Sonographic changes in the endometrium of climacteric women during hormonal treatment.

Vaginal sonography of the endometrium is a new method of monitoring the effects of progestogen replacement therapy during the post-menopausal period. The advantages of transvaginal ultrasonic diagnosis are that the uterus can be seen from the fornix vaginae (that is from close range) and that the examination can be carried out whether the bladder is full or not. The differences in the thickness and structure of the endometrium, and the boundary with the surrounding myometrium, enable most of the endometrial changes that occur during the post-menopausal period to be detected sonographically. In the first group of patients we investigated, endometrial carcinoma was detected in 2 out of 60 women suffering from post-menopausal metrorrhagia, the findings subsequently being confirmed histologically. It was observed that post-menopausal women who have regular bleeding during post-menopausal hormone therapy show signs of endometrial proliferation during oestrogen therapy and of secretion during progestogen therapy.

Beta-endorphin levels during the climacteric period.

Hot flushes are not caused by hypergonadotrophinaemia. This is apparent because peaks of gonadotrophin in the serum do not coincide with cutaneously measured hot flushes while such flushes still occur in hypophysectomized women. Gonadotrophin-releasing hormone and other neurotransmitters (possibly beta-endorphin) affect thermoregulation. The following hypothesis is advanced. During the climacteric period neurotransmitter changes, a decrease in catechol oestrogens, a decrease in alpha-2-adrenoceptor activity and cessation of ovarian steroid production may lead to alterations in endogenous opiate activity and thus to disturbances of thermoregulation, resulting in the occurrence of hot flushes. Low beta-endorphin levels in the peripheral plasma, which rise again following oestrogen treatment, are observed during the climacteric. On the other hand, women with severe hot flushes caused by a stress event show enormously increased beta-endorphin values, which are normalized by hormone substitution therapy acting via still unknown neuroendocrinological feedback mechanisms.

[Joint pain in pre- and postmenopausal arthropathia climacterica].

The menopausal syndrome consists of a variety of psychic and physical signs of different intensity. At our outpatient department for climacteric complaints and osteoporosis prevention (1. Dept. of Gynecol. and Obstetrics, Vienna) 820 women, who have been consulting this department for the first time because of prevailing complaints or prophylactic reasons, were registered in the year 1986. In the course of this work we paid particular attention to the symptom "pain in a joint" as a sign of the beginning climacteric period. Referring to our patient population we tried to analyse epidemiologically the frequency and the degree of severity of menopausal arthritic pain as well as the therapeutical response to a hormone substitution therapy. To what extent an endocrinologic explanation can be found and which facts let us assume such a statement, we tried to demonstrate in the discussion.

Abortion risk in chorionic villus sampling. Evaluation in elective termination of pregnancy.

100 patients were examined to evaluate the risk of abortion of chorionic villus sampling. In the 8th-10th week of pregnancy a catheter with a mandrin was introduced into the chorion frondosum under ultrasound guidance. The mandrin was then removed and trophoblast tissue obtained by aspiration. The patients agreed to postpone elective termination of pregnancy for 2 weeks. Another ultrasound was performed before suction curettage. With increasing experience, the abortion rate dropped to 4%, whereas the rate of successful biopsies and analyses rose to more than 90%. Based on these results, chorionic villi sampling was made available at the 1st Department of Obstetrics and Gynecology, University of Vienna, as an alternative method to amniocentesis. Out of 38 biopsies, 25 karyotypes were normal, two biopsies revealed pathological results (trisomy 21, 22), 11 samples showed no results (insufficient tissue or no mitoses). One patient had a spontaneous abortion (trisomy 22) and one an abortion due to infection. Chorionic villus sampling could replace amniocentesis because chromosomal anomalies may be detected already in the 1st trimester of pregnancy.

[The beta-endorphin level in climacteric women].

UNLABELLED: Hot flushes are not caused by a hypergonadotrophinemia. Peaks of gonadotrophin in the serum do not coincide with the cutaneously measured hot flushes. There is an occurrence of hot flushes in hypophysectomized women as well. GnRH and other neurotransmitters (beta-endorphin?) effect the thermoregulation. - HYPOTHESIS: During the climacteric period changes of neurotransmitters, a decrease of catecholestrogens, a decrease of alpha-2-adrenoceptor activity and the ceasing of the ovarian steroid production lead to changes of the endogenous opiate activity and thus to disturbances of the thermoregulation resulting in the occurrence of hot flushes. Low beta-endorphin levels in the peripheral plasma, which increase again after a treatment with estrogens, are found during the climacteric period. On the other hand women with severe hot flushes as an expression of a stress event show enormously increased beta-endorphin values, which normalize again after a hormone substitution therapy, effected by still unknown neuroendocrinological feedback mechanisms.

[Neuroendocrine changes in the climacteric].

In order to investigate the relationship between hot flushes and LH-peaks, we applied GnRH-Analogon for pharmacological hypophysectomy. Although the gonadotropin serum levels could be normalized, none of the women showed a cessation of the flushes. Therefore it can be assumed that not gonadotropins, but neurotransmitters are involved in the development of flushes. The determination of the serum-level of vasointestinal peptide in the course of a TRH-test showed high VIP levels only in two climacteric women. To verify the suspected relation of VIP to the climacterium, a larger collective of women has to be checked.

[Effect of a parenteral ozone-oxygen mixture on the concentration of immunoglobulins (IgA, IgG, IgM), of vitamin A and lysozyme activity in patients with cervical cancer].

In the literature you can find several therapeutic possibilities in the treatment of carcinoma by additive ozone therapy. We investigated the consequences of ozone therapy for the immunological status, lysozyme and vitamin A. 21 women with progressive cervical cancer (Stage III, IV) got besides the conventional irradiation therapy also an additive ozone therapy. After irradiation and ozone therapy a small decrease in IgG, IgA and IgM can be seen. A statistical significance could not be evaluated. There was no difference to the control group in lysozyme and vitamin A.

[Effect of ozone and ionizing radiation on an in vitro model--a pilot study of 4 gynecologic tumors].

In order to investigate the sensibility of cultured tumor cells to ozone and irradiation primary tissue cultures from one undifferentiated non-classified ovarian carcinoma, two solid adenocarcinomas of the ovary and one endometrial carcinoma were established. Cultivation was performed according to standard techniques involving the stem cell assay in soft agar as well as the monolayer technique in liquid nutrient media. The type of culture system did not influence the sensitivity of the culture cells to ozone and/or irradiation. Ozone treatment was performed with three different Ozone concentrations (0.03 ppm, 0.1 ppm, 0.3 ppm). Irradiation was done with 100 rd Ra226, Ir192 or Co60. A control experiment showed that the proliferative tendency of benign cells (skin fibroblasts) was not inhibited by the three ozone concentrations used in this study. Ra226 and even the combination of ozone and Radium did not influence the proliferative activity of these benign cells. Ir192 and Co60 were cytotoxic to benign as well as to carcinoma cells. Cultivated cells from endometrial carcinoma resisted to ozone treatment as well as to Ra226 (but they were destroyed by Ir119 and Co60). After pretreatment with ozone (0.1 ppm), Ra226 treatment of endometrial carcinoma cells induced a cytostatic effect implying that no cell divisions were observed after irradiation and the cells lysed within two weeks after irradiation. For the three ovarian carcinoma cell lines analysed in this study ozone treatment had a cytostatic effect even at the lowest concentration (0.03 ppm), with the two higher ozone concentrations a cytotoxic effect could be induced in ovarian carcinoma cells. Exclusive treatment with Ra226 induced a cytostatic effect, but it was cytotoxic after combination with ozone treatment of the lowest concentration (Ir192 and Co60 were cytotoxic in all cases). Our investigation confirmed the radiosensitizing effect of ozone treatment. Furthermore it was shown that exclusive ozone treatment even without combined irradiation displayed a selective cytotoxic action at the ovarian carcinoma cells.

[Micromanipulatory sperm injection--a new method in the treatment of infertile males].

Three couples were selected for this preliminary sperm injection study with their consent, since in all three cases the zona free hamster egg penetration test was negative and in one couple previous attempts of in vitro fertilization were unsuccessful. Of 22 eggs, recovered from three patients, 9 from each patient 3 were injected with sperm, while the others were inseminated by the standard IVF-protocol. Before manipulating human eggs extended experiments were performed with animals eggs, so decreasing the rate of damage to under 10%. Additionally we optimized the method by using an individual adapted equipment. Manipulation was performed by penetration of the zona pellucida with a 5 to 7 mu thick injection pipet into the perivitilline space so damaging of the egg cytoplasm was avoided. This special technique for sperm injection was not used till now in this indication. From the nine eggs used in this experiment one egg developed a pronucleus and one egg developed to a four-cell stage. We could not achieve a pregnancy.

Assignment of particle tracks to spores of Bacillus subtilis on silver chloride detectors.

In Biostack III B, flown in the Apollo-Soyuz Test Project, AgCl detectors were used to study ionizing effects of HZE particles on spores of Bacillus subtilis or eggs of Artemia salina. The tracks of these particles inside the detectors are used to extrapolate the path of the particle near the biological objects which are fixed at the detector surface. The closest distance to the geometric centre of the object, the so-called impact parameter, is determined with a mean accuracy of 0.3 micrometers for 1 micrometers spores. From knowledge of the lateral distribution of the energy transferred by primary and secondary ionization effects of the particle, the energy deposit and its localization at the objects can be determined. We describe some technical aspects of a video-electronic scanning system, Quantimet 720, which has been adjusted to the particular requirements of these experiments. The main improvements achieved are increased precision of coordinate measurements, objective focusing of the microscopic image combined with measurements of the density profile of particle tracks, and finally speeding up of the measurements by automatic data transfer.

[Prognostic value of changes in the fundus oculi in EPH gestosis].

We examined the ocular fundus in 100 unselected pregnant women with EPH-gestosis of different severity. In 52 women there were no retinal changes, in 32 women we saw I hypertensive changes according to the classification of Keith-Wagener, in 10 cases II, and in 6 cases III or IV retinal changes. There was no direct relationship between the severity of EPH-gestosis and I and II retinal changes; however, severe retinal changes (III IV) were associated with moderate or severe EPH-gestosis. We found a significant relationship between the degree of hypertensive retinal changes and fetal mortality. In the group of gravidae with severe retinal changes 4 out of 6 infants died. The examination of the ocular fundus permits the direct evaluation of fundal changes and the early recognition of fetal jeopardy.
