Primary gastric T-cell lymphoma of suppressor-cytotoxic (CD8+) phenotype: discordant expression of T-cell receptor subunit beta F1, CD7, and CD3 antigens.

Primary gastric T-cell lymphomas are rare neoplasms, and all but one of the previously phenotyped cases have shown a helper-inducer phenotype. The present case is the second reported case of a primary gastric T-cell lymphoma of suppressor-cytotoxic phenotype. The tumor histology was similar to that described in some forms of node-based peripheral T-cell lymphomas. Phenotypic analysis revealed low expression of pan-T marker CD7, reduced expression of CD3, but higher density and frequency of expression of CD8 antigens that could be predicted on the basis of the pan-T markers. Natural killer cell (NK) related markers CD16, HNK-1 and NKH-1 were not expressed by the neoplastic cells. T-cell receptor (TCR) beta subunit expression was detected on fewer cells than would have been predicted on the basis of CD3 and CD8 expression, and TCR delta chain expression was undetectable.

Communication and oral-motor function in Rett syndrome.

The communication skills, oral-motor function and respiration patterns of 20 girls with Rett syndrome were studied. Loss of communication has been previously recognized as a sign of the onset of Rett syndrome, but oral-motor dysfunction has not been reported. All the girls showed a regression in speech and language function by the onset of stage II Rett syndrome. Changes in oral-motor function and respiration correlated with the progression of the disease from stages II to IV.

Proteinaceous lymphadenopathy due to monoclonal nonamyloid immunoglobulin deposit disease.

We report two cases of lymph node enlargement due to massive extracellular nonamyloid immunoglobulin deposits that obscured the underlying cellular pathologic condition. In both cases, the deposits were demonstrated to be restricted to a single heavy and light chain, consistent with a monoclonal paraprotein, and cytoplasmic staining in the lymphocytes or plasma cells was identical to that of the paraprotein. The use of the protein A-gold technique was instrumental in revealing a monoclonal pattern in one case in which light microscopic immunohistochemistry did not reveal a clear-cut monoclonal pattern in the extracellular deposits. This case was subsequently shown to have multiple myeloma, while the second case has had an unusual history of hypocomplementemic vasculitis and normal bone marrow. Neither case had evidence of significant renal disease.

Augmentative communication, evolution, and progress for nonspeaking children.

Technology has provided a recent emergence of augmentative communications systems for nonspeaking children. The effective use of such systems is dependent on the knowledge and skills of physicians, speech-language pathologists, educators, parents, and other professionals working with the child. This article examines the components of an augmentative system including the mode of communication, which includes choices among manual, non-electronic, and electronic systems. The advantages of each mode are discussed. Another major component, the symbol selection process, follows an ascending hierarchy from a representational code involving actual objects to a complex symbolic code with spelling and numerals. Clinical application at the Child Development and Rehabilitation Center of the Oregon Health Sciences University points to the need for thorough interdisciplinary team evaluation during system selection or modification. Data obtained from assessment of 28 multihandicapped children reveal necessary changes, additions, and follow-up for both the mode and symbol system in the majority of children examined.

Proposition 13: experience in one community mental health center.

The impact of Proposition 13 on one community mental health center in California is described. Although the center has had no layoffs as a result of its passage, the measure has had a serious effect on staff morale, which was already at a low point because of pre-existing budgetary constrictions that led to staff reductions despite increased demands for services. It has also generated several changes in treatment programs designed to reduce overhead costs and make more efficient use of staff time. The author emphasizes that because of the supplemental funds made available by the state from its surplus during fiscal year 1978-79, the full impact of Proposition 13 on local service programs is still to be felt.

The Baltimore Sepsis Scale: measurement of sepsis in patients with burns using a new scoring system.

A new sepsis scale has been developed, and it consists of 13 parameters, each of which is measured daily on a variable point scale. In 41 patients with major burns the Baltimore Sepsis scale correlated well with death or survival, with occurrence of septicemia, and with the level of serum interleukin-6. It did not correlate well with the level of plasma endotoxin or with the type of organism that was grown in blood cultures (gram-positive or gram-negative). We propose that the Baltimore Sepsis Scale would be an accurate and easy scale to use for the measurement of interventions that are aimed at improving the septic state.
