[Acute cholestatic hepatitis caused by propafenone. Report of a case and review of the literature].

A case of acute cholestatic hepatitis associated with use of propafenone is reported. Hepatitis developed 3 weeks after the beginning of administration of this drug. The close time relationship between the administration of the antiarrhythmic drug and the acute onset of the liver damage, the exclusion of hepatobiliary disease and the rapid normalisation of biochemical parameters following withdrawal of the drug strongly suggest that propafenone was involved in the pathogenesis of this syndrome. Meticulous taking of patient history and clinical assessment are mandatory for the early identification of drug-induced hepatotoxicity and avoidance of more severe types of reactions, such as liver failure.

[Azithromycin-omeprazole. Treatment for the eradication of Helicobacter pylori].

AIM: To evaluate the benefit of 4-week regimen including azithromycin+omeprazole (vs omeprazole alone) for eradication of Helicobacter pylori. METHODS: Twenty HP positive patients with an ulcer dyspepsia (NUD) were included in this study. They were given either omeprazole 40 mg for 4 weeks alone or in combination with azithromycin 1 g/die for 1 week. Endoscopy was performed before 4 weeks after and 4 months after treatment. The presence of HP was assessed in antral and corporeal biopsies by urease test and histology. RESULTS: HP eradication was observed in 9/10 (90%) patients in the omeprazole+azithromycin group and 0/10 patients in the omeprazole alone group. CONCLUSION: Omeprazole 40 mg for 4 weeks in combination with azithromycin 1 g die for 1 week eradicates HP in 90% of these patients. The good eradication percentage and absence of collateral effect make us extend patients' number to test.

[Iproniazid-induced acute hepatitis].

Many drugs can cause an acute liver damage. The patient history is the guideline for diagnosis. Iproniazid, a monoamine-oxidase inhibitor not for sale in Italy, can frequently cause severe acute hepatitis. A case of acute iproniazid-induced, hepatitis in which the course was favourable, is reported.

[A case of acute simple volvulus of the cecum].

A rare and exceptional case of acute simple volvulus of the cecum, already affected by a gangrenous process, is described after it had been found by chance in a patient who was operated for acute abdomen. The torsion had occurred in a tract of the cecum which had been strictured by an acute aspecific inflammation. Having underlined that preoperative diagnosis of simple volvulus of the cecum is virtually impossible (and likewise the diagnosis of volvuli of the ascending cecum and ileocecumcholics), the paper continues to describe the etiopathogenetic mechanisms which are thought to have caused to volvulus in question.

Primary non-Hodgkin's lymphoma of the esophagus.

Esophageal involvement of non-Hodgkin's lymphomas is extremely unusual; primary lymphoma of the esophagus is even less common. This report describes a case of malignant small lymphocytic-plasmacytoid lymphoma with primary esophageal localization. Endoscopic diagnosis was confirmed by histological examination of a large portion of tumoral tissue spontaneously expelled after esophagogastroscopy. Lymph nodes, bone marrow, and other gastrointestinal sites were not involved in the disease. We describe the clinical history of the patient, with the remissions induced by chemotherapy, over a 5-yr observation up to the patient's death, which was not directly related to the tumor.

[A case of volvulus of the gallbladder].

A rare case of total volvulus of the gallbladder observed intraoperatively and treated by cleansing and cholecystectomy in a 96-year-old woman undergoing surgery for acute lithiasic cholecystitis is described. More than the clinical and laboratory data (insignificant considering the patient's anergic and debilitated condition) it was the results of hepatobiliary ultrasonography which revealed acute, rapidly progressing inflammation of the gallbladder that supplied the indication for immediate surgery.

[Massive inguinoscrotal hernia of the bladder].

A rare case of massive inguinoscrotal hernia of the bladder in an obese 73-year-old man is described. The condition was successfully treated by simple repositioning of the bladder followed by abdominal wall repair.
