Pelvic abscess associated with a Lippes loop. An unusual case.

An unusual case occurred of Lippes-Loop-associated pelvic abscess, characterized by a relatively mild clinical course and an unusual localization in the pelvis. This case emphasizes the importance of considering the association between intrauterine devices and pelvic abscess.

Fetal heart rate decelerations following the administration of meperidine-promethazine during labor.

A deceleration response lasting up to 7 min was observed in 53 fetuses (out of 1910 studied) following the administration of 75 mg meperidine and 25 mg promethazine intravenously to the mothers during labor. Thirty-six of these fetuses reacted with baseline tachycardia following the deceleration with or without loss of baseline variability. Seven fetuses showed a variable deceleration pattern and six fetuses reacted with a late deceleration pattern following the first deceleration. Two newborns from the group showing a late deceleration pattern were delivered with an apgar score below 7 in 5 min. The probable mechanisms and significance of these changes are discussed.

Relationship between the activity of pancreatic phospholipase A2 and the physical state of the phospholipid substrate.

When pancreatic phospholipase A2 is mixed with small unilamellar vesicles made from dipalmitoyl phosphatidylcholine at or above the phase transition temperature (tm), the time course of hydrolysis exhibits a distinct lag period. On the other hand, if the enzyme is preincubated with substrate for a short period of time below the transition temperature (during which time only limited hydrolysis occurs) and then assayed at high temperature, no lag period is observed. This phenomenon does not appear to be the result of product formation. Instead, the reported results lend substantial support to the hypothesis that a relatively slow substrate-enzyme organizational step is required as part of an activation process and this is most rapid when the substrate is in the gel state. However, the intrinsic activity (after the initial step of activation) is maximal when the substrate is in the liquid-crystalline state.
