[Arrhythmia in thalassemia major: evaluation of iron chelating therapy by dynamic ECG].

Arrhythmias and sudden death represent striking features in the natural history of thalassemia major. Antiarrhythmic treatment, however, does not appear to change the clinical course. During recent years the disease's therapeutics approach has undergone a substantial evolution, being more adequate the transfusional regimens as well as more effective the iron chelation therapy through subcutaneous infusion of deferoxamine. The aim of the present study was to determine possible influences exerted by the current treatment upon disease's arrhythmic disorders. Thirty patients of both sexes were enrolled in the study. The age ranged from 9 to 24 years. No congenital or acquired heart diseases were present. Each patient underwent concentrated red cell transfusions (in order to obtain pretransfusional hemoglobin levels of 10-11 g%), and iron-binding therapy through continuous subcutaneous microinjection of deferoxamine 40-50 mg/kg/day (6-8 hours/day, 6 days/week). Patients were divided in 2 groups: the first group (group A) comprising the 16 patients with good therapeutic compliance and regular pharmacological regimen; the second group (Group B) including the remaining poorly compliant 14 patients. The following parameters were analyzed: age, average hemoglobin levels during the last year, total amount of red cell transfusions, ferritin levels, starting age of iron-binding therapy. Moreover, each patient underwent 24-hour ECG Holter monitoring. Age (Group A: 18 +/- 4.6; Group B: 14 +/- 2.7; p < 0.02), total amount of transfusions (Group A: 272 +/- 73; Group B: 211 +/- 44; p < 0.03), and ferritin levels (Group A: 1697 +/- 860; Group B: 2908 +/- 730; p < 0.002) proved to be significantly different in the two groups.(ABSTRACT TRUNCATED AT 250 WORDS)

[Nodular lesions simulated on an anthropomorphic thoracic phantom. A comparison between analog and digital images of the thorax].

Aim of this work is to evaluate the diagnostic accuracy of different modalities of digital radiology, compared with conventional radiology, in the detection of lung nodules. Forty images of an anthropomorphic chest phantom, on which simulated nodules had been previously set, were analyzed by 6 radiologists with 4 different viewing modalities: a) conventional radiography; b) photostimulable plates digital radiography; c) photostimulable plates digital image on dedicated monitor; d) conventional radiography digitized with Film Laser Digitizer and viewed on Display Workstation. For each image the radiologists had to transfer on a sketch the site of the detected nodules giving an opinion on their detectability (uncertain-presumable-probable-certain presence) with a 1 to 4 score. The statistical analysis of the results was made using modified ROC curves. No statistically significant difference was found between the areas under a) and c) ROC curves (p = 0.262). A statistically significant difference resulted between the areas under a) and d) ROC curves (p < 0.05) and, particularly, between the a) and b) areas (p < 0.001), in favor of a).

[The descriptive epidemiology of patients with kidney failure on chronic dialysis treatment. The results of the Lazio Population Registry. The Lazio Dialysis Registry].

The study presents the methodology and selected results of the Lazio Dialysis Registry. The registry, begun in 1994, documents basic demographic data, minimal clinical information and vital status of all end-stage renal disease patients on dialysis treatment in Lazio region. At June 1994 the register included 2834 patients; during the next 12 months, 618 new patients had been notified. In one-year follow-up, among prevalent patients, 308 deaths and 90 renal transplants, and among new patients, 80 deaths and 9 renal transplants were observed. The mean age of the studied population was 59.6 (SD 15.1) for prevalent patients and 61.9 (SD 15.3) for new patients. Among prevalent patients 58.8% were males and 41.2% females, among new patients these figure were 62.1% and 37.9%, respectively. The prevalence rate was 55 per thousand population and the incidence rate was 12.0. The leading cause of renal failure was glomerulonephritis in prevalent patients (27.7%), among new patients the most representative group was nephropathies of unknown origin (27.0%). About 85% were on hemodialysis treatment, and about 4% on peritoneal dialysis. The cumulative survival rates during the one-year follow-up was 81.9%, among patients with age less than 65 years a better survival for males (94.9%) than females (90.1%) was observed. The results of the first year of activity show that a population-based registry of patients on chronic dialysis treatment can be an useful tool for service planning and epidemiological research.
