provide relief in 1 were observed.

Effect of high-dose prednisone on renal hemodynamics of normal and transplanted dog kidneys.

The effect of prednisone therapy on renal hemodynamics was studied in normal, autotransplanted as well as homotransplanted dog kidneys. After intravenous administration of prednisone (8 and 16 mg/kg/min) the total blood flow decreased and vascular resistance increased significantly. This effect was more pronounced if the drug was directly injected into the arteria renalis. The same changes were observed in autotransplanted and homotransplanted kidneys and in kidneys in rejection. Our results suggest that high dosage of prednisone decreases the renal blood flow and this adverse effect has to be considered in the therapy. The administration of the drug directly into the arteria renalis may have to be avoided.

Late prognosis in acute posttransplant renal failure in 102 patients.

An ivestigation was made of the effect of the duration of posttransplant renal failure on the late prognosis of graft as well as patient survival and on the frequency of complications in 102 patients who had a functioning graft at three months after transplantation. A direct correlation was found between duration of acute tubular necrosis (ATN) and the late prognosis of the graft. The creatinine clearance was significantly higher and the frequency of complications was lower in the group with immediate resumption of renal function. The mortality rate increased with the duration of ATN.

Acute Kidney Injury

[Clinical and experimental results on the significance of kidney damage for long-term prognosis after kidney transplantation].

In the clinical study the influence of the differently long times of warm ischaemia and of the duration of the acute renal insufficiency post transplantationem on the late prognosis of the grafts and the survival of the patients as well as on the incidence of complications in 93 patients who first underwent a transplantation of the CD-kidney and who more than two months after the transplantation lived with a working kidney was examined. A direct relation between the duration up to the beginning of the function of the kidney and the long-term prognosis of the patients was proved. In the group with immediate beginning of the function of the kidney at an average WIZ of 13 minutes the creatinine clearance was significantly higher and the incidence of severe infections in the late phase was lower than in the groups with longer WIZ and very retarded beginning of the renal function. Haemodynamic investigations on the autograft model of the dog after 30 minutes WIZ resulted in a pathologically increased renal vascular resistance and in a restricted filtration function up to 4 weeks post transplantationem.

[Our experience with the nephrourological care of children].

1. A successful medical care for nephro-urologically ill children can only be guaranteed by efficient cooperation.--2. For more than 5 years there has been close collaboration in this field between the Department of Urology and the Department of Pediatrics in the Berlin-Friedrichshain Hospital.--3. In evaluating our experience some basic conditions are described which may serve as prerequisites for a successful cooperation.--4. Details of the content and organisation of cooperation between different clinical specialities vary according to local conditions.

[Experiences with new methods for the preoperative localization and surgical removal of complicated kidney calculi].

In a survey is described the inpatients suffering from urolithiasis of the urological clinic Berlin-Friedrichshain of the last three years from 1 April 1975 to 31 March 1978. Especially the cast calculi of the renal calyx were analysed and the therapeutic approach was discussed. For the complete removal of coral calculi in our clinic preoperatively made tomographies and adequate graphs of the cast concrement in its relation to the hollow system at two levels also stood the test as the intraoperative use of the X-ray device "Renodor". The two methods were described in detail. Also in multiple and movable concrements in the system of the renal calyx the Renodor stood the test. Here we could make good experiences also with the fibrin pyelolithothomy--the indications and results of these methods are represented.
