[The ergonovine test with injection of increasing doses in the diagnosis of chest pain].

Ergonovine testing was performed in 40 patients with normal coronary angiograms (24 men, 16 women, mean age 53 years) complaining of chest pain. Esophageal and coronary spasm were assessed by simultaneous esophageal manometry, 12-lead-EKG and myocardial scintigraphy. Progressive doses of ergonovine were injected. Of the 17 patients experiencing chest pain during testing, coronary spasm occurred in 3 and esophageal motor disorders in 7 (peristaltic contraction waves of high amplitude in 3, diffuse esophageal spasm in 4). The dose necessary was more than 0.175 mg. No objective changes (EKG and manometry) were observed in the 7 remaining patients.

Relationships between the extent of ileal lesion of resection and vitamin B12, bile salt and fat absorption.

Vitamin B12 Urinary excretion, stool weight, fecal fat excretion, fecal 14C glycocholate excretion and 14CO2 output after I-14C glycocholate ingestion, were measured in 13 patients with non-operated ileal Crohn's disease (NOC), 14 patients with ileal resection for ileal Crohn's disease, with or without right colectomy (RC), and 11 patients with ileal resection with or without right colectomy for pathology other than Crohn's disease (RNC). A positive linear relationship was found between stool weight and 14C glycocholate fecal excretion. The logs of fecal fat and 14C glycocholate excretions were related to the extent of the ileal lesion of resection; a similar but negative relationship was observed for vitamin B12 urinary excretion. Fecal fat excretion and respiratory 14CO2 output were significantly higher in patients with right colectomy.

Flexible rectosigmoidoscopy in the detection of tumoral colonic lesions.

The fibrorectosigmoidocope Olympus TCF I S, has been systematically applied to the exploration of the distal colon (rectum and sigmoid) in patients with intestinal or digestive extra intestinal symptoms. In the first 300 patients the technical parameters of the method were tested: the fleet enema preparation was found excellent or satisfactory in 95% of examinations. The tolerance was good in 96% of examinations--the mean length of operation was 5.15 min--the mean length of colon examined was 48.5 cm, the distance of 50 cm being surpassed in 50% of patients. The efficacy in the detection of the lesions was tested in 476 patients. Tumoral colonic lesions were detected in 105 patients with a total of 125 adenomas and 31 cancers. In the area that could have been explored by a rigid rectosigmoidoscope (0-19 cm) we found 71 lesions (50 adenoma and 10 carcinoma), whereas beyond this limit (25-60 cm) 85 lesions (50 adenoma and 10 carcinoma).

[Reflections on 19 cases of asbestosis discovered by chance in the population of an industrial region in the Basse-Sambre].

The authors describe 19 cases of definite or highly probable asbestosis which were observed among unselected out-patients at a hospital in the industrial area of the "Basse-Sambre". These cases were of various types and were, almost exclusively, of occupational origin. The variety and severity of the asbestosis in many of the cases justify a more thorough survey in this area. Those people who have handled and inhaled asbestos fibres at work will be examined in the first place. A radiological pulmonary survey will also be organized among the people living in the neighbourhood of factories using asbestos. Precise information about the origin, quality and characteristics of the inhaled dust as well as the intensity and duration of exposure will be obtained. Cases discovered in the survey will be investigated systematically by means of an extensive range of techniques: frontal and oblique X-rays with an appropriate kilovoltage for the detection of pleural calcifications and of the reticular thickening of the parenchyma; spirometry and measurement of the transfer factor for CO; arterial blood gases at rest and during exercise; search for ferruginous bodies in sputum; pathological studies of pulmonary and pleural tissues and of the ultrastructure of pleural tumours.

CT findings of perforated duodenal diverticulitis.

The diagnosis of duodenal diverticulitis is rarely made preoperatively by upper gastrointestinal series or plain abdominal films. We describe a case of perforated duodenal diverticulitis diagnosed by CT.

Endoscopic therapy of Barrett's oesophagus: critical review.

Barrett's oesophagus is known as one of the most important risk factor of oesophageal adenocarcinoma. Because of the increasing incidence of these latter, many endoscopic methods such as argon plasma coagulation, photodynamic therapy or endoscopic mucosal resection are now in evaluation in order to eradicate Barrett's oesophagus or to treat dysplasia and early cancers arising from this metaplasia. The aim of this paper is to comment these techniques and discuss their usefulness.

Oesophageal self expanding metal stents. Preliminary report about covered and non-covered types.

OBJECTIVE: The aim of this study was to evaluate the effectiveness and complications rate of covered and non-covered self expanding metal stents in the palliative treatment of oesophageal dysphagia. DESIGN: In this retrospective non-randomized study, we evaluated 11 non-covered and 17 covered stents of different types. RESULTS: Grade of dysphagia and improvement after treatment were similar in both groups, all the seven fistulas were sealed by covered stents. Covered stents seem to be safer regarding the rate of life-threatening complications and reinterventions. In contrast to published studies, bleeding was our major complication with death related in half of these patients. Aorto-Oesophageal fistula was proved by autopsy in two of them. CONCLUSIONS: Covered stents lead to less drawbacks than non-covered ones and seem to be recommended in the palliation of oesophageal dysphagia even in the absence of fistula.

Report on the Belgian consensus meeting on colorectal cancer screening.

Screening and prevention of colorectal cancer must be a public health priority. It is the most frequent malignancy in Europe, the second leading cause of cancer death, including Belgium where more than 6000 new cases occur per year. Various screening modalities, from non invasive to invasive are available and currently in use and they are all cost-effective in comparison with no screening. The decision as to which screening test to use should be made by the patient and clinician. Consensus documents prepared by the Belgian scientific community appear in this issue of Acta Gastroenterologica Belgica, summarizing the scientific evidence in favour as well as the limitations of fecal occult blood tests, flexible sigmoidoscopy, videocolonoscopy and virtual colonoscopy.

[Pharyngo-laryngeal consequences of gastroesophageal reflux. Value of esophageal pH measurements in 74 patients].

Ambulatory 24 hour oesophageal pH monitoring was performed in order to detect gastro-oesophageal reflux disease (GERD) in 74 patients presenting with poorly explained ear-nose-throat complaints. A significant number of these patients also benefited from other currently used methods for diagnosing GERD. Oesophageal pH-monitoring demonstrated GERD in half of the patients. For atypical GERD, 24 hour oesophageal pH-monitoring seems to be the most sensitive procedure.

[Acute dilatation of the colon].

Dilated colon is provoked by obstructing lesions, toxic megacolon or colonic pseudoobstruction. The obstructing lesions of the colon are colonic volvulus, inflammatory bowel disease with stenosis or colonic cancer. Toxic megacolon is more often caused by I.B.D. and rarely by infectious diseases. Etiological diagnosis is possible after clinical and radiological evaluation. Colonoscopy is always indicated, except in toxic megacolon. Balloon dilatation of strictures, palliative treatment of colonic carcinoma by Laser procedures, reduction of colonic volvulus and aspiration of colonic pseudoobstruction are the principal indications of therapeutic colonoscopy in the non surgical treatment of dilated colon.

[Etiopathogenesis of colonic diverticular disease; role of dietary fiber and therapeutic perspectives].

The authors review the literature about the aetiopathogenesis, the epidemiology and the treatment of uncomplicated diverticular disease of the colon. The influence of dietary fibre deficiency explaining the increasing prevalence in industrialized countries and the variable geographical distribution are explained, as well as the effects of dietary fibres on colonic functions. Progressive elastosis of the taeniae may explain the influence of age on the incidence of diverticulosis. The rationale for the introduction of dietary fibre into the treatment is based on the observation that a high fibre diet such as coarse bran is lowering the colonic pressure and therefore may prevent the development of new diverticula and complications, although an effect on symptoms is not always obtained.

[Comparative value of anorectal manometry and electrocardiography in the diagnosis of diabetic autonomic neuropathy].

Searching for silent anorectal autonomic neuropathy, we investigated the anorectal sensorimotor function in 79 continent diabetics. Thirty four per cent of the patients had a basal anal sphincter pressure lower than 40 mm Hg N = 54.5 +/- 15.5 mm Hg and 11 p. 100 had a threshold of conscious rectal sensation higher than 20 ml (N less than or equal to 20 ml). An electrocardiogram was performed and the change in heart rate resulting from successive deep breaths was recorded for each patient. Cardiac autonomic neuropathy was diagnosed when cardiac frequency did not change with ventilation. This was reflected by a ratio E/I (largest RR space during expiration on the electrocardiogram to the shortest during inspiration) close to one. Compared to cardiac criteria for autonomic neuropathy, the sensitivity of anorectal manometry in the detection of diabetic autonomic neuropathy was 77.2 p. 100 and specificity was 82 p. 100. Low basal anal sphincter pressure in asymptomatic diabetics seems to be a sensitive and specific criteria for the assessment of diabetic autonomic neuropathy and correlates closely with cardiac criteria.
